Oviva ‘Diabetes Support’ solution The diabetes challenge in North

West London

• Diabetes prevalence: 7% in Central London to 11% in Brent CCG

• Diverse ethnicity: 53% White, 27% Asian, 10% Black, 5% Mixed

• 41% of type 2 diabetics achieved all 3 treatment targets (NDA 2014-15)

• Structured education attendance was 2% of c. 8,000 newly diagnosed, despite 67% referral rate (NDA 2014-15)

• Being tested in 5 NWL CCGs

• Initial engagement call, personal dietitian assessment, 8 week personal support and an exit call

• 88% uptake and 95% completion rate in 239 patients to date

• Proven clinical outcomes: 7% bodyweight loss and 12 mmol/mol HbA1c reduction at 12 months

• Main benefit: reducing demands on practice nurses

• Challenge: recruitment as GPs unfamiliar with programme
