A Message from the Leadership of ACHIEVE

Dear Friends,

2014 was one of ACHIEVE’s best years! We successfully completed our Capital Campaign – Growing Spaces, Improving Lives, and finished our renovation of 125 Cutler Pond Road to support our Day Habilitation Program. Everyone has settled in and is enjoying their new program space. We thank all of our donors and the community for their support in making our project a success.

Many of our programs have grown over the past year as you will read in this report. As our regulatory agency, the Office for People with Developmental Disabilities, continues to focus on their Transformation Agenda, we continue to expect changes to traditional service provision. ACHIEVE stands ready to assist with this transition and will insure that the agency remains focused on our mission to continue to be the premier provider of services to individuals in need in our community.

One of the most significant changes we will be facing in the next year is the transition of our employment centers into “integrated” employment opportunities for our workforce. Planning continues with NYSARC and OPWDD to determine exactly what this business model will look like, and we are optimistic that we will meet the requirements of OPWDD and the Centers for Medicare and Medicaid Services. We will also be focusing on the development of more self-directed services for the individuals we serve, to encourage and promote increased independence and integration into our communities. To this end, ACHIEVE is pursuing accreditation through the Council on Quality and Leadership (CQL). For more than 40 years CQL has been a leader in working with human service organizations and systems to continuously define, measure, and improve the quality of life of all people. ACHIEVE’s site visit is scheduled for mid-July 2015.

Our challenges are many, but so are our opportunities; and we remain poised and willing to adapt to change in a positive and productive manner. In the words of John F. Kennedy, “Change is the law of life. And those who look only to the past or present are sure to miss the future.”

ACHIEVE held a Donor Reception and Open House on Friday, May 9th for individuals and businesses who supported the Growing Spaces, Improving Lives Building Rehabilitation Project at ACHIEVE’s Cutler Pond Road Site. Tours were given throughout the evening to showcase the completed renovations of this monumental $3.8 million project.

ACHIEVE’s subsidiary, Country Valley Industries (CVI), was honored as the “Greater Binghamton Chamber of Commerce Business of the Week” November 17 - 21.

2014 Awards and AccoladesFebruaryACHIEVE successfully completed its $3.8 million Growing Spaces, Improving Lives Building Rehabilitation Project at its Cutler Pond Road facility! Throughout the renovation the health and safety concerns that were present were all addressed – asbestos was effectively removed from all parts of the building, and each core room was given direct exterior access in case of an emergency. In addition, program space was expanded and updated to include private bathrooms and program office space in each core room as well as the installation of a ceiling lift track system to assist individuals with ambulation. Security improvements were also made to both the main entrance and the Day Habilitation program entrance which includes cameras and key card access for the security of all building occupants.

SeptemberKathy Muller, CVI Employee, was recognized as a finalist for the 2014 William B. Joslin Outstanding Performance Award. This award recognizes the personal and professional growth achievements of New Yorkers with disabilities.

OctoberACHIEVE was chosen by the Greater Binghamton Chamber of Commerce as the recipient of their Annual Evening of Wine Tasting, Craft Brews, Food & Music event that took place on October 17th. Lou Santoni, President & CEO of the Chamber presented ACHIEVE with a check in the amount of $2,000.

ACHIEVE took second place in the NYSARC, Inc. Choice Awards in the Commercial Category for a :30 spot created by marketing firm, RVSA, that aired numerous times on local television stations throughout 2014. In addition, ACHIEVE also took third place in the Media Awards News Article Category for a piece done by WBNG-TV Action News in March 2014 entitled “ACHIEVE Advocates for More Statewide Support”.

James BattleA Residential Services Success Story

James, who is better known as “Jimmy”, first resided at ACHIEVE’s Broad Street residence during the 1990’s and then moved to ACHIEVE’s Carlton Street residence in 2002. During his time at Carlton Street Jimmy worked on his independent living skills and then realized his dream to move to a Supportive Apartment in January 2015. Jimmy is a gentleman by nature. He is kind, considerate, and always willing to help when needed. He has been a mentor for other individuals and is a leader among his peer group. He can be quiet, but then comeback with a great sense of humor. He is a reliable friend and always has your back. Jimmy is athletic and enjoys all kinds of sports. He has participated in many Special Olympics events and has gone to State Competitions for basketball. He also enjoys power lifting, bowling, volleyball, swimming, bocce, and golf. He will try anything once. He enjoys bike riding and has participated in an event to learn the history of Binghamton while riding throughout the community.Jimmy has traveled with People and Places and loves to travel with

his peers on trips arranged by his program. One of his favorite trips was to a Syracuse versus North Carolina basketball game. He didn’t know which jersey to wear as he enjoys both teams. Jimmy has worked at Chat A Wyle restaurant in Apalachin for 9 years as a dishwasher. He has a strong work ethic and takes pride in his work. He has developed positive relationships with his coworkers and boss. You never know when he will walk through the door with a bouquet of flowers to brighten someone’s day. Jimmy has proven to be an excellent choice for the Supportive Apartment Program. He is a self starter who handles all of his responsibilities with very little assistance from staff. He keeps a clean and tidy apartment and enjoys having his friends over for dinner. He is developing friendships with other people who live in the building and is enjoying his new found freedom!Jimmy has been triumphant throughout the years. When told by others that he couldn’t do something, he would work hard to prove them wrong, time and time again. Throughout it all, Jimmy is always respectful of decisions made and marches on with his head held high.

Devon WoolfolkA Supported Employment Program Success Story

Devon entered ACHIEVE’s Supported Employment Program in 2011. At that time, Devon did not have a lot of employment experience, but had a drive to work. He also cared deeply for his family, wanting to be a role model for his younger siblings and a source of support for his mom. With the help of his job coach, Devon found his first employment experience as a maintenance associate at Walmart, where his caring and friendly personality and willingness to take on tasks outside of his job description made him a valuable employee. However, no road is without its bumps, and Devon struggled to balance his work and family responsibilities. He eventually resigned from his position at Walmart to spend more time helping his family. After leaving Walmart, Devon held part time positions as both a lobby attendant and as a dietary aide at an assisted living facility. At each, Devon got to work independently using the BC Transit bus, did any task asked of him, and was friendly and well liked – especially by the residents of the assisted living facility, who appreciated his kindness and caring, individual attention. However, he continued to struggle to maintain his attendance at work and be there to help care for his siblings.In December of 2013, Devon and his job coach attended a job fair for Sodexo at Binghamton University. Sodexo was opening a new food court and Devon was among those hired to be the first crew of the brand new operation. He started the next month as a food preparer and line server for their salad bar, “Garden Toss”. During that time, Devon learned many things about food preparation. Not only did he learn to take and record food temperatures, but he learned how to cut, chop, slice, and dice all the vegetables and toppings in preparation for the day. He grilled chicken and tofu and built salads to customers’ orders. Managers quickly realized his talent and Devon became the main opener of his work station. With a little help from his job coach, he learned to manage his time and streamline his tasks so that the station opened on time.Devon has maintained this job for nearly a year and a half. He was even asked to work over the winter session, an opportunity that he gladly took! He found that, while he worked well as a line server, he really excelled at and enjoyed prep work. Devon put in for a position in the kitchen as a food preparer and started his new job this past January. The hours were right, too. Devon works while his siblings are in school, gets home around the same time that they step off the school bus, and is off during the summer months when they are home from summer vacation. Devon now maintains a healthy balance of work and family responsibilities, has excellent attendance, and is a valuable member of the Sodexo team. He and his family have a lot to be proud of.

Steven ParkerA Day Habilitation Program Success Story

Steven has been receiving services from ACHIEVE since July 2007. When he first began attending ACHIEVE’s Day Habilitation Program he was quiet and kept to himself. With encouraging words he would sometimes try an activity or take part on an outing. Steven has undergone an amazing self transformation over the past year.Steven is now communicating through words and his vocabulary has vastly expanded. While once unable, he now puts words together to communicate his wants and needs. Not only is Steven verbally communicating, he is also much more involved with activities in the core room. He volunteers to return items to the cafeteria, sweep the floor, and run errands around the building. Steven has also overcome his fear of dogs. Before, he would briefly look at Cheerio, the dalmation who visits weekly for pet therapy, and back away. Now, Steven independently approaches Cheerio as well as other dogs that come to visit individuals in the Day Habilitation Program. He pets the dogs, and even allows for quick puppy kisses. Steven doesn’t hesitate to reach out if he feels that someone is in need

of help or assistance. On the bus, if a peer drops an item, he brings it to the attention of the bus aide. In the core room, if a peer needs help tying their shoe, he will let the staff know. This is a huge accomplishment, as Steven who didn’t speak when he first joined the program will now tell staff, “uh oh, tie shoe”.One of Steven’s favorite things to do is to take a walk through the building with a hand puppet and visit his peers in the other core rooms. Steven enjoys making other people smile and laugh, and tries to get them to dance with him or gives them a friendly “boop” on the nose with the puppet.Steven has come so far in such a short amount of time, and continues to improve and succeed.

Vocational ServicesCountry Valley IndustriesCountry Valley Industries (CVI), a subsidiary of ACHIEVE, serves as the core facility-based vocational program. CVI provides a variety of training and job opportunities through subcontracts with business and industry. CVI also provides work experiences at local community businesses as a means to foster education, awareness, and community integration. The program is certified by the New York State Office for People with Developmental Disabilities (OPWDD), State and County Social Services Departments, and the United States Department of Labor.v 368 individuals served in 2012v 354 individuals served in 2013v 328 individuals served in 2014

Pre-Vocational ServicesACHIEVE pre-vocational services are meant to understand peoples’ interests, help them discover their capabilities, and merge the two into fulfilling lifelong careers. ACHIEVE offers Pre-Vocational Services at each of four Country Valley Industries facilities. Pre-Vocational Services are designed to teach responsibility, teamwork, initiative, and problem-solving skills necessary to succeed in the workplace.v 257 individuals served in 2012v 268 individuals served in 2013v 261 individuals served in 2014

Pathway to EmploymentPathway to Employment is a new twelve month pre-employment service implemented by OPWDD designed for individuals who are interested in obtaining competitive employment and who are enrolled in the Home and Community Based Services (HCBS) waiver. Pathway to Employment provides comprehensive career and vocational employment planning and support services to assist individuals in obtaining, maintaining, or advancing in competitive community employment.v NEW Program in 2014 – 11 individuals served

Supported EmploymentThe Supported Employment Program provides services for individuals looking for community employment and for those already employed at local community businesses. Services include seeking out job opportunities, providing assistance with interviews and on-the-job training, as well as long-term follow along supports. The Supported Employment Program is certified and fiscally supported by ACCES-VR and OPWDD.v 217 individuals served in 2012v 467 individuals served in 2013v 500 individuals served in 2014

Community Residences, Supervised Apartments, and Supportive ApartmentsEach of ACHIEVE’s 13 residential facilities are regulated by the New York State Office for People with Developmental Disabilities (OPWDD). They are designed to support individuals with intellectual and other developmental disabilities in their community with access to recreational and work opportunities. Admission requires a developmental disability diagnosis before the age of 22, with the individual requiring a supervision level other than one-on-one support. v 115 individuals served in 2012v 116 individuals served in 2013v 117 individuals served in 2014

Respite Program**Program Closed in September 2014**The Residential Services Program operated two overnight respite beds that could be accessed by families when they needed temporary and overnight care of a family member with a disability. These temporary residential placements allowed the individual to stay overnight at an ACHIEVE residence, socialize with peers, and learn independent living skills.v 24 individuals served in 2012v 20 individuals served in 2013v 25 individuals served in 2014

Day Habilitation ProgramThe Day Habilitation Program provides a comprehensive spectrum of intense rehabilitative, clinical, and support services in a medical model to individuals with moderate to severe levels of disability whose skill deficits prevent participation in vocational programming. Services include: teaching activities of daily living, cognitive skill development, recreation, communication development, occupational, physical, and speech therapy, and comprehensive nursing services.Day Habilitation Services are offered to residents living in Broome and Tioga Counties who have been diagnosed with a developmental disability or another disability prior to their 22nd birthday. Services are individualized and assist in developing, enhancing, and maintaining skills used while living, working, or socializing in the community. Individuals in the Day Habilitation Program are also given volunteer opportunities in the community to help increase self-esteem, develop individuality, and promote independence. v 123 individuals served in 2012v 137 individuals served in 2013v 149 individuals served in 2014

Family Support Services offers an array of programs and services that provide assistance to families who have an individual with intellectual or other developmental disabilities living at home.

Family Empowerment ProgramThe Family Empowerment Program provides financial reimbursement for short-term respite services to families enrolled in the program. Families are responsible for finding, training, and supervising their own providers. An annual allotment of respite hours is provided at intake, with a set reimbursement rate established by ACHIEVE.v 109 individuals served in 2012v 131 individuals served in 2013v 145 individuals served in 2014

Community Habilitation ProgramCommunity Habilitation is a Medicaid waiver service that assists individuals who either live at home with a loved one or independently in the community to maintain or develop the highest level of independence possible for them. Individuals work on goals to build independent living skills in the following areas: personal hygiene, self care (eating, grooming, etc.), general household chores, mobility training, personal health care, financial management, appropriate social skills, and life safety. Weekly schedules are established by the individual and ACHIEVE staff.In addition, ACHIEVE also offers Community Habilitation with a Self-Directed option. Selecting a Self-Directed option allows the individual and his/her family to select their own provider with the option to choose someone who is currently involved in their life.v 43 individuals served in 2012v 55 individuals served in 2013v 61 individuals served in 2014

In-Home Respite Program**Program Closed in September 2014**The In-Home Respite Program offered families a brief respite by providing a care provider identified and provided by ACHIEVE. The care provider spent a predetermined number of hours with the individual and engaged in activities in or out of the home. Respite was short-term, temporary care of family members with a disability that provided families with some relief from the demands of routine care of an individual with a disability.v 28 individuals served in 2012v 25 individuals served in 2013v 25 individuals served in 2014

Recreation ProgramThe Recreation Program provides organized and structured recreational activities and social opportunities for adults and teens living at home with their families while, at the same time, providing a brief period of respite to family members. These programs are available on Friday evenings and Saturdays at ACHIEVE’s Cutler Pond Road facility in Binghamton as well as on Friday evenings at ACHIEVE’s Tioga facility in Owego.v 121 individuals enrolled in 2012v 140 individuals enrolled in 2013v 94 individuals served in 2014

Party NightACHIEVE’s Party Night is an opportunity for adults with intellectual and other developmental disabilities to socialize with their peers. Community Behavior Supports ProgramThe Community Behavior Supports Program provides ongoing behavioral supports to individuals who live in the community and receive services through the Community Habilitation and Waiver Respite Programs. Medicaid Service Coordination Medicaid Service Coordination (MSC) is a program that provides ongoing support to individuals with developmental disabilities and their families. Medicaid Service Coordinators assist people with obtaining available services in the community as well as services under the Home and Community Based Services (HCBS) Waiver. ACHIEVE’s Medicaid Service Coordination program serves adults and children of all ages both in Broome and Tioga counties. Service Coordinators work together with families to develop and maintain a personalized plan designed to meet their loved one’s individual needs and unique goals. v 264 individuals served in 2012v 301 individuals served in 2013v 341 individuals served in 2014

ACHIEVE Children’s Summer ProgramChildren with intellectual and other developmental disabilities benefit from summer programs that help them maintain the skills they developed during the school year. The Children’s Summer Program exposes them to drama, music, physical activity, arts and crafts, and field trips that reinforce their academic skills, gross and fine motor coordination, and social interaction. The goal of the program is to broaden the life experiences of children with disabilities and provide them with basic learning concepts that will enhance skill levels. Students are grouped by age and led by a team leader and counselors who design activities to meet each group’s needs.v 100 individuals served in 2012v 100 individuals served in 2013v 101 individuals served in 2014

Medicaid Service Coordination

Guardianship Servicesv In 2012 ACHIEVE served as Primary Guardian for ten

individuals, and Stand-by Guardian for seven individuals. v In 2013 ACHIEVE served as Primary Guardian for

eleven individuals, and Stand-by Guardian for seven individuals.

v In 2014 ACHIEVE served as Primary Guardian for fourteen individuals, Stand-by-Guardian for three individuals, and Alternate Stand-by Guardian for one individual.

Guardianship ProgramThe Guardianship Program is a service for individuals who require assistance in making decisions or managing their affairs when other family members are not able or willing to do so. Through NYSARC, Inc.’s Corporate Guardianship Program, ACHIEVE may serve as guardian, stand-by guardian, or alternate stand-by guardian to ensure that an individual has assistance with making critical decisions in areas related to healthcare, housing, day programs, or recreation. The Guardianship Program also provides information, referrals, and technical assistance to families who are interested in establishing legal guardianship.
