    The primary goal of the Aging and Disability Services programs is to promote the maximumlevel of independent living for older adults and persons over 22 with intellectual anddevelopmental disabilities. Services are oriented toward providing the support needed forpersons to remain safely in the community and to prevent unnecessary or prematureinstitutional placements.

    Customer Service Center - 703 -...

    This program makes the assessment of, and designs the service plan for, individuals 60years of age and older who because of their age, frailty, medical condition, or living situationare at risk of institutionalization in a hospital, nursing home, or shelter. Services are initiated

    to support and stabilize their living situations and to prevent or avoid abuse, neglect, orexploitation.

    The Customer Service Center also plays an important part in preventing orstopping abuse, neglect, or exploitation of individuals who are unable to protectthemselves. This responsibility, often called "Adult Protective Services," usessocial workers investigate reports of abuse, neglect, or exploitation and initiateprotective services. To make a report after-hours, call the State Hotline at 1-888-832-3858 .

    Arlington Agency on Aging - 703 -...The goal of the Agency on Aging is to promote the maximum level of independence ofpersons 60 years of age and older by providing a range of varied services. The Agency isone of a national network of over 670 Area Agencies on Aging established by the federal

    Older Americans Act to ensure that older people live as an integral part of society in dignityand with access to programs and services that meet their needs and preferences. ThePersonal Advocate Service enables older persons to gain access to the services they needwith assistance from volunteer advocates who are recruited and trained to provide thisassistance.     Social Services for Seniors and Adults With Disabilities - 703 -...

    The program goal is to prevent premature institutionalization of persons who are frail, older,or have disabilities. An array of services is provided to enable persons to remain safely intheir home as long as possible. When staying at home is no longer feasible, social workersassist in finding the least restrictive setting that will meet the client's needs.

    Senior Adult Mental Health Services - 703 -...

    The primary goal of the program is to promote or enhance independent living of persons 65

    years of age and older through the stabilization of their mental illness or emotionaldisturbance. The program uses a multi-disciplinary approach to psychiatric treatment andincludes comprehensive assessments, medication services, psychotherapy, case coordination,outreach, and consultation. Home-based services are available.

    RAFT is a team of three mental health therapists, a nurse practitioner, a psychiatrist, and apsychologist providing mental health treatment at one assisted living facility and one nursinghome facility, for the Northern Virginia Region. RAFT serves Northern Virginia individualswith mental illness, age 65 and over, who are ready for discharge from a state psychiatric

    hospital and provides geriatric mental health services needed to maintain stability in thecommunity, and prevent a return to the state hospital. RAFT also allows Northern Virginiaelders with severe mental illness to be diverted from a state hospitalization stay, by beingplaced at one of the RAFT sites.

    Volunteer Guardianship Program - 703 -...Volunteers serve as court appointed guardians, conservators, representative payees and billpayers for adults who are limited in their inability to manage their personal and financialaffairs and who have no one else to help.

    Personal Advocate Program - 703 -...

    Volunteers assist Arlington residents 60 and older (and their families or other caregivers) inidentifying and accessing community resources, making sense of medical bills andMedicare/Medicaid statements or long term care insurance policies, or finding options thathelp meet unforeseen needs that arise.

    Nursing Case Management- 703 -...The Nursing Case Management unit supports and empowers individuals and families toachieve and maintain an optimal level of health and well being in the community throughhome visits to older adults who have disabilities or who have multiple chronic illnesses, andlive in their own homes. The program impacts the community at large by maximizingknowledge of health care issues. Services are designed to prevent unnecessary emergencyroom visits, hospitalizations, and premature institutional placement. Physician's referral isrequired.

    Housing Grants (Rent Relief) - 703-228-1350This locally funded program assists eligible low-income households comprised ofseniors or persons with disabilities, or working families with minor children, withtheir rental costs to maintain them in the community and prevent homelessness.

    Real Estate Tax Relief - 703-228-1350Eligible older adults and homeowners with disabilities may be deferred or exemptedfrom paying real estate taxes. Download and print the application.

    Madison Adult Day Health Care Center at W alter Reed Community Center- 703 -...

    This program seeks to enable impaired senior adults to maintain their maximum level ofindependence and to remain in the community as long as possible by providing a day healthprogram in a protected environment. Services offered include health care monitoring, nursingcare and support, personal care, therapeutic recreation, special therapies, nutritionalguidance, lunch, snacks, and support for family breakfast and caregivers.     Intellectual and Developmental Disability Services - 703 -...

    The goal of the program is to promote maximum independence of eligible Arlington Countyresidents with intellectual or developmental disabilities, and to assist them and their familiesin accessing and maintaining appropriate services which will enable them to participate andremain safely in the community. Among the services are assistance to eligible persons infinding suitable residential and vocational placements.

    Office For Persons With Physical Disabilities - 703 -... Voice/ TTY

    The goal of the Office is to promote maximum independence for persons with disabilities byproviding technical assistance for the removal of barriers, thereby enabling persons to haveaccess to County programs, services, and facilities. The Office has the lead responsibility ofensuring that the County government complies with Federal and State disability mandatesincluding the Americans with Disabilities Act (ADA).

    Senior Centers With Meals

    The following senior centers provide half-day programs that offer physical and mentalstimulation for older adults in need of a supportive environment, but not a protected healthsetting. Activities include specially designed exercises, group discussions, counselingservices, health education and screening, and recreation programs. These two centersprovide a hot, noontime meal that meets one-third of the daily nutritional requirement.
