ACUTE CARE TRAINING PROGRAM REQUIREMENTS
1. Valid California ID / Driver’s License / Passport (copy)
2. Social Security Card (copy)
3. Physical Examination(with Doctor’s stamp) –within one(1) year upon enrollment a. 2 steps PPD or skin test for Tuberculosis- within six (6) months upon enrollment b. Chest X-ray (if PPD or skin Test is Positive (+)) - a copy of the result of X-ray at least one (1) year upon enrollment. c. Immunizations :
i. MMR - must provide proof of vaccination or positive titer results(if titer result is equivocal or negative a new booster shot is required)
ii. Varicella (waiver/declination applicable) - must provide proof of vaccination or positive titer result (if titer result is equivocal or negative a new booster shot is required)
iii. Hepatitis B (waiver/declination applicable) - must provide proof of vaccination or positive titer results iv. Tetanus /Diphtheria (TDAP) - Booster is required every 10 years. v. Influenza(waiver/declination applicable) (Seasonal)
4. CPR Card @ (Providence Vocational School $80.00 with book)
5. C.N.A License or Congratulations Certificate (from American Red Cross)
6. 10- Panel Urine Drug Test @ (Providence Vocational School $ 55.00)
7. 7 Year Criminal Background check @ ( Providence Vocational School $ 50.00) All requirements must be submitted before clinical begins. (No requirements No clinical)
