• Due close of business 9/29/2012
• Required documents:1. A Final Financial Status Report
2. Property Inventory and Disposition Statement
3. Final Program Progress Report
Submission
• Send one copy electronically to your AIDD Project Officer via email and one original hard copy to your Grants Specialist
• You can find the Project Officer and Grant Management assignment list on URC website
Final Progress Report
• Section 1: Introduction• Section 2: Organizational Profile• Section 3: Evaluation Methodology• Section 4: Final Report on UCEDD
Accomplishments• Section 5: Funds Leveraged Overall• Section 6: Summary of Collaborative Efforts with
the DD Network• Appendix
Process
• Project Officer will review reports October through December
• Project Officer will contact ED with questions and feedback
• Grants Management Specialist will review financial reports and close out grants after receiving approval certification from Project Officer
