    Assessment Instructions All About Me

    Diagnostic Assessment

    Duration: 30 minutes Standards Assessed: LA.A.1.1.1.1.1, LA.A.1.1.2.1.1, LA.A.1.1.2.1.3, LA.B.1.1.1.1.1, LA.B.2.1.2.1.1, MA.A.1.1.1.1.1, MA.B.1.1.1.1.2, MA.B.1.1.1.1.3, MA.E.1.1.1.1.2 MA.E.1.1.1.1.3, MA.E.1.1.1.1.4, SS.A.1.1.4.1.1, HE.B.3.1.3 Description of Assessment Activity This diagnostic assessment is given to students to assess their knowledge of the unit standards. Teacher directions:

    1. Download the diagnostic assessment tool from below. 2. Have magnetic letters out to display to class from the board. 3. Duplicate pages 7-13 for each student. 4. Each student needs a glue stick and scissors for #20 and 25 counter for #22. 5. Obtain book: Froggy Goes to School, by London, Viking, 1996 6. Each student needs a journal in his/her desk. 7. Familiarize yourself with questioning procedures and books.

    Day of Assessment

    8. Explain to students that is an assessment to show what they know about themselves and other language arts and math standards covered in this unit. This is only to help the teacher know what to teach, so they should not worry if they dont know some answers.

    9. Read the questions and answers to the students. Circulate around the room to make sure students are following directions and keeping up with you. Make sure you model each question with an overhead transparency or with a copy of the diagnostic clipped to the board.

    10. In questions 1 & 2, use the [Froggy Goes to School] book. 11. In questions 3-17, students circle the answers or fill in the blanks. 12. Questions 18-20 are graphing. 13. Question 21 is a performance activity that the teacher must see the children use

    one-to-one correspondence. Be sure to mark their answers in the space below #23. Write the number of counters the student correctly counts out.

    14. Dont let students linger too long. Have them move on if they dont know the answers.

    Student Directions: Listen carefully to me. I will read each question and the answer choices to you. This is an assessment to show what you know. If you do not know all the answers, its ok. This will show me what to teach. 1. Do you think this book is about a frog going to school? Circle yes or no. 2. Look at page 18 of Froggy Goes to School. Circle the picture that lets you know

    they are in school. Now read the book. 3. Look at this word from the story, box. How many sounds are there? Circle your

    answer. 4. Substitute an f for the b. Write it in the blank. What word do you have now?

    Circle the correct picture. 5. Look at the words hop and top. Circle the word family that rhymes. 6. Look at the word mop. Circle the beginning letter or onset. 7. Where could you write that tells how you felt the first day of school? Circle the

    picture. Then get yours out and write a sentence or two on page 1. 7a. Since you are going to write in your journal about how you felt the first day of school, how would you begin? Brainstorm or show how you get your ideas to write on page 2. 8. To measure this book, which would you use? Circle the correct answer. 9. To measure this chalkboard, which would you use? Circle the correct answer. 10. To weigh this pencil, which would you use? Circle the correct answer. 11. What would I use to weigh this desk? Circle the correct answer. 12. There are ___hours in a day. Fill in the blank with the correct answer. 13. How many days make a week? Circle the correct answer. 14. There are ___month in a year. Fill in the blank with the correct answer. 15. Write the month of year you were born. Spell it the best way you know how. 16. Circle the best way to get your teachers attention when you dont understand

    directions. 17. Pretend an adult walks into class to talk with your teacher. What should you do?

    Circle the correct answer. 18. How many of you come to school on a bus, car, or bike? How many of you walk to

    school. Record the information in the blanks. 19. How could you group (organize) the information from #18 into a graph? 20. Interpret the graph by completing the sentence. 21. I will pass out a hand full of counters to each of you. Please count out 25. I need

    to see you counting. Scoring Method and Criteria: Each question directly relates to the unit standards. The teacher will mark the question with a check if it is correct and an X if it is not correct. Then mark the student checklist. 7 and 7a should be marked as either correct or incorrect. This will show the teacher how to adjust the instruction of this unit. Some adjustments that might need to be made would be to skip lessons that students already show mastery of, or extra time should be spent on a certain lesson that seems to be missed consistently. Use the diagnostic checklist to measure student growth after the last summative is given.
