ACHARYA HARIHARAAn Illustrious Personality
Acharya Harihara Das, one of the leading soldiers of the freedom struggle of India,was born to Mahadeb Brahma and Sradhadevi of Sri Ram Chandrapur of Sakhigopal inPuri district in 1869. His educational career began with the village Chahali and themiddle vernacular school after which he joined the Puri Zilla School with a scholarshipfrom the government to complete the entrance examination. He did his F.A. from theRavenshaw College.
Non-acceptance of others belongings is a lesson he imbibed from his childhoodand followed. From his early childhood Acharya Harihar was dedicated towards socialservice. He had laid the foundation stone of an associated social service in the land ofOdisha. He united the student forum and youth for service to the needy and downtrodden. The childhoodfriendship with Gopabandhu, Nilakantha, Krupasindhu and Ananta matured at Ravenshaw College. Being boundby self-resolution, Harihara accepted the invitation to join as teacher in Nilgiri School, then RavenshawCollegiate School and finally Pyari Mohan Academy at Cuttack. He took the challenge to confront the unrulyboys in the school and innovated a novel technique to set them right. He visited their houses, watched theirmovements and established a cordial relationship with their parents offering the scope to teach the boys freely.This changed the chemistry between them. He never punished the boys for their faults rather explained themconvincingly till they admitted their faults.
How to liberate India from the British-Yoke was the sole thought from his childhood which he tried totranslate into action by creating people of extraordinary caliber who could face all the grim situations heroically.He left Pyari Mohan Academy to join the Satyabadi Open Air School which was to be conducted in the greenverdant trees of vakulaand chhuriana at Sakhigopal. Virtually, the school was the brainchild of UtkalmaniGopabandhu Das and Acharya Harihar. As a Superintendent of the hostel, he became popular among 4 thestudents. Though students referred to other teachers as Pundit, Harihara was referred to as Acharya and wasa great favourite with the students. The basic objective of Satyabadi School was to inculcate the sense ofnationalism among the masses by uniting all, irrespective of caste, creed and religion. The decline of SatyabadiSchool did not cast any negative impact on him rather he cheerfully accepted Gopabandhus proposal to workfor one Bidhaba Ashrama at Puri. The young widows were brought to this ashram where all sorts of care likeeducation, for moral and mental development was extended by Acharya Harihara. When Gopbandhu wasimprisoned, he became the editor of The Samaj and kept the paper afloat as a medium of socio-politicalawareness in Orissa, He also managed the Swaraj Ashram at Cuttack.
The clarion call of Mahatma Gandhi to carryout Salt-Satyagraha in1930 created a stir in the nook andcorner of India. Acharya Harihar led the procession to Inchudi receiving welcome from village to village till arrivalat the destination. He was arrested there by the police and spent six months at Hazaribagjail in Bihar. Therehe spent much of his time to spread messages against the British rule. He was a true disciple of Gandhijiwho sincerely worked for attaining freedom of India.
He was elected unanimously as the president of Provincial Congress Committee after the OrissaProvince came into existence.
Acharya Harihara and other freedom fighters were arrested and put to jail. Acharya taught these fellowsinside the jail. Here Acharya Harihara took food just for sustenanceand translated Bhagabat Gita. He wasreleased from jail in the year 1944. He became an active member of Bhoodan Movement and accompaniedVinobajee from village to village in the year 1955. Presiding over the 12th Annual Conference of SarvodayaSammilani held at Sevagram on 26th March, 1960 he told, Freedom has come. But it does not reach thevillage. The sunrises in the east. But does not find the bright sunrays as we close ourdoors.The great souldid unceasing work for the upliftment of the people of Orissa and passed away on 29th. February, 1971 at93 years.

AIMS AND OBJECTIVES
1. To undertake basic, applied and statistical research in various specialties of oncology.
2. Generation of technical and scientific man-power in collaboration with other Medical Collegesof the State.
• To organize and conduct under-graduate, post-graduate and doctoral training courses formedical personnel.
• To organize and conduct PG & doctoral courses in Radiation Physics, Immunology, Bio-Chemistry, Radiation Pathology, Radio-Biology, Cyto-Genetics, Cell-Biology, MolecularBiology and Cancer Epidemiology.
• To organize paramedical courses in Laboratory science, Nuclear Medicine,Radiotherapy,Radio diagnosis, Cytology, Nursing and Cancer epidemiology.
• To train medical professionals in Oncology.
3. To provide modern and advanced clinical services to cancer patients in collaboration with theMedical Colleges of the state.
4. To institute professorship, readership, fellowship and other teaching & research positions forthe above purposes.
5. To design preventive services for the General Health Care Delivery System for extension toother areas.
6. To conduct lectures, seminars, study groups, workshops, conferences on problems relatedto cancer.
7. To organize hospital based tumor-registry on modern lines.
8. To institute and maintain an up-to-date library, related to various specialties of cancer.
9. To publish research papers, treaties, books and periodicals & other literatures relating tovarious specialties of cancer.
10. To work in collaboration with other medical institutions, Health Service Departments, otherUniversities, aid agencies and institutions of repute and to co-ordinate with other societies inthe pursuit of any of the above objects.
11. To perform all such acts as may be necessary or appropriate or incidental for achievementof any or all of the above objects.

I am extremely happy and feel satisfied that Acharya Harihar Post Graduate Instituteof Cancer is making rapid strides in its progress to serve the cancer affected persons of thestate & adjoining areas. All the service parameters are in upward trends. Newer serviceslike upper & lower GI endoscopy, Oro-pharyngoscopy have been added besides strengtheningof the existing services. This is the beginning. A lot more work needs to be done to put thiscentre at par with other national cancer centres. The name of Acharya Harihar RegionalCancer Centre (AHRCC) has been changed to Acharya Harihar Post Graduate Institute ofCancer (AHPGIC). This itself confers a great recognition and at the same time raised theexpectations. We must live up to the expectations & ready to share more responsibilities.
Space is a big constraint for the growth. Government has kindly provided around 4acresof additional lands available besides AHPGIC for its expansion. The expansion of AHPGIC hasbeen linked with the expansion of SCB Medical College & Hospital. There will be one masterplan for smooth flow of the patients keeping identity of both the centres separate.
Growth is a continuous process. This process must not halt. Sometime it will be faster& at times it may slow down. But it must not stop. This can only be achieved with a teamwork with the right kind of attitude & motivation. I feel happy, over the last four to five years,we could build that team. I sincerely hope, the team delivers, deliver fast with compassion& humane approach.

REPORT OF THE DEAN AND PRINCIPAL
Acharya Harihar Post Graduate Institute of Cancer (AHPGIC) formerlyknown as Acharya Harihar Regional Cancer Centre (AHRCC), born in 1984,is now a prominent developing regional cancer centre of Eastern India.Around 90,000 patients are attending the OPD annually out of which around26,000 patients are admitted to indoor to avail different services from thiscentre. Out of them around 11% are from KBK districts and 3% are fromadjoining states of West Bengal, Jharkhand, Chhatisgarh, Andhra Pradesh.There are 10 different departments in our centre providing diagnostic andtherapeutic services like Surgery, Radiotherapy and Chemotherapy. Palliative care and preventiveOncology services are provided to improve the quality of life.
AHPGIC, Cuttack is now a full-fledged post-graduate institute with superspecialitycourses. The courses have been approved by Utkal University. Teaching and training programmesof these courses are going on as per MCI guidelines in AHPGIC. The final university examinationof first batch of MCh student is being conducted in the month of August, 2020.
During the year 2019 four MCh students - two in Gynaecological Oncology and two inSurgical Oncology have been admitted in the 3rd batch as per the MCI guideline. The followingfour students selected through NEET superspeciality counselling-2019 had taken admission inthe month of September, 2019. They will appear in their university examination in the year2022.

The PG course in Radiation Oncology has been inherited from SCB Medical College whenAHRCC was declared as an autonomous institute in 1984. Now, six PG students are takingadmission each year for Radiation Oncology course.
Besides, this year twenty students have taken admission in the 3rd batch in Diplomain Medical Laboratory Technology (DMLT) course and continuing their study as per the DMLTCouncil guideline of Government of Odisha. They will appear in their final examination in theyear 2021.
We are planning to open PG courses in the subject of Anaesthesiology, Onco-Pathology,PG Diploma in medical physics and training programmes for medical physics interns andDiploma in Radiotherapy Technician (DRTT) course.
Research is an integral segment of medical education and patient care. We have aregular institutional ethics committee (IEC) under the chairmanship of Dr. Asok Das, Bhubaneswarand Dr. Dillip Kumar Agarwalla as Member Secretary. We are holding meetings at regularintervals.
The PG students are continuing their research for their thesis programme. A researchprogramme sponsored by I.C.M.R is going on with Prof. Sagarika Samantray of Onco-PathologyDepartment.
The objective of publishing a year book is to compile all academic activities going onin AHPGIC. This will encourage its faculties, PG students, Doctors, Research Scholars, Paramedicalworkers to improve their academic activities and patient care.
This centre is the first of its kind as post-graduate medical education centre of ourstate which will help to produce cancer specialist and para-medical workers to fulfill the needof human resources for better patient care in our state as well as the country in the fieldof Oncology.
For this endeavour, I am grateful to our Director, faculties, residents, doctors, PGstudents and staffs for their participation and encouragement.

ACADEMICS & RESEARCHAcharya Harihar Regional Cancer Centre
has been recognized as Acharya Harihar PostGraduate Institute of Cancer, (AHPGIC) dueto its dedication towards cancer research andtreatment in 2019-20. Two M.Ch students eachin the subject (Gynaecological Oncology andSurgical Oncology), six PG students of RadiationOncology and Twenty DMLT students have takenadmission during the academic year 2019-20.The first batch of internship in medical physicshas been completed successfully.
Research is an integral segment of medicaleducation and patient care. Institutional EthicsCommittee of the centre conducts meetings atregular intervals for review and approval of newresearch work to be performed. Several Govt.and industry sponsored intramural andextramural research works are being conductedin the institution resulting in good amount ofpublications in premier journals of national andinternational repute.
Library Committee supervises themaintenance and development of AHPGICcentral library and e-library. Post graduatecommittee monitors PG & M.Ch. education aswell as inter departmental and inter institutionalacademic activities.
Hospital Based Cancer Registry &Patterns of Care & Survival Studies in CancerCervix, Cancer Breast and Head & NeckCancers (HBCR-POCSS) project under NationalCancer Registry Programme is working in thecentre. Similarly the centre is enrolled in NationalCancer Grid network. The NCG was created toform a union of cancer centres in the countrywhich could provide uniform and high standards

Telemedicine centre is a great help forcancer patients of different districts minimizingthe long distance travel to the centre. This isalso being utilized for teleconferencing withpremier institutes for academic discussions andmeetings. This service became the best supportfor the patients during the lock down due toCovid-19 pandemic outbreak.Community Activity
AHPGIC is actively engaged in thecommunity service in the form of cancereducation, consultation and early detectionthrough camps and various media across thestate.

SIGNIFICANT EVENTS• Acharya Harihar Regional Cancer Centre
has been officially renamed as AcharyaHarihar Post Graduate Institute of Cancer(AHPGIC).
• The Foundation Stone has been laid byHonble Minister of Health & Family Welfarefor construction of bunkers for newRadiotherapy Unit on 09.08.2019.
• The adjacent land of around four acreshousing quarters of Industry, DTET andGA department handed over to AHPGICfor expansion as per master plan of TataTrust (OCCF).
• Ten no. of beds have been added toPaediatric Oncology Unit with existing tenbeds and the total bed strength hasbecome twenty.
• A ten bedded separate Pain & PalliativeCare unit has been started.
• Both upper-GI and lower-GI endoscopyservices has been started by videoendoscope systems.
• Oro-pharyngo-laryngoscopy has beenstarted by Rigid Hopkins video endoscope.
• An MOU has been signed with CDAC,Noida for implementation of O-eHMISProject as per the decision of Health &FW Department on 08.11.2019.
• Third batch of M.Ch. student in SurgicalOncology and Gynaecological Oncologyhave joined for the session 2019-20.
• The OT Technician course (5 seats) willbe started as per the recognition of StateBoard for Allied Sciences.
• An MOU has been signed with SambalpurUniversity for initiating the Internship coursein Medical Physics in collaboration withAHPGIC.
• On the occasion of World Hospice andPalliative Care Day, Acharya Harihar PostGraduate Institute of Cancer, AlphaPalliative Care, Kerala and TeamUmeedein organized a one day workshopon Palliative_care on 13.10.2019.
• A workshop on Breast Cancer AwarenessMonth was organized on 12.10.2019.
• A state level CME on Cancer Awarenesswas organized for paramedical and nursingstudents on the occasion of NationalCancer awareness Day in AHPGIC on07.11.2019.
• A CME on Good Clinical Practice wasconducted in collaboration with ICMR-RMRC, Bhubaneswar on 23.11.2019 asper the New Drugs and Clinical TrialsGuidelines.
• The State level World Cancer Day wascelebrated on 04.02.2020 at AHPGICwhere Sri M. Balakrishnan, IAS (Retd.),Chief Advisor, CMO and Chief Advisor,Special Initiatives was the Chief Guest.
• Several Cancer Screening and AwarenessCamps have been organised in differentdistricts of Odisha by faculties fromAHPGIC.

ANESTHESIOLOGYThis Department caters to the need of the
departments conducting surgical procedures inthe form of preoperative, intraoperative, post-operative anesthesia, respiratory and recoveryroom care. The department also provides servicesto:(a) the Radiation Oncology department for
treatment of pediatric and uncooperativepatients,
(b) the Medical Oncology department duringintrathecal administration ofchemotherapeutic agents,
(c) training to postgraduates in Anesthesiologyof SCB Medical College & Hospital.

PALLIATIVE MEDICINEThis unit provides prompt pain relief and
palliative care through different modalities ofservices & immediate management of pain topatients at any stage of the disease (mostlyterminally ill) with an intention for an improvedquality of life. The services provided here havemanaged to garner recognition throughoutdifferent modalities of services & immediatemanagement of pain to patients at any stage ofthe disease (mostly terminally ill) with an intentionfor an improved quality of life. The servicesprovided here have managed to garner recognitionthroughout the region which is reflected throughthe influx of patients from various governmentand private medical colleges & hospitals of easternIndia. IAPC certification courses are beingconducted in 2 sessions in a year for all doctors,nurses, volunteers and social workers at thecentre.
Our centre, being the State Nodal Centrefor Palliative Care, coordinates with the 30 satellitecentres established at district headquarterhospitals of the state, to offer Palliative treatment.
There is a plan to strengthen this departmentby creating separate faculty post, thereby openingPG course in Palliative Medicine.• 60 Doctors & Nurses from 30 Districts ofOdisha had a 2 days of Class room teaching atNABM & One day Hands on Training at AHPGICregarding Pain and Palliative Care treatment forcancer patients.• A sixteen hours volunteers programme inPalliative Care at the Theosophical Society ,Cuttack on 14th March 2019 in collaboration withIAPC was Conducted and a group of 75participants we are trained to address physical,psychological, Social and Spiritual needs ofterminally ill Patients by Dr. Sumita Mohanty.• 30 Home care Nurses under OSDA haveundergone Training in Pain Palliative Care Unitfrom 16th August to 5th Sept 2019 (A group of10 over five days period)

HEAD & NECK ONCOLOGYOral cancer is one of the 10 most common
cancers of the world and the commonest in India.There is excellent evidence from many sourcesthat approximately 90% of these cancers are dueto tobacco abuse in any form and poor oral &dental hygiene. Almost 70% - 80% of the casesreported have advanced stage of cancer whichis practically in Stage-III & IV. Cancers of theThyroid gland, parotid gland and minor salivaryglands are also frequently reported in this Dept.The incidence of pre-malignant lesions likeleukoplakia and sub-mucous fibrosis hasincreased considerably. Cosmetic value being avital factor, various reconstructive procedures isbeing performed by this department. The post-graduate students, Paramedical staffs and ex-service doctors are imparted regular trainingprograms keeping in view the early detection,
prevention and treatment of Head & Neck cancersas the main theme.

RADIATION ONCOLOGYThis department treats about 200 patients
daily using telecobalt therapy and HDRbrachytherapy. About 65% of all cancer patientsattending this institute avail radiation therapyduring their course of treatment. External Beamtherapy is delivered by one telecobalt units(Theratron-780C) from 8:00 AM to late night inthree shifts on all working days. Multiple-fields,wedge, block and tangential fields at 80cm SSD

are practiced as required. Now the department iswell equipped with Linear Accelerator andCTSimulator. More than thousand patients weretreated successfully with most sophisticatedtreatment modalities like VMAT and IMRT. PG-studies in radiotherapy leading to MD (RT) isavailable in the institute since 1973. Six PGstudents admit every year for their MD degree.Post graduate seminars are organized twice amonth.

SURGICAL ONCOLOGYSurgical oncology is one of the important
departments of the cancer centre. Usually all thenew patients coming to the center are first seenin the Surgical Oncology OPD and after gettingthe tissue or cytological diagnosis and evaluationfor operability both clinically and radiologically,those patients are taken for surgical admissionand operations. Inoperable cases detected eitherclinically or radiologically are referred for othermodality of treatments like chemotherapy and orradiotherapy.
Modified radical mastectomy, Breastconservation surgeries, radical gastric resections,Abdomino Perineal Resection of rectal cancersare few of those surgeries being done routinely.Radical as well as conservative surgeries forcancer penis are also done frequently. Many atimes palliative and bypass surgeries are beingdone to alleviate pain and agony, Oesophago-gastrectomy, Whipples procedures are beendone less frequently. Wide local excision of softtissue cancers are also being done in routinebasis along with Reconstructive procedures. Onereconstructive surgeon is working full time buton deputation basis from civil hospital. A fulltimefaculty in plastic and reconstructive surgery isbadly needed for this department. Post-graduatestudents from the department of Surgery, SCBMedical College, Cuttack are undertaking trainingon rotation, They are getting much betterexposure and benefit as more than 50% of casesin their examination period are oncology subject.We need to get more and more support from theHOD Surgery from SCB Medical college in thisregard posting them on rotation and also for atleast 3 months instead of 1 month. Surgeonsfrom peripheral health services are alsoundergoing re-orientation training in surgicaltechnique on rotation basis. This reorientationprogram for the peripheral surgeons needs to beon a more regular basis which is not the factnow. The departmental seminar is beingconducted regularly. The faculties have also

delivered many guest lecturers, presented papersin conferences and taken part in varioussymposia and workshops. Faculties have regularlytaking part in early cancer detection andeducational programs joining the cancer controlcamps and various awareness programs. NATIONAL CANCER GRIDThe NCG was created to form a union of cancer centres in the country which could provide
uniform and high standards of cancer care, follow uniform evidence based guidelines formanagement of patients, develop human resource adequately trained to fulfill the cancer healthcareneeds of the entire nation and conduct collaborative clinical research of a high standard.
The union would to ensure provision of same quality of care to patients from any part ofthecountry, without having to travel long distances and regardless of their ability to pay for treatmentachieved by following the same evidence-based management guidelines, which are implementablein all cancer centres.
Aims and Objectives1. Evolve and implement uniform standard of cancer care across India2. Offer the state of art services for those afflicted by cancer.3. Create Human Resources to tackle the rising need of cancer care.4. Gauge the magnitude trend and type of national cancer problem through registry and run

COMMUNITY OUTREACH ACTIVITIES
INTRODUCTION:Every year World hospice and palliative care
day is celebrated on second Saturday of October.On the occasion of World hospice and
palliative care day, Acharya Harihar Post GraduateInstitute of Cancer, Cuttack, Alpha Palliative Care,Kerala and Team Umeedein organized a one dayworkshop on Palliative care.
Various doctors social workers and socialorganizations across Odisha participated in theevent and had a constructive discussion on thetopic.
We are really thankful to the director,AHPGIC cuttack, Mr Sarangi, Dr KrupasindhuPanda, Panda cancer hospital. Dr Sukdev Nayak,AIIMS Bhububaneswar. Dr Mami Parija, Dr SumitaMohanty and all the participants.OBJECTIVE:
Palliative Care is very much essential butone of the most neglected service in Odisha.Palliative care focuses on providing relief fromspiritual pain, physical stress, and mental stressat any stage of illness.
The goal is to improve the quality of life forboth the person in illness and their family.EVENT SUMMARY:
The event was inaugurated by Dr LalatenduSarangi, Director, AHPGIC and attended byeminent doctors, social activists and NGOs fromdifferent districts.
Addressing the session, Dr Sarangiemphasized on the need of hospice & palliativecare in Odisha and appealed to the audience forjoining hands.
Dr Sumita Mohanty, Associate ProfessorDept. of Anesthesia, AHPGIC addressed theaudience about What is Palliative care. She alsoshared case studies of some patients.
Dr Sukdev Nayak, HOD, Deptt ofAnaesthesiology, AIIMS, Bhubaneswar addressedthe audience about Timeline of palliative caredevelopment in Odisha and spoke about thechallenges faced in introducing palliative care inOdisha, present scenario and shared case studieswhere he was able to bring a smile in the face ofpatients with pain management.
Shri Nitai Panigrahi, Settler-cum-ManagingTrustee, Umeedein spoke about CancerAwareness for Early detection & Accessibletreatment and the role Umeedein is playing tocreate awareness in Odisha. He also emphasizedon the necessity of palliative care and painmanagement of cancer patients.
Dr Jose Babu, Head of Med. Division, AlphaPalliative Care shared with the audience aboutthe Hospices and Link Centres - Alpha ModelPalliative Care i.e. their working model andstressed how a workable model can be startedfor Palliative care in Odisha with collaboration.
Shri K M Noorden, Founder and Chairman,Alpha Palliative Care, Kerala spoke about thehistory of Alpha Palliative Care and challengesfaced in the journey so far to become one of thelargest home based palliative care provider inthe country based in Kerala. He shared with theaudience about Role of Volunteers in palliativecare and also appealed for commencing hospiceand home care in Odisha with their collaboration.He shared audio visuals about history of AlphaPalliative care and impact of community basedpalliative care

A draft blueprint was formed to starthospice, home care and palliative care in Odishawith joint collaboration of Umeedein, AlphaPalliative Care and other district level NGOs.

BREAST CANCER AWARENESSPROGRAMME
The month of October is celebratedworldwide for Breast Cancer Awareness andAHPGIC, Cuttack joined hands with all enthusiasmin the global event on October 12th 2019. ABreast Cancer Awareness Programme andScreening Mammogram were held in AHPGIC,Cuttack in the OPD Block with the theme.
From Screening to Survival: An AHPGICInitiative.
Several dignitaries like the Secretary,Cuttack Municipal Corporation Mrs Ananya Das,Inner Wheel President Mrs Gayatree Das,Advocate Odisha High Court & Supreme CourtMr Abhijit Pattnaik, Rajyogini PrajapitaBrahmakumari BK Saral, Editor Orissa MedicalJournal & Senior Psychiatrist Dr. Samrat Kar,
Secretary Odisha Youth Journalist Mr DeepeshMohapatra graced the occasion.
The event was presided by Dr LalatenduSarangi, Director, AHPGIC, Cuttack who welcomedthe august gathering in a background of pinkribbons and balloons, aptly suiting the occasionin the OPD Block of AHPGIC. The targetpopulation for the event were selected membersof the Inner Wheel Association, Cuttack, membersof Prajapita Brahmakumari, Cuttack andDhenkanal, members of Jain Community,Choudhury Bazaar, and others from various partsof Cuttack.
Prior to organising the event, the targetpopulation was sensitised about the requirementof Screening Mammogram and only those withhigh risk factors like positive family history, ageabove 40 years, obesity, history of other benignconditions of the breast were selected forScreening Mammogram in the Dept of Radiology,AHPGIC, Cuttack.
More than 100 females participated in theAwareness Programme and 58 of them underwentMammogram. Reporting was done as per BIRADSScore (Breast Imaging Reporting and DataSystem). Twenty two females had BIRADS 1,twenty nine had BIRADS 2, six had BIRADS 3and one had BIRADS 4 Score. Those with BIRADS3 and 4 were further called for USG of breast thenext week and the one with BIRADS 4 was advisedbiopsy of the breast mass.
Various faculties of AHPGIC deliveredlectures on the risk factors of breast cancer, earlydetection, techniques of breast self examinationand the importance of Mammogram in detectingbreast cancer. Prof Padmalaya Devi, SurgicalOncology & Superintendent, AHPGIC delivered alecture on the theme From Screening to Survival.
