COTE reported that her son.

was a resident at,

Lakeview Neurorebabilitation Center, 244 Highwatch Road. Effingham. NH

539-7451. COTE reported that she had removed him from the facility that morning and he is now at Spring Harbor Hospital in Maine. She told me that LIBBEY is a non verbal eleven year old with a bead injury. She said he has scratches on his right side and bruises on his body. She told me that'he had taken photographs of his injuries. COTE told me that LJBBEY was living in Mont Il. She told me that she has been in contact with.

On July 28.2011 at approximately II :44 AM, I called DCYF Central intake and gave a report. They told me the information would be forwarded to Special1nvestigations.

I later learned that the case had been screened out and would not be investigated by DCYF.
