WELLNESS BENEFIT POLICY
Benefit: As a benefit to regular, full-time, benefits-eligible employees, SWAN allows up to $20 per month/$240 per year to be reimbursed for wellness-related activities (upon review and approval from the Executive Director). Eligible employees requesting wellness benefits must complete and have the Executive Director’s signature on an Employee Wellness Request Form in order for the activity to be reimbursed once completed. When the employee has finished the event or activity, documentation must be provided along with a completed expense form in order to receive the reimbursement. Acceptable documentation includes payment receipt and/or attendance forms. Reimbursement will then be provided to the employee in check form. Guidelines for Use of Wellness Reimbursement: Below are some basic guidelines for use of the wellness benefit, but these should not be considered to be all-inclusive. If an employee has a question about requested use of the wellness benefit, he/she may contact the Executive Director or Human Resources for guidance. Wellness as a concept should incorporate a direct physical, nutritional, or other health-related benefit. Examples include:
• Gym or fitness club membership • A la carte fitness classes, such as zumba, yoga, Pilates, Crossfit, cardio kickbox, spin, and run
clubs • Registration for activities such as runs, fitness workshops, and triathlons • Flu shots and health screenings • Smoking cessation programs • Nutrition programs, nutrition counseling, or nutrition consults
