1. อาการโรคสมองเสอม แบงไดเปน 3 ระยะ ระยะแรก/ตน : สมองเรมเสอม ผปวยสามารถด าเนนชวตและปฏบตภาระกจประจ าวน ในครอบครวและสงคมได แตเรมมปญหาเรอง - ความจ า - ความสนใจ - การตดสนใจ / พฤตกรรม - อารมณ อาการเหลานจะมการเปลยนแปลงไปในทางลบมากขนเรอยๆ โดยท ผปวยไมรตว แต ญาต / คนขางเคยงร 6

Definition : NINCDS – ADRDA
A progressive decline of intellectual function in comparison to the patient’s previous
normal level of function.

FIT IN DSM IV CRITERIA
A: Multiple cognitive deficit by both

CBC (ตองด red cell morphology และมองหา hypersegmented neutrophils ดวย)
Serum electrolyte
Glucose
BUN/creatinine
Liver function tests
Thyroid function tests (ขนกบความสามารถของการตรวจทท าได แตถาไปไดควรตรวจหา TSH ดวย)
Screen for syphilis (VDRL หรอ treponent test อนทสามารถท าได)

การตรวจคนเพมเตม หมายเหต Neuroimaging Structure Functional Metabolic
recommendation for CT scan CT, MRI MRI, nuclear SPECT, PET scan MEI, nuclear SPECT, PET scan
Anti HIV เมอมขอสงสยหรอขอบงชทางคลนก Calcium เมอมของบงทางคลนก Serum B12 เมอมขอบงทางคลนก แตเนองจากยงไมมขอมลชดเจนถงความชกของปญหาการขาด
B12 ในประชากรไทย และยงมปญหาการสงตรวจทเชอถอได
EEG EEG จะเปนประโยชนในการชวยวนจฉยภาวะสมองเสอมจากบางโรค เชน Creutzfeld – Jakob disease
LP and CSF examination อาจตองพจารณาท าถาสงสย leptomeningeal metastasis, CNS infection, reaqctive serum syphilis serology, hydrocephalus, rapidly progressive or unusual dementia, immunosuppression, CNS vasculitis
Genetic markers Genetic study : สามารถสงตรวจในตางประเทศและในระดบโรงพยาบาลตตยภม Familial AD : presenilin และ APP mutations Familial prion disease : prion gene mutations Familial FTD : tau gene mutations Huntington’s disease, SCA. DRPLA : CAG repeats testing Mitochondrial disorders : mitochondrial DNA mutation CADASIL : notch 3 mutations

present
Age less than 60 years
Rapid (e.g., over 1 or 2 months) unexplained decline in
cognition or function
“Short” duration of dementia (less than 2 years with
reliable history)
Recent and significant head trauma
Unexplained neurologic symptoms (e.g., new onset of
severe headache or seizures)
History of cancer (especially in sites and types that commonly metastasis to brain)

DEMENTIA WITH LEWY BODIES AND PARKINSON’S DISEASE
Share common pathologies: Lewy bodies.
Similar neuropsychological symptoms and signs: visual hallucination, fluctuating course, parkinsonism, cognitive decline.
Impaired attention, executive function, verbal fluency.
FTD
Gliosis, neuronal loss, and spongiform
degeneration in a superficial area of the frontal
and temporal cortex.
Less than 25% of the patients with this syndrome
had inflated neurons and intraneuronal
argyrophilic, ubiquitin-positive inclusion bodies
called Pick bodies.
NPH
50%: known causes, 50% idiopathic
impairment of periventricular blood flow
Improvement after shunting 30-50% of idiopathic and 50-70% of secondary NPH
Triad: gait difficulty, subcortical cognitive problems, urinary incontinence
