DESCRIPTION

Activ8 is the award winning CSU camp program offered by Continuing Education, and it is sure to please kids from ages 4 to 14. Kids come from near and far to build their own schedules and set out for a summer of fun. The Activ8 program is broken down into camps for three different age groups: Activ8 Academy for ages 4-7, Activ8ers for ages 8-11, and BeTween 2 Teen for ages 11 & up. Activ8 is a program of enrichment and sports camps created just for kids. These camps create fun, hands-on learning without the pressure of homework, tests, or quizzes.

This will be a fun-filled summer with over 180 offerings within 7 categories of interest; there is truly something for everyone! Also, we are thrilled to present over 100 new classes this year! We look forward to seeing your child for a summer of exploration and fun-filled, hands-on activities. Please join us and have fun at Activ8!

Full of OptionsActiv8 is a program of enrichment and sports camps created for kids that allows them to make their own schedule. Our camp consists of fun, hands-on learning without the pressure of homework, tests, or quizzes. Camps fall in a variety of categories:

Brain Training: The most educational aspect of our program! There are no textbooks, so you get to participate in hands-on experiments and other creative projects. You’ll have so much fun; you will forget that you are learning.

Create-A-Lot: You have never seen this many arts and crafts options in one place.

iTech: Build computer skills and even learn how to design your own game.

Just For U: Camps designed especially for you to make your life better, now and later.

Scienceologies: Since there are no textbooks, you get to participate in hands-on, in-and-around experiments and other fun projects.

Showbiz: Singing, dancing, acting, modeling or performing, you will leave camp with a new sense of self confidence and enthusiasm for the skill that you learned in class. From the stage to the circus arena; if it keeps you moving, you can find it here.

Sports & Dance: Multi-sports and a variety of dance camps will keep you active all summer.

Built for You Activ8 Academy: Our special program for campers ages 4-7. Every week you will dive into a variety of arts, crafts, games, and songs designed around a central theme. You’ll also get a chance to “show your stuff”!

Activ8ers: From brain training to sports and dance, our 8 to 11 year old campers can choose a camp to fit their active and creative lifestyle.

BeTween 2 Teen: Our special program for campers 11 and up. Camps are designed with you in mind. You’re not a kid anymore, so it’s time to take it up a notch! From just-for-fun to functional, we have you covered. Here you can learn to improve your artwork, your grades, your life, or even, just your wardrobe.

Summer StaffThe people who work for Activ8 are our greatest asset. All of our Activ8 instructors are certified teachers or professionals in their field and are passionate about the programs they are teaching. Their enthusiasm and devotion to our youth will make this a fun and rewarding experience for your child. Our program aides assist the instructor in the camp and provide supervision throughout the day. Activ8 instructors and aides are carefully screened and trained.

Activ8 – Before and After Caref Offered to meet your family’s needs!

f Constant staff supervision and escort to and from camps.

f Offered Monday-Friday of each camp week.

f If you will be dropping your child off before 8:45am or picking up after 4:15pm, your child must be registered for Before or After Care.

f BEFORE CARE – 7 to 9am

fAFTER CARE – 4 to 6pm

f Child can be dropped off or picked up anytime during these hours.

f Drop off and Pick up only in designated areas (see page 22).

f Students who are not promptly picked up at the end of After Care will be charged $5.00 for each 5 minutes of lateness.

f FEES are $20.00 per week for a 5 day week and $15.00 per week for a 4 day week (May 31-June 3 and July 5-8).

LunchBring your own lunch! Activ8 offers lunch supervision for a fee of $5 per week. Supervision during the weeks of June 1-4 and July 6-9 will cost $4. We kindly request that lunches not include energy drinks. Please DO NOT BRING NUT PRODUCTS out of concern for other campers with nut allergies.

Pirate PartyChart a course to adventure, as we kick off the summer with a pirate party! Create a beard, hat, hook and parrot to transform yourself into a pirate. Learn to talk like a pirate, make a spy glass and design a ship of your very own. And of course, you can’t play pirates without a treasure chest and map. The time will sail away with plenty of music, dancing and games!

Seuss CityInside Dr. Suess’s noodle, there is a lot to see and doodle. Play with words and counting toodle. Make a rhyme or use your time to dance and sing. You’ll do a bit of everything. Like playing games where you toss a ball inside a striped hat, but please don’t hit our friend, the Cat.

Ride the WindTake flight as we take a crash course in rockets and airplanes! You will learn what makes them fly, build a variety of flying machines, test them, race them, and design one of your own. You’ll also make a pair of goggles and a pilot’s scarf so you feel right at home in the cockpit!

Building An EmpireCompare the ancient civilizations of Egypt, Greece and Rome to discover what it takes to create a powerful

empire. Create a Grecian clay urn, design and build gladiator weapons, and dance like an Egyptian. You will go on an archeological dig to look for artifacts and use what you learn about empires to build your own!

Myths and LegendsTravel back in time to learn about the ancient myths and legends of the world. Discover what it takes to be a young hero or heroine as you go through a series of trials, tests and contests of mind and body!

Holiday MagicRelive your favorite holidays in our week long celebration! Help make some traditional snacks, create decorations, play games and learn new songs for Christmas, St. Valentine’s Day, Halloween, and July 4th so you can get the most out of your holiday vacation fun.

Weather WizardsThe weather person on TV is not just pointing to a map. They have learned all about the weather and now it is your turn! Fun projects, activities about different types of weather and learnng to read the skies will give you the ability to predict the weather, too.

Activ8 AcademyActiv8 Academy offers a special program for future Activ8ers who are 4-7 years old. All of these camps will be held at the Elizabeth Bradley Turner Center (EBTC). Every week you will dive into a variety of arts, crafts, games, and songs designed around a central theme. All participates should bring their own lunch and supervision is included. Drop off and pick up will be at the EBTC. Before and After Care will be available for an additional fee. All 4 year olds must turn 5 before December 31, 2011 to participate.

SPRING BREAK Soccer Camp 2011The Spring Break Soccer Camp is designed to give extra training to both boys and girls who want to improve on the fundamentals of soccer. Skills will be taught according to age and ability with a small sided approach. Each session will focus on various areas of the game including dribbling, finishing, passing, receiving, possession, and goalkeeping. Classes will run every day during the Muscogee County Spring Break. All campers are required to bring their own soccer ball, water bottle, and a snack. Soccer CampsThe CSU Soccer Camps will focus on making the game of soccer easy and fun. Learn the skills of dribbling, passing, kicking, and making goals. High School Elite Soccer CampElite soccer camp is for campers who would like more of an opportunity to grow as a player. Camp will focus on helping players speed of play and decision making capabilities. A recruiting seminar will be offered during this camp to help campers better understand the recruiting process. Camp begins on Sunday with check-in at the dormitories from 2-3pm followed by a meeting to begin camp at 4pm at the Mock Pavillion.

CSU Speed and AgilityAn elite sports specific speed, agility and quickness program designed to improve players’ athletic ability on the playing field. The training will develop the proper form and technique in motor ability in order to increase players’ success as athletes. The speed and agility program is devised to jumpstart players’ training and show them the exercises necessary to be able to improve in the following areas: Muscle Strength, Speed, Flexibility, Changing Direction, Power when Running and Jumping, Coordination, and Acceleration. All athletes should bring the following items to camp: Sport required footwear, tennis shoes, and a water bottle. Campers are encouraged to bring plenty of water.

FUN-Damentals of Golf CampAre you going to be a champion golfer one day? Then FUN-damentals of Golf is a summer camp for you. Come to the CSU golf camps at the Mark Immelman Golf Teaching Center located at the Bull Creek Golf Course. Gain a deeper understanding

of golf, and build confidence in your ability while having a fun-filled time with other campers. During the camp you will learn and practice driving, iron play, chipping, bunker play, and putting as well as learning the rules and correct etiquette. Light snacks and refreshments will be provided. Bring your own lunch. All campers are required to wear appropriate clothes and golf or tennis shoes. Note: to be a champion golfer you also have to have your own golf equipment! Tennis CampThe CSU Tennis Camps explore the game of tennis and the fundamentals needed to be a successful player whether you are a beginner or advanced player. 38th Cheerleading CampThe 2011 Cheerleading Camp is the 38th Annual and is the longest running camp on the campus of CSU. The CSU Cheerleading program is one of the best Division II programs. The CSU Cheerleaders have won several Peach Belt Titles, are nationally ranked and are the 2004, 2005 & 2008 UCA National Champions. The 38th Annual Columbus State University Cheerleading Camp will feature cheers, chants, jumps, dances and a lot of fun. The week will end on Friday morning with a Pep Rally for family and

Activ8 FAQ’sPLACEAll Activ8 camps are held on Columbus State University’s main campus (does not include ballet, math collaborative, athletics and CCSSC):

fActiv8 Academy is housed solely in the Elizabeth Bradley Turner Center.

fActiv8ers and BeTween 2 Teen camps will be in the Elizabeth Bradley Turner Center, CSU Recreation Center (Sports), and the Center for Commerce and Technology (iTech).

fAll Activ8 campers must be dropped off and picked up at the Elizabeth Bradley Turner Center. They will be escorted by our staff to other locations as needed.

Drop-off & Pick-upThere is only one designated drop-off and pick-up area for all children involved in the Activ8 program: the Elizabeth Bradley Turner Center. This will be the only supervised area on campus to drop-off and pick-up your child. Children are not allowed to be on campus without supervision before 8:45am or after 4:15pm. If you cannot drop-off after 8:45am or pick up before 4:15pm, you must sign your child up for Before and/or After Care.

We will be enforcing a sign-in/out policy for campers. Parking is only allowed in the parking lot of the Elizabeth Bradley Turner Center. Curb parking is prohibited.

AGE GUIDELINESfSeven year old students who have completed the 2nd grade may

attend the Activ8 Academy or Activ8ers.

fTen year old students who have completed the 5th grade may attend the Activ8ers or BeTween 2 Teen camps.

ORIENTATIONWe will be holding orientation for participating families to see how Activ8 camps work. Orientation will last approximately 45-minutes and will be held in the Elizabeth Bradley Turner Center. Below are the dates and times. You do not have to reserve a spot, so just come and be sure to bring your children!fSaturday, May 14 – 11amfSaturday, May 21 – 11am

SAFETYSafety is our number one concern for your children. All of our employees have passed criminal background checks. At no point in our schedule will your child be left unsupervised. Our program aides go through comprehensive training and will be on campus before, between, and just after classes begin to help direct children to their camps. To accommodate those with allergies, NO NUTS please.

SUPPLIESAll supplies are included with the cost of Activ8 camps unless specified otherwise.

WAIT LISTSWhen our classes fill up, we will put you on our waiting list. If there is an opening, you will be notified. Avoid missing a camp by registering early!

WAIVERAll participants must have a fully completed and signed waiver before they can join the camp. Please make sure that we have all waivers prior to the start of the program. If we do not have your child’s waiver, your child cannot participate.

ABSENCESIf your child is going to be absent or late, please call the Activ8 office at 706.507.8282 to let us know. We need to know where your child is at all times. Activ8 aides take roll at the beginning of each camp. If your child is absent, we will call you to verify their absence.

SPECIAL NEEDSWe welcome all children to our program and make our best effort to accommodate everyone. If your child has special needs, please let us know one week prior to your child attending camp.

HEALTH NEEDSWhen filling out your child’s waiver, please make sure to include all medical information that would be relevant and important for our staff to know. If there are special health needs, please contact our Activ8 office at 706.507.8282.

BAD WEATHERIn case of bad weather (i.e., heavy rain, lightning, or tornadoes), we will keep all children indoors until the weather has passed and the staff has been authorized to go back outside. For the safety of your child, we ask that you allow bad weather to pass before coming to find your child. Our staff will make sure all children get to their destinations once authorization has been received. Please send an umbrella or poncho with your child if the weather requires it.

REFUNDS/CANCELLATIONSIf you choose to withdraw from a camp, you must contact us before 5pm, on the Wednesday prior to the start of the camp, to get a full refund minus a $15 processing fee. If you withdraw after Wednesday, but before the camp begins, you will receive a 50% refund. No refunds are given after a camp begins. We reserve the right to cancel a camp and will notify you before the start date to issue a refund or transfer your child to another camp.

DISCOUNTsfEARLY BIRD DISCOUNT A 5% discount is available to anyone who registers for any summer camp by

April 15, 2010. fFULL SUMMER REGISTRATION DISCOUNT Activ8 is offering a 5% discount to anyone who registers for the entire summer. In

order to be eligible for the discount, campers must be enrolled in a morning camp, lunch, and an afternoon camp for each of the ten weeks. Camps fill early so be sure to sign up now. The Full Summer Discount may be combined with the Early Bird Discount. Please contact the Continuing Education Registration Desk for more information on how to obtain this discount.

* Camps may have material fees included in the class cost. Discounts are not applied to material fees.

This is a legally binding Consent Form and Release of Liability made voluntarily

by me, the undersigned Releasor, on my own behalf, and on the behalf of my

heirs, executors, administrators, legal representatives and assigns to the Board

of Regents of the University System of Georgia and its institutions, Columbus

State University (hereinafter “CSU”).

The undersigned hereby acknowledges that participation in the above named

activity involves inherent risk of physical injury and assumes all such risks. The

undersigned hereby agrees that for the sole consideration of CSU allowing the

undersigned to participate in the above named activity for which or in con-

nection with which the university has made available any equipment, facilities,

grounds or personnel for such programs or activities, the undersigned does

hereby release and forever discharge CSU and the Board of Regents of the

University System of Georgia, its members individually, and its officers, agents

and employees of any and from all claims, demands, rights and causes of action

of whatever kind of nature, arising from and by reason of any and all known

al1 unknown, foreseen and unforeseen bodily and personal injuries, damage to

property, and the consequence thereof, resulting from my participation in or in

any way connected with the above named activity.

In an emergency, I acknowledge that I am solely responsible for all medical and

other costs arising out of bodily injury or any loss sustained through participa-

tion in this activity. I authorize program staff to secure any licensed hospital,

physician and/or medical personnel any treatment deemed necessary for the

participant’s immediate care.

By the execution of this agreement, I accept and assume full responsibility for any

and all injuries, damages, and losses of any type, which may occur to me, and I

hereby fully and forever release and discharge CSU, its officers, employees, and

insurers including any self-insurance funds of the State from any and all claims,

demands, damages, rights of action, present and future, whether the same be

known or unknown, anticipated or unanticipated, resulting from or arising out of

my participation in this class.

I understand that the acceptance of this release and waiver of liability by the

Board of Regents of the University System of Georgia shall not constitute a

waiver in whole or in part of sovereign immunity by said Board, its members,

officers, agents, and employees.

I have read the above carefully before signing. Further, I understand that this

release and waiver of liability shall be effective for a period of time for the

dates listed above.

STUDENT CODE OF CONDUCTDisciplinary action may be imposed whenever a student commits or attempts

to commit any act of misconduct on the CSU Campus, or at any activity, func-

tion, or event sponsored or supervised by CSU, including but not limited to:

1. Possession, use or distribution of an illegal or controlled substance, or

look-alike drug.

2. Unauthorized and/or illegal possession, use or distribution of any alco-

holic beverage.

3. Theft of property or services.

4. Intentional or willful and wanton destruction of property

5. Assault and/or battery.

6. Possession of a weapon.

7. Conduct which constitutes harassment or abuse that threatens the

mental well-being health or safety of any individual.

8. Do not bring laptops, MP3 players, iPods, CD’s/DVD’s, or any electronic

devices to camp. Cell phones may not be used during camp time; they

must be turned off.

Consequences include, but are not limited to a conference with the student,

conference with parent/guardian, and removal from the program (without

refund) for the safety and wellbeing of other campers.

*Disciplinary action may also be imposed whenever a student commits any

acts of misconduct during an off-site event or activity.
