the 17th June 2014 were discussed and agreed

2 Hate Crime GH has invited the police to attend this meeting to discuss hate crime but to date they have not yet attended GH to keep chasing SP reported in the absence of the police that the hate crime campaign has been very successful with a 76 rise in reporting to the police The PCSOrsquos met with the voice group recently The visit helped to break down barriers because some people with learning disabilities had said that they had been fearful of the police The session had an important message but was delivered in light hearted way People tried on the police clothes looked at what they had in their pockets and then a police van turned up which turned out to have an office in the back of it The PCSO visit helped a lot and people found out more information about how the police help people and that PSCOrsquos canrsquot handcuff anyone they can only sit on them Hate crime is not acceptable and it is not right

Linzi discussed her presentation At the moment Linzi is only working in Bedford Borough KW will discuss the project with the Childrenrsquos SEND Steering Group The volunteers at Mencap have to be between 16 ndash 25 years of age It is a free service and there is the possibility for groups to apply to Mencap to ask them to fund their project

4 Service User News Voice Groups Todaysrsquo topic is Health The group have been discussing health and one of the issues is around AampE and having to wait so long to be seen The voice groups have liaised with the Hospital Forum and AampE to take the concerns forward and AampE will be attending the next Hospital Forum on the 21st October 2014 2pm ndash 4pm Leaflets ndash we are looking through the hospital leaflets at what is good and what is not Voice groups will then give feedback to help to make things be more accessible Lots of working with the police around hate crime the group has learnt a lot from the police and the police have learnt a lot from the group The Helping Together Group will be meeting at the Tiger Moth pub on the 21st October 2014 at 630pm all are welcome SP has sent out some emails asking to meet with MPrsquos about voting GH to send out the paperwork for the new co-chair The next Voice Group meeting is at Mill Street on Thursday SP advised she is going to start working with Tony again regarding annual health checks

5 Carers News The Carers Forum is on the 30th September 2014 at The Pilgrim Centre the topic is lsquowhat is the care act all aboutrsquo The carers grant has been updated to make it more simple and clear and will go live on the 1st October 2014 The grant is still health related LB to send information to GH AT advised that family carers are concerned about staff shortages in the care homes and that this sometimes means that activities are being cancelled ie day trips not happening due to staff being on sick leave AT queried when should complaints be made KW advised that the concerns should be made to the manager of the respective service in the first instance

6 Health News The Hospital Forum is on the 21st October 2014 and AampE will be attending Everyone is very keen to know what is happening at Twinwoods with the Services for People with a Learning Disability review (SPLD) Michelle Bradley to be asked to attend the next meeting and feedback LB will ask MB for a brief update to be sent out with the minutes People would like to have this information in an accessible way as possible

7 Health Facilitation ndash (Attached) Terri and Sam discussed their presentation They have links with other hospital Liaison Nurses such as Addenbrooks Hospital and can contact and ask them to support as and when required They work with physiotherapy through the SPLD contract if there are changes to an individualrsquos physical need then they would require a reassessment If the physiotherapy exercises are documented then there needs to be evidence that they are being completed The team work Monday ndash Friday 9am ndash 5pm but can start earlierwork later as and when required to support someone if they are made aware in advance SM described an incident where someone had been playing a football game and had been injured and knocked unconscious they were taken to the LampD hospital as it was the closest hospital and the health facilitation service had contacted the hospital and arranged support for him before he had even arrived there through the Health Facilitator

8 Joint Health and Social Care Self-Assessment KW advised that this is due to arrive any time now and will be very similar to the big questionnaire completed last year Meetings will be arranged in the near future with health and social care professionals in the first instance to start the work but feedback from everyone at the Partnership Board will again be very important We will be asking for lsquoI-Storiesrsquo and personal experiences good or bad on a range of topics KW said that everyonersquos help to complete the assessment would be greatly appreciated and she will update everyone as time moves on Any outstanding items from the SAF can be shared through the Partnership Board KW thanked everyone for their support in completing last yearrsquos SAF GH to send out last yearrsquos template for I stories- to help people to think about their experiences- this can be completed by a carer professional supporting a person or by services areas about experiences

9 Winterbourne View Update Feedback on how we are doing in Bedford Borough The Improvement Advisor has attended the Winterbourne Steering Group Meeting The plan has been extended to now focus on supporting those out of Borough who may want to move closer to their family networks and to prevent where possible younger people and others from moving away

The 1st June was an important government date where each Local Authority had to give an update on anyone who was in a hospital setting and this date was a deadline for individuals to have moved by This was to make sure that people in a secure hospital were only there if it was agreed to be appropriate No one from Bedford Borough is deemed to be inappropriately placed in a hospital setting Anyone who is in a secure hospital setting at this time is regularly reviewed and plans for their future (when they are well enough to be discharged) to a community setting are person centred and are on-going The Councilrsquos Adult Services continue to work with Childrenrsquos Services to help to identify what services are available and what services needed locally to help people to live closer to their family and local networks To help people and their carers to see what services are available The lsquoLocal Offerrsquo is now live on the Councilrsquos website The Special Educational Needs and Disability (SEND) steering Group is continuing to update the plans httpssendguidebedfordgovuk The Joint Health and Social Care Assessment asks questions about secure hospital placements linked to the Winterbourne plan

What is this information about This easy read information is about the Joint Health and Social Care Self-Assessment Framework The framework is a list of things that areas check to see how well services are doing in supporting people with learning disabilities and their families

This information includes

An introduction to the framework and what it is about

A presentation that tells people what is being checked

A paper that explains some of the words used

A flyer that areas can use to invite people to any events they might run

Why have we put this together We hope this information will help people understand what the framework is all about and what will happen We want people with learning disabilities and their families to have a strong voice in the Framework and we hope this information helps with this

How to use this Information We hope that areas will use this information in a way that works for them For example we do not expect that people will talk through the whole presentation about what is being checked Areas may just use this to check their scores when action planning or if they want to explain the lsquobig ideasrsquo We have left in some words that might be called jargon We have done this because we think it is really important that people have the chance to learn what certain words mean We think this makes people have a stronger voice This is why we have also written the Talking Health and Social Care Sheet However we understand that lots of this information will be new or hard to understand for lots of people You might want to talk through this with others such as your colleagues or other members of your-self advocacy group

What is governance

Governance can mean different things to different people and organisations For this framework we think it is about

Reporting or telling people about what is happening for people with learning disabilities and their families

Planning what needs to happen to make things better for people with learning disabilities and their families

Making sure that the right people can take the right action

This happens at three levels and by different people and organisations

In your local area Clinical Commissioning Groups the council local government cabinet and your Health and Well Being Board should make sure the work is being done

Across your region NHS England and your regional ADASS are making sure the work is happening

Across England it is the job of the Ministerial Learning Disability Programme Board NHS England and National ADASS who make sure the work is happening

4 Service User News Voice Groups Todaysrsquo topic is Health The group have been discussing health and one of the issues is around AampE and having to wait so long to be seen The voice groups have liaised with the Hospital Forum and AampE to take the concerns forward and AampE will be attending the next Hospital Forum on the 21st October 2014 2pm ndash 4pm Leaflets ndash we are looking through the hospital leaflets at what is good and what is not Voice groups will then give feedback to help to make things be more accessible Lots of working with the police around hate crime the group has learnt a lot from the police and the police have learnt a lot from the group The Helping Together Group will be meeting at the Tiger Moth pub on the 21st October 2014 at 630pm all are welcome SP has sent out some emails asking to meet with MPrsquos about voting GH to send out the paperwork for the new co-chair The next Voice Group meeting is at Mill Street on Thursday SP advised she is going to start working with Tony again regarding annual health checks

5 Carers News The Carers Forum is on the 30th September 2014 at The Pilgrim Centre the topic is lsquowhat is the care act all aboutrsquo The carers grant has been updated to make it more simple and clear and will go live on the 1st October 2014 The grant is still health related LB to send information to GH AT advised that family carers are concerned about staff shortages in the care homes and that this sometimes means that activities are being cancelled ie day trips not happening due to staff being on sick leave AT queried when should complaints be made KW advised that the concerns should be made to the manager of the respective service in the first instance

6 Health News The Hospital Forum is on the 21st October 2014 and AampE will be attending Everyone is very keen to know what is happening at Twinwoods with the Services for People with a Learning Disability review (SPLD) Michelle Bradley to be asked to attend the next meeting and feedback LB will ask MB for a brief update to be sent out with the minutes People would like to have this information in an accessible way as possible

7 Health Facilitation ndash (Attached) Terri and Sam discussed their presentation They have links with other hospital Liaison Nurses such as Addenbrooks Hospital and can contact and ask them to support as and when required They work with physiotherapy through the SPLD contract if there are changes to an individualrsquos physical need then they would require a reassessment If the physiotherapy exercises are documented then there needs to be evidence that they are being completed The team work Monday ndash Friday 9am ndash 5pm but can start earlierwork later as and when required to support someone if they are made aware in advance SM described an incident where someone had been playing a football game and had been injured and knocked unconscious they were taken to the LampD hospital as it was the closest hospital and the health facilitation service had contacted the hospital and arranged support for him before he had even arrived there through the Health Facilitator

8 Joint Health and Social Care Self-Assessment KW advised that this is due to arrive any time now and will be very similar to the big questionnaire completed last year Meetings will be arranged in the near future with health and social care professionals in the first instance to start the work but feedback from everyone at the Partnership Board will again be very important We will be asking for lsquoI-Storiesrsquo and personal experiences good or bad on a range of topics KW said that everyonersquos help to complete the assessment would be greatly appreciated and she will update everyone as time moves on Any outstanding items from the SAF can be shared through the Partnership Board KW thanked everyone for their support in completing last yearrsquos SAF GH to send out last yearrsquos template for I stories- to help people to think about their experiences- this can be completed by a carer professional supporting a person or by services areas about experiences

9 Winterbourne View Update Feedback on how we are doing in Bedford Borough The Improvement Advisor has attended the Winterbourne Steering Group Meeting The plan has been extended to now focus on supporting those out of Borough who may want to move closer to their family networks and to prevent where possible younger people and others from moving away

The 1st June was an important government date where each Local Authority had to give an update on anyone who was in a hospital setting and this date was a deadline for individuals to have moved by This was to make sure that people in a secure hospital were only there if it was agreed to be appropriate No one from Bedford Borough is deemed to be inappropriately placed in a hospital setting Anyone who is in a secure hospital setting at this time is regularly reviewed and plans for their future (when they are well enough to be discharged) to a community setting are person centred and are on-going The Councilrsquos Adult Services continue to work with Childrenrsquos Services to help to identify what services are available and what services needed locally to help people to live closer to their family and local networks To help people and their carers to see what services are available The lsquoLocal Offerrsquo is now live on the Councilrsquos website The Special Educational Needs and Disability (SEND) steering Group is continuing to update the plans httpssendguidebedfordgovuk The Joint Health and Social Care Assessment asks questions about secure hospital placements linked to the Winterbourne plan

How to use this Information We hope that areas will use this information in a way that works for them For example we do not expect that people will talk through the whole presentation about what is being checked Areas may just use this to check their scores when action planning or if they want to explain the lsquobig ideasrsquo We have left in some words that might be called jargon We have done this because we think it is really important that people have the chance to learn what certain words mean We think this makes people have a stronger voice This is why we have also written the Talking Health and Social Care Sheet However we understand that lots of this information will be new or hard to understand for lots of people You might want to talk through this with others such as your colleagues or other members of your-self advocacy group

What is governance

Governance can mean different things to different people and organisations For this framework we think it is about

Reporting or telling people about what is happening for people with learning disabilities and their families

Planning what needs to happen to make things better for people with learning disabilities and their families

Making sure that the right people can take the right action

This happens at three levels and by different people and organisations

In your local area Clinical Commissioning Groups the council local government cabinet and your Health and Well Being Board should make sure the work is being done

Across your region NHS England and your regional ADASS are making sure the work is happening

Across England it is the job of the Ministerial Learning Disability Programme Board NHS England and National ADASS who make sure the work is happening
