ACADEMIC PLANNING FORM INSTRUCTIONS Form must be typed. Be sure to consult your Degree Progress Report (DPR) associated with your major/minor. Step 1: Find out what degree requirements you need to fulfill while abroad. Also list the courses you plan to take when you return, so that you are aware that you will be on track with your degree progress/course sequencing when you return. Step 2: Identify potential courses at the host institution: course options and syllabi/descriptions are available on most host university websites. If students have trouble finding course information, they should consult with their study abroad advisor. Final registration may not take place until arrival; therefore, it is imperative that students select alternate courses. Seek approval for twice the number of credits required. Step 3: List your desired study abroad courses and alternate choices on this form. Step 4: Schedule an appointment with your adviser(s) to discuss courses you plan to take abroad and your degree plan. Consult the list below for obtaining approval for the following requirements: Major Requirements: Meet with your major/departmental adviser. Minor Requirements: Meet with your minor/departmental adviser. GE Requirements: Consult with an adviser at the department that houses the major associated with the GE coursework (i.e. if it's a history course, consult with the history department). IMPORTANT NOTE: Bring course descriptions and/or syllabi to your appointment if possible. Adviser(s) will write their initials next to each course you have listed on the form. Both you and your adviser(s) should sign the last page. Step 5: Keep a copy for your records, give a copy to your adviser(s), and turn in the completed form to your Study Abroad advisor. WHILE ABROAD: Students must request approval from the relevant academic department and college for any courses not previously approved on the course approval form Ensure that the host university will send an official transcript to the Study Abroad Office at the end of the program. UPON RETURN: The process for courses to appear on a student's transcript may take up to four months from the end of the program. The Study Abroad Office will let students know when transcripts have arrived. The Study Abroad Office will forward the transcript to the Registrars Office. Students will need to submit a syllabus or equivalent to the appropriate college advisor for any courses not previously approved on the course approval form, along with any relevant e-mail correspondence regarding the equivalence that may have occurred since the course approval form was originally completed. Once coursework has been applied to the transcript, students will need to meet with the academic advisor from the appropriate department(s) associated with the course(s) taken abroad to ensure that the course work is applied appropriately to the DPR via the "Request for Course Substitution Form," to be submitted to the Registrars Office. ACADEMIC PLANNING FORMINSTRUCTIONS
