Accommodation Checklist for MAP & PEAKS Directions: This form is to record accommodations provided during testing. Select the accommodations listed in the student’s IEP/504/ELL Plan, then highlight the accommodations used during each test. If a
checkbox is grayed-out, then the accommodation is not available for that particular test. Upload to Goalview after PEAKS testing.
