Person Centred Planning is…

A set of tools and approaches that can be

used to plan support with someone rather

than for them. It is a process of continual

listening and learning that focuses on what

is important to someone now and in the

future.

• My children, I have two sons and one daughter

• My family history, I have been through a lot in my life. I am normally a private person but I might share a little of my life story… listen!

• My physical health, keeping active and maintaining good circulation to keep mobile, I want to remain independent

• My mental health, having stimulating conversation and keeping in control of the things happening around me

• Being able to sort things out in my home at my own pace, I know there is a lot to do but I really value my belongings, they all have a lot of memories attached

• Being able to go out shopping, go to the bank and post office when I need to, I like having control over my dealings

• Seeing people I know on the street, it makes me feel good to know I am known by my neighbours

• My friendship with Eddie and June across the road, they look out for me

• Do not call me SIR!!• My memory is not what it used to be, I

sometimes forget names but I am good with faces, don’t be offended if I don’t remember your name

• If you are coming to visit me, call the day before to remind me and again before you arrive to warn me so I can be ready on time

• Knock on the door and step back so I can see you from the window

• Ask me if I would like to take part in any activities or programmes, I like to have some variety

• I use my trolley for balance when I am walking, I prefer this to a walking stick as it has weight. I can use a stick too so ask me if I am needed to

• Sit and talk with me, I am a reserved person with a cheeky side!

• Give me information in writing and ask me to put any meeting dates onto my calendar so I can check what is happening during the week

• I attend St Mauritius House on a Monday and Friday, my meals on wheels are delivered to my home on other days between 12pm-1.30pm, support me by reminding me not to go out during that time

What the families say...

“This has enabled her to confront her problems & reflect upon what is really important to her to maintain harmony

in the family”

“..it taught me not to

presume anything

about who she is or

how she perceives

things. Mum has plenty

of wishes and dreams

and this made me

realise that so much

more is not only

possible but essential”
