4. W-9 tax form dated in the current year.

The prequalification needs to be renewed every year. Your firm needs to be prequalified prior to submitting for any projects (as a prime or sub) and before our audit department will proceed with its financial review.

Please keep track of your firm’s expiration date to avoid a lapse in status and submit your request at least a few weeks before this date.

Notice of renewal will be e-mailed and the original signed prequalification letter will be mailed.

Uploading in the Portal: You’ll need an account first. If you need to create one, contact [email protected] or Louis Wynn at [email protected] and ask for help getting set up.

Once you have an account, go to https://www.codot.gov/business/consultants and press the ‘click here’ link towards the bottom of the screen:

Enter your User ID and Password to login:

Press the RFx and Auctions tab on the upper left corner:

Open the box called Show Quick Criteria Maintenance:

Enter 30041 into the box called Event Number and click Apply. This will bring up the Architect-Engineer Prequalification:

Click on the highlighted 30041 number to go into the prequalification. In the screen that comes up, press Create Response (if there's a Register button press that first) on the top row of buttons:

Under the RFx Information tab, go to Questions and complete them. Red indicators will alert you to mandatory fields:

In the Notes and Attachments tab, click Add Attachment to upload the prequalification form and W-9 dated in 2016 from wherever these documents are on your desktop.

When you complete the questions and add your two documents, press the Check button. The red indicators

should be gone and you’ll see a green one saying the RFx is complete.

Finally, press the Submit and then Close (don’t press the red X button in the upper right corner).

-If your firm is considered a vendor, it’s not suited for the architecture/engineering prequalification and won’t need a Master Pricing Agreement.

-Other prequalifications:

1. Procurement (under ‘RFx 50040, Procurement Registration 2016’ in the portal). Most vendors should go through this prequalification process. More information is found here: https://www.codot.gov/business/procurement-and-contract-services.

2. Contractor- for highway and bridge construction firms (this prequalification is not in the portal yet). Bids, contacts, and other information are here: https://www.codot.gov/business/bidding.

-Use the Log Off button whenever you’re done with the portal (pressing X closes the window but leaves your session open, so you could have problems logging in again as a session would still be active).

-When you log back in the portal, remember to click on RFx and Auctions in the upper left corner, open the box called Show Quick Criteria Maintenace, and enter 30041 in the Event Number box to bring up the Architect- Engineer Prequalification.

-How to see whether the prequalification has been submitted:

A Response Number is generated for your firm’s prequalification. The Response Status shows whether your prequalification has been saved, submitted, accepted, or rejected. It needs to show Submitted for the prequalification to come through the system and be processed. If it is in Saved status, press the Response Number to go back into your prequalification.

Press the Edit button, Check (red indicators will alert you to anything still needed), Submit, and then Close.

-Fill out the prequalification form as completely and accurately as you can (typed or hand-written is fine). A more thorough examination of your firm’s qualifications will take place as part of the selection process when a statement of interest is submitted.

–The Submission Deadline of 11/30/2017 is the expiration date of this particular RFx in the system. It’s not related to your firm’s prequalification expiration date.
