DVC/SRC High School Application/Registration Information

Welcome to DVC San Ramon Campus! Your registration is not complete until all forms have been processed by the DVC/SRC Admissions

Office. If you are not enrolled, you will not receive credit.

Always bring a photo ID (HS ID, CA ID, Driver’s License, or passport) with you to the Admissions Office.

High school students may not register for classes online. Registration must be completed in-person.

11th & 12th GRADE 1. Apply Online to DVC – If it has been over a year since you last attended, you must submit a new application. 2. Clear Course Prerequisites – If a course has a prerequisite, a Prerequisite Equivalency Form and transcript

must be submitted to Admissions and Records at least 3 business days prior to registration. 3. Register for the Course In-Person starting on January 15th – Check the DVC schedule for the High School

Registration Day for the term you are registering for. Special Admission Recommendation forms are only accepted when a student is officially enrolled or waitlisted into a class. Instructor signature is required for all PE courses regardless of the student’s high school grade level.

10th GRADE & BELOW 1. Apply Online to DVC – If it has been over a year since you last attended, you must submit a new application. 2. Clear Course Prerequisites – If a course has a prerequisite, a Prerequisite Equivalency Form and transcript

must be submitted to Admissions and Records at least 3 business days prior to registration. 3. First Day of Instruction – Register for the Course In-Person – 10th graders and below must go on the first

day of class and get instructor approval to register for classes. Special Admission Recommendation forms are only accepted when a student is officially enrolled or waitlisted into a class.

All Students – Courses with Prerequisites Official or unofficial transcripts with proof of completion of the prerequisite course must be turned in to the Admissions Office, along with a completed Prerequisite Equivalency Form, at least 3 business days prior to enrollment. We will not register you until your Prerequisite has been cleared. Some prerequisites may also be satisfied by taking the assessment test. To enroll in MATH-120 or below, the online Informed Student Self-Assessment (ISSA) may be taken to clear those prerequisites. Please check the DVC catalog or schedule of classes for prerequisite requirements.

Transcript Request In order for your grades to be sent to your high school, please submit a transcript request online. Transcripts are sent only upon the student’s request. You may submit the online request after final grades are posted. The first two standard service process copies are free – charges will apply after that.

How Do I Apply Online?

1. Go to www.dvc.edu/apply2. Pick an option that best suits your

status. Please note—this guide is notintended to be used for InternationalStudents or Community Education.

3. Review the information on the nextpage—this provides you with anoverview of the enrollment process.Click the “+” symbol to expand “Stepone: apply” and click on the link for“CCCApply”.

4. Click “Create an Account”. If you alreadyhave an account, please sign in andskip to Step 7.

5. Complete the steps to create aCCCApply account*.

*Note regarding Social Security NumberYou are unable to receive Federal Financial Aid or important tax forms without your Social Security Number (SSN) onfile with DVC. If you do not have it when completing your application, you can: submit your application and visit the Admissions Office at a later time with your physical Social Security Card and

photo ID to add it to your accountor

8. The first page contains information on navigating the application. Click “Start Application” to continue.

9. On the “Enrollment Information” screen, you will be asked to select a term to apply for, an educational goal, an intended major. Your major can be changed afterwards at DVC, so do not worry if you feel unsure about your choice!

10. Continue to make your way through the application. Check your progress on the left hand side of the screen. Please note that you do not need to provide *exact* dates, for your educational history or military service, but you should still enter your best guess.

11. When you reach the “Review Application” screen, look through and confirm the details you have entered are correct.

12. Submit your application. Once submitted, an application can take up to 24 hours to process. Look out for an email that will contain your 7 digit DVC Student ID number, and your DVC username.

Assessment test and ISSA results may also clear some prerequisites. Students receiving permission to register for waitlisted courses are required to return to Admissions to register in person.

You must make an appointment to take the test.

In order to complete the assessment test you must have the following:

A DVC application must be on file with the Admissions Office and Records Office

DVC ID number & photo id are required to take the test

Our retest policy is as follows:

Effective June 1, 2010 we will only be testing students who are enrolling at DVC in either the present or future semester. We will not test students who are planning on attending another school. Accordingly, we will not send test scores to other schools. If you have previously taken an assessment test at DVC and would like to use your scores at another school you will need to print your Test Summary from the WebAdvisor system to send to the school. An interpretation of these scores are available on our Test Score Results page.

NEW INFO ABOUT THE MATH TEST The math assessment test is designed to determine your math

skills in order to place you into the appropriate math classes at DVC. Currently we offer two different

options for placement testing. Which math test should you take? Can you answer these questions?

1) The graph of which of the following equations is a straight line parallel to the graph of y = 2x?

a) 4x – y = 4 b) 2x – 2y = 2 c) 2x – y = 4 d) 2x + y = 2 e) x – 2y = 4

2) If log 10X = 3, then X =

4) An apartment building contains 12 units consisting of one- and two-bedroom apartments that

rent for $360 and $450 per month, respectively. When all units are rented, the total monthly rental

NO - If you can't answer the questions above AND/OR you haven't completed Algebra II or higher

within the last two years, then take the ISSA at http://www.dvc.edu/issa/. This can be done from

your personal computer.

Informed Student Self Assessment (ISSA)

In addition to assessing your skills and background in math, the ISSA explores your overall time

commitments and activities outside of math. This test is available online and may be taken multiple

times a semester. Students do not come to the Assessment Center to take the test, as it is designed

to be taken at your convenience at any computer of your choice with internet access.

This test is used for placement into:

Math 75 - Arithmetic and Pre-algebra

Math 110 - Elementary Algebra

Math 114 - Geometry

Math 120 - Intermediate Algebra

YES - You can take the test at DVC Pleasant Hill or San Ramon Campus. If you have taken Algebra II or

higher within the last two years AND you are able to successfully answer the questions on the College

Level Math sample test then you should take the test.

How Do I Request an

Official Transcript? 1. Go to http://m.4cd.edu (or the

InSite app) and sign in using

your username and password.

2. Click on the “Grades-Transcripts”tile, and then on

“Official Transcript Request”.

3. CAREFULLY read the information on the next page. This is important information regarding the transcript ordering process, and pricing for

transcripts.

4. Fill in the request. You can have transcripts mailed directly to the college of your choice, or to your own address. When ordering online, you can only select standard or rush processing. Express transcripts must be requested in person, and have an additional fee. Note: Check the box if you attended DVC before 1999.

5. If you need to pay for your transcript request, enter your credit card information on the next screen, submit your

request, and print a receipt.
