ASR Family,

For those of you who may not know me personally yet, I want you to understand that all-star cheerleading is my passion in life. I was a football player in high school and never imagined that competitive cheerleading was in my future. But when an injury changed my path I realized I could actually love a sport more than football and I found my place in all-star cheerleading at the age of fifteen. Not a day goes by that I am not thankful to have found my calling in life at such a young age. This is what I do and I’m in it for life.

I have been coaching cheer for over fifteen years now. During that time, I have had the privilege of working alongside very talented coaches, choreographers, gym owners and athletes and am grateful for all they have taught me. It was always my dream to start my own competitive cheerleading program and I’m so excited that this dream is now a reality.

I believe that every successful journey in life starts with a vision. My vision for our inaugural season was to create a gym environment where athletes can’t wait to get to practice, parents are excited for every team in the gym and a coach is not only a coach, but a mentor and a role model. I truly believe that ASR accomplished those things last season and it is my top priority to maintain that atmosphere for all future seasons. I have surrounded myself by a staff that shares my vision and is every bit as passionate as I am.

Every athlete deserves to feel that same passion and motivation every time they walk in the gym. I want them to know that they are part of something bigger than just a cheerleading team….it’s here that athletes will form bonds and friendships that will last a lifetime. It’s here that athletes learn life lessons like discipline, teamwork and leadership that will shape them into successful adults. And it’s here that athletes will gain confidence and self respect to help them reach their full potential in life.

These positive results won’t come without a price; we expect our athletes to set goals and work hard to achieve those goals, no matter how big or small they might be. Competitive cheerleading is just that - it’s competitive. While the focus can’t be all about winning, it’s important that we train our athletes to succeed. So we will continually strive for athletes to compete at their highest level and demand their very best 100% of the time. We take the commitment it requires to be competitive VERY seriously. If you prefer to do a few cheer classes at your convenience and miss classes regularly, All-Star Revolution is not the program for you.

We will help these athletes reach their full potential in cheer, but more importantly, help them to be better individuals. ASR is a program committed to helping your child grow into a good person, and we’re using cheerleading as our tool. All-Star Revolution has a family environment that parents, athletes and coaches all take pride in.

TRYOUT INFOThe tryout process is very brief and laid back. We want all cheerleaders to feel comfortable so we do our best to put athletes at ease as we get to know his/her goals for the upcoming season. Each athlete will sign up for an individual evaluation time on one of our designated tryout dates. Athletes should arrive 15 minutes prior to his/her scheduled time slot to check-in, warm-up and stretch. Although previous experience is not required, when the athlete’s name is called he/she may be asked to perform jumps, running tumbling, standing tumbling, stunts and/or flexibility. Each athlete should attend tryouts with hair pulled up in a pony tail, wearing shorts or hot pants, lightweight tennis shoes and a t-shirt, sports bra or tank top.  To register and sign up for tryout time, each athlete must:

1) Turn in the 2012-2013 Registration Form included at the end of this packet.2) Turn in the Rules and Regulations Agreement included at the end of this packet.3) Turn in the Credit Card Authorization Form included at the end of this packet.4) Submit the non-refundable $55.00 registration fee and May 2012 tuition

payment of $130.5) Request a tryout time slot on one of the days below either upon registration in

person or by emailing ALL completed forms and fees to [email protected].

*New forms are required for every season, so even returning members must turn in all of the above listed paperwork to register for the 2012-2013 season. 

NO ATHLETE WILL BE ALLOWED TO REGISTER WITH A BALANCE.All fees must be paid in full to register for a tryout time. Accounts with a balance will not

be allowed to register and/or tryout until the account is clear – regardless of payment arrangements. No exceptions.

 2012-2013 Tryouts will be held on the following dates:Friday, April 13th 4:00 PM – 8:00 PMSaturday, April 14th 11:00 AM – 6:00 PMSunday, April 15th 11:00 AM – 5:00 PMWalk-ins are welcome at tryouts, but please understand that there could be a longer wait without an appointed tryout time. If you have a conflict on the tryout weekend and cannot schedule a time that works for you, a make-up tryout can be scheduled for an additional $25 fee by emailing [email protected].  Tryout Results:Team placement results (including practice day and time) will be posted to www.All-StarRevolution.com on Tuesday May 1st by 5 PM. Results will also be posted on the All-Star Revolution gym window.  OFFICE HOURSOur business office is open Monday – Thursday from 9am – 3pm. You can register in person anytime during those hours or by emailing [email protected]. The front desk (located in the navy parent viewing room) is open from 5pm – 9pm Monday thru Thursday and from 5:30pm – 8:30pm on Fridays. You can drop off registration forms at the front desk, but you can only receive a tryout time by emailing [email protected].  REQUIRED MEETING AND SEASON KICK-OFFThere will be a required parent/athlete meeting from 1 – 2 PM on Saturday, May 5th at the All-Star Revolution facility. We will let you know exactly what can be expected for the 2012-2013 season. Immediately following this mandatory meeting, there will be a season kick-off celebration from 2 – 4 PM. Athletes, family and friends are invited to celebrate the beginning of another successful season at ASR!

TEAM PLACEMENTAthletes are divided into teams based on two factors: age and skill level. Both factors are outlined below to give you an understanding of team placement. Age Breakdown:Show-Team Ages 3-4 as of August 31, 2012Tiny Ages 5 and younger as of August 31, 2012Mini Ages 8 and younger as of August 31, 2012Youth Ages 11 and younger as of August 31, 2012Junior Ages 14 and younger as of August 31, 2012Senior Ages 18 and younger as of August 31, 2012*These age groups are subject to change according to USASF updates. Skill Level Breakdown:Athletes are evaluated based on execution of tumbling, jumps, motion technique, stunting, and performance. All are important and taken into consideration when putting together successful teams. Preferred skills are listed for each level. All skills must be maintained to remain on any given level/team. Edgar Ruiz reserves the right to move an athlete from one team to another at any point in the season (up OR down) based on performance, ability, necessity, etc.  

Show TeamNo skill requirements-athletes must be fully potty-trained. 

Level 1Tumbling-each athlete must have a cartwheel, round off and forward roll.Jumps-each athlete must demonstrate three different jumps. 

Level 2Tumbling-each athlete must have a standing back handspring and a round off back handspring.Jumps-each athlete must demonstrate technical mastery of jumps (pointed toes, landing with feet together) including a double (whip) combination.Stunts-each student must demonstrate a level 2 stunt (examples: prep level liberty variations, extensions).Motions/Dance-each student must demonstrate an average understanding of motion technique (sharpness/arm placement). 

Level 3Tumbling-each student must have a standing back handspring series (2 or more) and a round off back handspring back tuck.Jumps-each student must demonstrate technical mastery of jumps including a triple (whip) jump combination with at least a level toe touch.Stunts-each student must demonstrate a level 3 stunt (examples: extended liberty variations, awesome full down cradle, inversion to sponge level, full up to prep).Motion/Dance-each student must demonstrate an above average understanding of motion technique and counts. 

Level 4Tumbling-each athlete must have a standing back tuck, standing back handspring back tuck, and either a running layout or a specialty pass through to a layout (example: front handspring, punch front, or whip through to layout).Jumps-each athlete must demonstrate technical mastery of jumps including a triple (whip) jump combination with an above level toe touch.Stunts-each athlete must demonstrate a level 4 stunt (examples: extended liberty variations with a full down cradle, inversion to prep level stunts, full up to extended stunts, double down from 2 legged stunt, and kick single baskets).Motion/Dance-each athlete must demonstrate an excellent understanding of motion technique and counts. 

Level 5Tumbling-each athlete must demonstrate a standing full, one to full OR two to full/double and a running specialty pass through to full or double full. *Important: athletes without standing tumbling to full or specialty pass through to full, will be eligible for level 4. Jumps-each athlete must demonstrate technical mastery of jumps including a triple (whip) jump combination with back tuck, variety and above level toe touch. Stunts-each athlete must demonstrate a level 5 stunt (examples: extended liberty variations including an overstretch with double down dismounts, full up to extended liberty variation, and kick double baskets)Motion/Dance-each athlete must demonstrate superior understanding of motion technique and counts.

ESTIMATED FINANCIAL OBLIGATIONSTo help families compensate with the expense of all-star cheer, All-Star Revolution may have parent volunteers organize occasional fundraising opportunities. These must first be approved by ASR, however ASR employees will not assist in the organization or execution of any fundraisers. In addition, no fundraisers will be permitted on ASR property.  Competitive all-star cheerleading requires a significant financial commitment. The following estimated costs may be subject to change and other expenses are not foreseen, but could arise. Absolutely no special financial arrangements will be made. Please carefully consider the financial responsibility required before committing to the season.

Reminders and emails will go out on a regular basis with details on fees and due dates. All balances for any fees owed are charged to the card on file on the 6th of each month. No account will be allowed to have fees roll over from month to month. Registration:The $55 per athlete non-refundable registration fee is due up front to hold your tryout reservation and process paperwork. No participants will be placed on an All-Star Revolution team or allowed to practice with an ASR team without having paid this fee each season. Returning members are required to pay this fee every March/April. Monthly Tuition:Monthly tuition is $130 for all competitive team members. Families with multiple athletes will receive a 50% discount off the additional sibling’s tuition – cousins, neighbors, etc. are NOT considered siblings. (All other fees will apply per athlete.) Tuition is due on the 1st of each month and can be paid by check, cash or credit card. Even if you pay by cash or check, however, a valid credit card authorization form is required on file. Any tuition fees not paid before the 6th of each month will be automatically charged to the card on file. Tuition fees not paid by the 10th of the month will be charged a $20 late fee and $10 each day thereafter. Tuition fees and late fees are not refundable for any reason. USASF Membership:The United States All Star Federation (USASF) is currently the main governing presence in competitive all-star cheerleading. USASF requires that ALL athletes certify to be eligible to compete during the 2012-2013 season. All-Star Revolution will handle all the paperwork for the certification process. The fee is $40 (to cover the registration and processing on our end) and is due May 1, 2012. Any USASF membership fee not paid by May 6, 2012 will be charged to the card on file. USASF fees not paid in full by May 10, 2011 will be charged a $10 late fee.

2012-2013Practice Wear:Athletes will be issued three full practice outfits. Practice wear changes every year, so this is required for new and returning members. The fees for this apparel will not exceed $175 before tax for female athletes (male athletes could be less.) The exact amount plus tax will be determined and posted to accounts by May 1, 2012. Payment for practice wear must be received in full by May 6, 2012 or the card on file will be charged. Any practice wear fees not paid by May 10, 2012 will be assessed a $50 late fee. Choreography Fee:All athletes are required to attend a mandatory choreography camp for his/her team. The $215 mandatory choreography fee includes the cost of choreography, instruction and music production. All members MUST pay this non-refundable fee, regardless of camp attendance, which would also include members who may join later in the season. The $215 choreography fee must be paid in full by June 6, 2012 or the card on file will be charged the balance. Any choreography fee not paid by June 10, 2012 will be charged a $50 late fee.  Uniform:We will keep the same uniform used for the 2011-2012 season. If you have an ASR uniform in good condition that still fits, you will not need to purchase a new one. ASR will not be selling any used pieces. If you have items you’d like to sell or if you’d like to purchase used pieces, you will be responsible for doing so on your own. Due to increased material costs, the price of uniforms has increased. The uniform fee is $300 plus tax for females and males ($180 plus tax for the top and $120 plus tax for the skirt or pants.) This fee is due on July 1, 2012. Mandatory uniform pieces include top and skirt for female athletes and top and pants for males. Additional items such as bows, warm-ups, backpacks, make-up, etc. are not included in this uniform cost. Payment for uniform pieces not received in full by July 6, 2012 will be automatically charged to the card on file. Any uniform fees not paid by July 10, 2012 will be assessed a $50 late fee.  Competition Fees:Competition fees will not exceed a total of $850 per athlete for the entire season and are NON-REFUNDABLE (even if the athlete quits or becomes injured.) Teams will attend 6 – 10 events per season. Competition Fees include event registration fees for the season and an additional fee to cover staff expenses. The All-Star Revolution competition schedule for the 2012-2013 season will be determined based on school squad conflicts, event location, other competitors in attendance and a number of other factors to ensure the most successful year for all teams. Once this schedule is set, the exact fee will be calculated and broken into three equal installments. The three equal payments for competition fees will be due by August 6, 2012, September 6, 2012 and October 6, 2012. Any balances left on accounts will be charged to the card on file on the 6th of each month. Each competition fee installment will be charged a late fee of $30 if it is not paid by the 10th of each month in which it is due. *Please note that the above fees do not include travel and lodging for out of town events. Warm-ups:Warm-up suits will be required for all athletes for the 2012-2013 season. If you purchased one last season, you will not need to order a new one if it is still in good condition and fits properly. Please note that warm-up and uniform pieces cannot be changed in any way…i.e. no added bling or embroidery is allowed. The cost is $150 plus tax for the jacket and pants together – both pieces are required. Payment in full for warm-ups is due by November 6, 2012 or the card on file will be charged. Any warm-up fees not paid by November 10, 2012 will be charged a $25 late fee.  Accessories:Athletes are required to purchase competition accessories such as make-up, hair bow(s), backpack, etc. The required items and exact cost plus tax will be determined and posted to accounts by November 1, 2012. Fees will not exceed $150 plus tax. The accessories fee must be paid in full by November 6, 2012 or the card on file will be charged the balance. Any accessory fees not paid by November 10, 2012 will be charged a $25 late fee.  

Any balances not paid by the 15th of each month will result in removal from the program.

ALL-STAR IN TRAININGWe understand that not everyone wants the time commitment or the financial responsibility that comes with competitive all-star cheerleading, so we have included training classes that will keep your athlete active and learning the same skills as our competitive athletes. TUMBLING CLASSES New for the 2012-2013 Season!Tumbling classes will be offered year-round for competitive ASR athletes and/or non ASR members. Classes are once a week for an hour and a half and will be scheduled based on USASF levels (and ages when necessary). The tumbling class schedule will be posted and registration will begin on May 1, 2012. To register for tumbling, you can visit www.All-StarRevolution.com and log into the parent portal or email [email protected]. 2012-2013 tumbling classes begin the week of May 7, 2012. The cost is $75 per month per athlete. No sibling discounts or proration will be given for tumbling classes. No make-up classes will be offered. All ASR holiday dates and gym closures will apply to tumbling classes. The $75 monthly rate is based on a 4-week month, so there are months where athletes will have 5 practices and others where athletes may have only 3, but the rate remains $75 per month and will not be prorated. Athletes with an unpaid balance will have to sit out of the class until the account is clear.

If you come to try the class, it will count as your official start date. Upon returning to the class, you will be responsible for payment for that entire month (from the date you first attended). You will be responsible for paying for any missed classes. General Registration/Medical release forms are required for participants who are not on an ASR team and can be found online at www.All-StarRevolution.com as well as at the end of this packet. Non ASR members will be required to pay the $55 registration fee for the 2012-2013 season upon registration.  

PRIVATE LESSONSAll-Star Revolution offers private instruction for both members and non-members of our all-star program. In other words, you do not need to be on an ASR competitive team to participate in private lessons.

All instructors provide private lessons as contract labor so you can contact them directly for pricing and scheduling details. All private lesson athletes are required to fill out a registration/medical release form (which can be found on the website or in the parent viewing area of ASR or at the end of this packet) before participating in any activity in the gym. Athletes who are NOT on an ASR team must also pay the $55 registration fee for the 2012-2013 season to take private lessons in the facility. Registration fee and release forms are good for one full season (April 2012-April 2013).

Private lessons can be scheduled for tumbling, jumps, stunts, motion technique, dance, routine memorization, tryouts and more.

HOLIDAY DATESMany other programs allow students to miss practices without accountability or regard for the ones that do show up. We are able to keep extra practices to a minimum because our practices are productive and mandatory. The following dates are available for holidays and vacations throughout the 2012-2013 year. Friday, May 25th – Monday, May 28th 2012 (Memorial Day Weekend)Wednesday, July 4th 2012 (Fourth of July)Friday, August 31st – Monday, September 3rd 2012 (Labor Day Weekend)Monday, November 19th – Saturday, November 24th 2012 (Thanksgiving)*Sunday November 25th 2012 will be a mandatory practice day for teams who practice Sundays.Thursday, December 20th 2012 – Tuesday, January 2nd 2013 (Christmas and New Years)Monday, March 11th 2013 – Friday, March 15th 2013 (Spring Break)

2012-2013TEAM COMMITMENTSAll-Star Revolution is an organized program. Athletes are expected to attend all competitions and practices as a team. We realize that many children and/or young adults are involved in a variety of activities. However, we cannot make exceptions for schedule conflicts. We expect top priority and 100% commitment and attendance.  Practices and Absence Policy:1. Each team will have a 2 hour practice once a week in the summer. During the school year each

team will keep that weekly practice and have an additional regular practice on Friday, Saturday or Sunday (depending on the team.) Extra practices could also be called on an as-needed basis. The two weekly practices and any extra practices are included in tuition.

2. The only excused absence is a school function that results in a grade. All other absences are unexcused. Part-time jobs, dances, concerts, banquets, long distance driving, family reunions, weddings, recitals, school/church socials, and any other nonrelated school activities are unexcused. Absences are NOT allowed the weekend and week prior to a competition.

3. Every athlete will be allowed to miss up to two practices during the summer.4. If for any reason there is a need to miss a practice (including the two summer absences) you must

submit the Absence Request Form at least 2 weeks in advance and it will either be approved or denied via email by Edgar Ruiz. (See back of this packet for the absence request form or you can pick one up in the navy parent viewing area.)

5. Forms left on a desk or handed to ASR staff will not be valid – you must fax or email the form to [email protected] or drop it in the drop box in the navy parent viewing area. Forms received the day of a scheduled practice will not be accepted. This includes school cheerleading, gymnastics, choir, band, and all other school sports. Your school activities are planned well in advance. Please submit your request in advance. A completed form does not automatically excuse the absence. Absences can ONLY be approved by Edgar Ruiz.

6. We will verify with school administration and the teaching staff if we think that someone is being dishonest with an absence request. Be prepared to show documentation from a teacher or instructor. You will be removed immediately if the request is falsified. No absences are permitted the week of an event. The greater the team’s attendance, the less extra practices will be needed.

7. You must attend practice when you are sick. Students can be removed from our program for failing to follow this rule. We will not make exceptions. You will not be required to participate; however, you must be present. An athlete will jeopardize his/her position if this rule is not followed. In addition, you must provide a doctor’s note with detailed information regarding the illness, treatment and length of recovery. This includes dental, medical and any other health related issue that would prohibit a student from participating. Extended sickness such as mono or strep throat could jeopardize an athlete’s position on the team.

8. If an athlete is injured, please notify ASR staff immediately. ASR staff reserves the right to replace an injured individual for some or all of the remaining events.

9. All-Star Revolution practice wear must be worn to all practices. The schedule of what outfit to wear at each practice can be found on www.All-StarRevolution.com on the calendar link. If an athlete is missing any practice wear pieces at practice, a replacement item will be provided and charged to your ASR account.

10.No jewelry can be worn in the gym at any time. No gum! Any tattoos must be covered at all times.11.We are here to work so please turn your cell phones off.12.There will be no food or drinks allowed in the gym area.13.You must arrive to practice on time. Five minutes early is better than five minutes late! Athletes

who arrive late to practices (or competitions) can expect extra conditioning after practice – regardless of the reason. Recurring tardiness could result in removal from ASR.

14. If you are not working on a skill and you arrive early to practice, please stay off the equipment, sit/stretch quietly and wait for your practice to start so as not to disturb other athletes in the gym.

15. If you are not an All-Star Revolution athlete or staff member, do not enter the gym area at any time. Parents are NEVER allowed to spot an athlete at the ASR facility or at an ASR event.

16.Parents (including team moms) should NEVER coach a team, call extra practices or provide any kind of cheer instruction for athletes.

17.No unsupervised tumbling! An instructor must be present at all times. 18.ASR reserves the right to close practices to parents or any other spectators.19.Please try and stay healthy both in and out of the gym to help prevent any injuries from occurring.20.Every athlete must be respectful to teammates, staff and parents at all times.

Competitions:1. The official All-Star Revolution competition schedule for the 2012-2013 season will be determined

and sent out no later than July 31, 2012. 2. Every athlete must not only attend every competition, but they must be available the entire

day/weekend - no exceptions. Failure to do so will result in permanent removal from the ASR program.

3. Athletes should be available to practice the entire weekend prior to AND the week leading up to any event/competition.

4. Upon arriving at competition, every athlete must be in full uniform from head to toe. This means a clean uniform top and bottom, ASR warm-ups, hair out of face and secured with the proper bow, full make-up and lashes, no show white socks, clean white cheer shoes and no jewelry, nail polish, or gum. Athletes will remain in uniform the entire time he/she is at an event – even after performing. Athletes cannot wear anything other than the official ASR uniform /warm-ups at events.

5. Remember that you are representing All-Star Revolution, all of its athletes, parents and staff. Please represent us, and yourself, well.

6. Athletes should have great sportsmanship, regardless of whether we win or lose. This includes respect for athletes, parents, coaches AND toward other programs. We want to be perceived as a class act program so please act accordingly.

7. We expect for every athlete to watch and cheer for other All-Star Revolution teams. This means you may have to arrive earlier or leave later than required for your own performances/awards. Poor sportsmanship by athletes or parents may cause dismissal from the program or other consequences.

8. At competitions the main priority of a coach is to safely prepare the athletes for performances. After that, our next priority is to support our teams as they perform. This unfortunately means that, depending on the schedule, all of our team’s coaches may not be able to stand with teams as their awards are handed out, or in rare cases, during their performances. We will do anything we can to get as many of the coaches there, but sometimes it is just not possible.

9. Some competitions may require us to compete on a Friday and may require your athlete to miss a part/full day of school. When considering competitions for our program, this is one of the first things we take into consideration. We will strive to keep our athletes from missing as much school as possible.

10. It is important for our fans to be loud and supportive for ALL of our ASR teams. In fact, it should sound like you are at a play-off game cheering on your favorite sports team. (Face paint optional!)

11.We ask, but will not require, our athletes to stay in the “team hotel” for out of town competitions. We will select “team hotels” after we release our competition schedule so that those that wish to stay in the “team hotel” can make reservations. This is a wonderful time for athletes and parents to get to know each other.

12.NCA, however, requires that all athletes adhere to the “stay to play” policy so everyone must book through their designated housing company. For NCA (and any other events that may require it) all athletes MUST make reservations through the designated booking agency. ASR will send out details on how to book these rooms after our competition schedule is released.

13. Individual room reservations will NOT be made by All-Star Revolution so each family will be responsible for their own lodging at out of town competitions. DO NOT contact ASR for questions or concerns regarding any hotel accommodations since we will not have access to that information. You should contact the hotel or housing agency directly for reservation details.

14.Teams may be required to meet or practice at the “team hotel” any time throughout the weekend of a national’s competition at the coach’s request.

15.Feel free to find a roommate on your own to cut costs. Some families prefer to travel with siblings and spouses, which is also fine. Parents will be responsible for athletes in the hotel. ASR will not assist with any rooming arrangements or finding roommates.

Choreography Camp:16.All teams will attend a 3-4 day camp at the ASR facility during the summer to learn routines.

This is mandatory for all athletes.2. Each camp will run from 9AM-4PM with an hour lunch break from noon-1pm. Snack will be

provided by ASR.3. For the safety of our athletes, they will not be allowed to leave the premises. Please be sure

to pack a nutritious lunch.4. Dates for each team’s choreography camp will be released by June 1, 2012.

I will treat my teammates, staff and other parents with respect. I will treat and talk to others in the same manner I wish to be spoken to. Pettiness, gossiping, cliques or anything that excludes certain members will not be tolerated.

I understand that if I threaten to quit or pull my child from a squad, I could be dismissed from ASR immediately.

I understand that athletes are required to wear the appropriate practice attire to every practice and if they do not, a replacement item will be issued at my expense.

I understand the importance of punctuality and will arrive at all practices and scheduled events on time.

I will not participate in negative gossip in any way. This includes on-line communication. Please remember that anything you ever say is a direct reflection on this organization.

I understand that comments about other teams and their programs will not be tolerated. We should respect their teams, as we will get respect in return.

I will strive to maintain and improve upon the skills I have and set goals to acquire more skills. I understand there must be perfection before progression. I will not attempt to learn advanced tumbling

skills before perfecting the basics. If parents and students are not patient and do not have a clear understanding of our teaching methods, they should not be cheering at ASR.

I understand that ONLY perfected skills will be choreographed into an ASR routine. I will not keep my child from practice or performing as a form of punishment. I will maintain good sportsmanship at all times when representing All-Star Revolution. Furthermore, I

understand that I must be a strong representative of All-Star Revolution at all times and a positive reflection of my teammates. Abusive behavior, lying, and or any other form of negative behavior are grounds for removal from this program.

I will not use foul language in practice, at events, or at any time while representing ASR or in ASR attire. I understand that at times an athlete will need to be moved from one team to another. I will be flexible

and understanding if such a situation arises. I also understand ASR reserves the right to change a team level or division at any point during the

season. I understand that the parent viewing area should be a positive place, and I may be prohibited from

using the parent viewing area if I cannot maintain a positive attitude or keep comments to myself. I understand that parents should not collect or request money for ANYTHING without approval from

Edgar Ruiz. Competitive cheer is an expensive sport and excessive team gifts or get-togethers may add more financial pressure for participants. ASR will organize occasional parties, gifts and/or goodie bags through your team parent ONLY.

I understand that I am making a year round commitment and will honor that commitment. I understand that I am not to contact any competition companies or their officials with any questions or

comments. If I have any questions I will contact Edgar Ruiz at 832-566-6290. I understand that uniforms are to be used only at competitions and should only be worn by All-Star

Revolution athletes. I will take very good care of my uniform. I understand that all routines and choreography including cheers, dances, stunts, transitions, and

tumbling should be highly protected and should not be shown or discussed with others. ASR choreography should never be copied or taught to anyone without consent from Edgar Ruiz. This also includes any and all portions of music. NO VIDEOS of routines, practices, competitions or any choreography should be uploaded to any online site such as YOUTUBE!

I will not try to sell any All-Star Revolution items without the permission of Edgar Ruiz. Use of All-Star Revolution logo or any part of the logo is strictly prohibited. I understand that any violation to this agreement may result in legal action.

I understand that solicitation on ASR premises is not permitted. I will not try to promote my personal business or interests while at the ASR facility or at an ASR function.

I will not hold ASR responsible for any lost or stolen items since it is not possible to monitor all valuables brought into the facility.

I understand that if I plan to drop from the program, I must give withdrawal notice to [email protected] and [email protected] and [email protected] by the 15th of any month to avoid being charged for the next month’s tuition.

CREDIT CARD AUTHORIZATION All monthly balances will be automatically charged to your credit card on file on the 6 th day of each month. If you do not want your credit card to be charged you must pay your account in full before the 6th day of the month using cash, check or through the parent portal on www.All-StarRevolution.com.
