Application InstructionsEvery Entrance Awards application must include the following information:

� Application Form A – Clearly mark each Entrance Award you have chosen to apply for.

� Application Form B – Complete and sign in ink.

� Personal Statement – Refer to Application Form B, Section 5.

� A copy of your secondary school transcript – You can photocopy the front and back sides of your final secondary school transcript for the purposes of your Entrance Award application.

� A copy of your transcript from every post-secondary institution you have attended. Photocopies and unofficial copies of your transcripts are acceptable. English translations must accompany transcripts that are in another language.

� Award-specific Information – Some awards have additional criteria. Any award-specific information that must be included with your application is listed in the Entrance Award description.

� Application Form C (if required)

� Two Application Form D or confidential reference letters, completed by two different referees, in support of your application (if required). Completed letters may be returned to applicants, or be mailed, emailed or faxed directly to BCIT. Be sure your BCIT ID number and your full name are included.
