Ms. Carrie Cobos is currently the Stroke Educator at Loma Linda University Medical Center where she develops stroke education for staff including nurses and physicians, provides community outreach for local communities and referring facilities, collects and interprets data for quality improvement projects, and develops project improvement processes utilizing six sigma. She has extensive experience in organizing and executing evidence based education, as well as knowledge of accreditation and licensing standards. Ms. Cobos over 20 years of experience as a nurse with strong knowledge of performance improvement, strategic planning, and project management.

Ms. Deborah Brehm is the Stroke Program Coordinator for Riverside University Health System. She manages The Joint Commission Disease Specific Care Primary Stroke Center providing patient care services. She ensures staff provide high quality, evidence based patient care while maintaining a safe and patient centered environment. She develops and maintains clinical standards of care and practice. She co-leads a quarterly stroke committee to target performance improvement, stroke program data and regulatory updates. She has led successful TJC stroke reviews as well TJC intracycle calls. In addition to her full time Stroke Coordinator position, she works for The Joint Commission as an intermittent Stroke Field Reviewer. Prior to her role as stroke coordinator, Deborah worked in the Emergency Department as a staff nurse, charge nurse, preceptor and mentor for 12 years. She holds a Master’s of Science in nursing with focus on healthcare education. She received a Daisy Award for extraordinary nursing in May 2014 and the Excellence Award from the Medical Executive Committee at RUHS in June 2014. Ms. Brehm maintains her NVRN-BC certification.

Vladimir Cortez, DO FACS Arrowhead Regional Medical Center and Desert Regional Medical Center Faculty Speaker No relevant financial disclosures.

Dr. Vladimir Cortez holds a position at Arrowhead Regional Medical Center and Desert Regional Medical Center. He is a neurosurgeon and has an extensive educational background. He received his DO from the Michigan State University College of Osteopathic Medicine and participated in an internship at the Riverside County Regional Medical Center. He completed his residency in neurosurgery and a fellowship in endovascular care at Arrowhead Regional Medical Center. Dr. Cortez is board certified by the American Board of Neurological Surgery and maintains a professional membership in the Congress of Neurological Surgery.

Thomas Hemmen, MD, PhD UC San Diego Health Faculty Speaker No relevant financial disclosures.

Dr. Thomas Hemmen is the Director of the UC San Diego Stroke Program. He received his medical training from the Free University of Berlin, Germany. Following a medical internship at New York University Downtown Hospital, he completed a neurology residency at New York Hospital, Cornell Medical Center and Memorial Sloan Kettering Cancer Institute in New York, where he served as Chief Resident. Dr. Hemmen is a board- certified neurologist and vascular neurologist. He has broad clinical expertise in neurology, cerebrovascular disease and neurological critical care. In 2000, he joined the UC San Diego Stroke Center for a two-year cerebrovascular fellowship. In addition, he completed a Neurological Intensive care fellowship at Philipps University in Marburg, Germany. He was appointed Assistant Professor of Neurosciences at UC San Diego in 2004 and was since promoted to Professor in 2013. Dr. Hemmen is the principle investigator of various clinical trials in cerebrovascular disease investigating the use of neuroprotection and reperfusion therapies after acute ischemic stroke, as well as stroke prevention, recovery and research in intracerebral hemorrhage. He provides leadership and education to medical students, residents and fellows in cerebrovascular research and therapies and was instrumental in establishing UC San Diego as the first Certified Stroke Center of Excellence in San Diego County. He leads the cerebrovascular research and trial coordination center that conducts national and international collaborative trials, researching for new cures in all areas of cerebrovascular medicine.

Mary Kim, MD Loma Linda University Health System Faculty Speaker No relevant financial disclosures.

Dr. Mary Kim is a physiatrist and the Director of Stroke Rehabilitation at Loma Linda University Health System. Dr. Kim is a graduate of Loma Linda University School of Medicine. She completed her residency in Physical Medicine and Rehabilitation at the Rehabilitation Institute of Chicago/Northwestern University in Chicago, Illinois, where she served as a chief resident during her training. Afterwards, she worked for the Center for Neuroscience and Regenerative Medicine (CNRM) at the National Institutes of Health (NIH) Clinical Center. Dr. Kim’s current practice specializes in the care of patients recovering from strokes, including spasticity management. She is also the residency program director of the Physical Medicine and Rehabilitation residency training program and an assistant professor at the Loma Linda University School of Medicine. She is board certified by the American Board of Physical Medicine and Rehabilitation in physical medicine and rehabilitation and brain injury medicine.

Ruby Koshy, MBBS, MD Riverside University Health System Planning Committee No relevant financial disclosures.

Dr. Ruby Koshy is a board-certified neurologist and Chief of Neurology at Riverside University Health System. Through her passion and dedication to serving her patients, Riverside University Health System achieved the designation as a Joint Commission certified Primary Stroke Center. As a part of the hospitals exceptional efforts, a multidisciplinary stroke team, led by Dr. Koshy focuses on early identification and treatment of acute strokes to ensure improved changes of survival and recovery outcomes, including further stroke prevention and education. Dr. Koshy, also specializes in diagnosing and treating other disorders affecting the brain and spinal cord, such as multiple sclerosis, epilepsy, headaches and movement disorders. She leads the Clinical Neurodiagnostic services at the hospital, providing inpatient and outpatient services for Electroencephalograms (EEG’s) to evaluate for seizure activity, as well as Electromyography studies (EMG) to evaluate diseases of peripheral nerves. She serves as an Assistant Professor of Neurology at Loma Linda University School of Medicine and is actively involved in research. Dr. Koshy completed her residency in neurology and fellowship in clinical neurophysiology at Loma Linda University Medical Center after earning her medical degree from Kasturba Medical College in India. She is a member of the American Academy of Neurology.

Matthew Pond, MD Loma Linda University and VA Loma Linda Healthcare System Faculty Speaker No relevant financial disclosures.

Dr. Matthew Pond received his undergraduate degree from the University of Utah and attended medical school at George Washington University. After graduation, he completed his internship at National Naval Medical Center in Bethesda, Maryland. He then completed additional training in Pensacola, Florida where he became a United States Naval Flight Surgeon. After serving for several years as a flight surgeon in Texas, Dr. Pond returned to civilian life and entered his diagnostic radiology residency at Loma Linda University. He then completed his fellowship training in Neuroradiology and Neurointerventional Radiology at Loma Linda. He currently is an Assistant Professor of Radiology, Division of Neurointerventional Radiology and Neuroradiology. His academic and clinical interests include stroke imaging and interventions, intracranial embolization, carotid stenting and vertebroplasty.

Christina (Crista) Qi, PharmD, BCCCP, BCPS Riverside University Health System Faculty Speaker No relevant financial disclosures.

Dr. Christina Qi has worked for Riverside University Health System for the past 7 years. She began working there as an Inpatient Clinical Pharmacist, assisting in the medication management of patients in the multidisciplinary care team. In 2011, she began working in the Emergency Department to help care for patients of various acuity of illness, especially with a focus on the critically ill with complex medication needs. Her additional responsibilities included the development and growth of pharmacy services in the ED: expanding new services such as Pharmacy Technician driven Medication History Collection prior to or during triage, developed treatment protocols with physicians and nurses, and drove the expansion of pharmacist coverage in the ED. Dr. Qi currently serves on several hospital-wide committees working with physicians, nurses and other members of the healthcare team to improve patient care. In the past two years, Dr. Qi had taken on the development of a new Pharmacy Practice Residency Program, and is now the Pharmacy Residency and Academic Program Director. Dr. Qi worked previously at the City of Hope as a research associate in translational research for cancer cell molecular targeting after obtaining her Bachelor’s degree in Molecular and Cell Biology – Biochemistry at the University of California at Berkeley. After which, she attended pharmacy school at University of Southern California. She currently serves as faculty at the University of the Pacific Thomas J. Long School of Pharmacy, and coordinates Advanced Pharmacy Practice Experiences for student pharmacists.

Ms. Hope Read is a Community Relations and Event Planning professional at Riverside University Health System. Serving with both the Marketing department and the Foundation, she works to create relationships within Riverside County to promote the many things being done at Riverside University Health System. Before coming to Riverside University Health System, she served in higher education where she was an integral part of a startup division for an online education program designed for non-traditional learners. She also served in other capacities including finance coordination for Title IV regulation and, also worked on a team to create the university’s first online academic advising module. With over twenty years of recruiting, community involvement, finance, and event experience in fast paced environments, she is able to navigate and maneuver through challenges with professionalism. She enjoys the demand of a continuum of change in the health care industry.

Ms. Janis Rustad is currently Chief Nursing Officer at Riverside University Health System, providing leadership to the Moreno Valley and Riverside campuses. She is a Registered Nurse with an Associates Degree in Nursing from Los Angeles Harbor College and a Bachelors of Science in Nursing at Statewide Nursing Program, California State University, Dominguez Hills. She achieved a Masters of Science in Nursing at Western Governors University. Prior to her four years at Riverside University Health System, Ms. Rustad was in a variety of nursing leadership positions in the last 17 years at Rancho Springs Medical Center, Inland Valley Medical Center and Kapiolani Medical Center in Honolulu. Navdeep Sangha, MD Kaiser Permanente Los Angeles Medical Center Faculty Speaker No relevant financial disclosures.

Dr. Navdeeep Sangha has worked for Kaiser Permanente Los Angeles Medical Center since 2012. He began working as a Vascular Neurologist, and later became the Co-Assistant Chief of Neurology. He is also the Director of the Kaiser Permanente Southern California Tele-Stroke Program. Dr. Sangha received his Bachelors of Science in Biology from University of California, Irvine and graduated from Dayanand Medical College and Hospital in Punjab, India. He then went on to become a Post Graduate Trainee in Adult Neurology at Kaiser Permanente, Los Angeles Medical Center from 2007 through 2010, and later completed his Stroke Neurology Clinical and Research Fellowship from 2010 through 2012 at the University of Texas at Houston, Texas Medical Center. Dr. Sangha is a Diplomat of the American Board of Vascular Neurology, as well as a Diplomat of the American Board of Psychiatry and Neurology. He is currently an abstract grader for the American Academy of Neurology, Stroke Division, and he has also published a large array of publications regarding stroke. Dr. Sangha is currently involved in a wide range of different clinical research topics including, “Optimizing Patient’s Selection for Endovascular treatment in Acute Ischemic Stroke (SELECT): Local PI”.

Dr. Reza Vaezazizi has over 30 years of experience, and is currently the Chief of Services/Medical Director at Southwest Healthcare System and the Medical Director of Inland Counties Emergency Medical Services Agency. He received both his Doctor of Medicine and Master of Science in Applied Physiology from Finch University of Health Sciences/The Chicago Medical School. Dr. Vaezazizi is a Diplomat of the American Board of Emergency Medicine, as well as a Fellow of the American College of Emergency Physicians. He is a Board Member of the Riverside County Medical Association and is a part of the Trauma Audit Committee at Riverside County Emergency Medical Services Agency. Ms. Noelani Warren has had a nursing career for 28 years with a focus in Neuro ICU for 15 years. Ms. Christina Garcia was born in McAllen, Texas, and was raised in Bakersfield, California with her adopted parents. She currently resides in Southern California with her husband of 18 years. Raising her 5 children and 5 grandchildren and owning a construction company lead to a very busy life where there was a never a dull moment. Due to her busy life, she was not aware of her health. Ms. Garcia was overweight, her blood pressure was out of control, she had high cholesterol and high sodium intake. Then on May 21, 2015, her life was forever changed at the age of 46 as she suffered a stroke. After surviving a stroke, she wanted to give up many times, but with the amazing support of her husband, children and grandchildren, she was determined to change her lifestyle and get healthy. Almost 2 years later, she wakes up every day appreciating everything in life so much more and is grateful for a second chance in life.

Ms. Lori Hoopingarner is a wife of 18 years and a mother of 3. She owns a franchise of Ameriprise Financial, helping people achieve their dream of retiring as a Financial Advisor and Wealth Manager for the last 20 years. Ms. Hoopingarner is very active in her community. She is president of the Professional’s Women’s Roundtable in Temecula, California and sits on the Advisory Board to the Temecula Valley Hospital. Most recently, she is also a stroke survivor.

1. Select the activity type (All, Live, or Online).

2. Enter a partial activity name or keyword.

3. Enter the activity location address, city, or zip for live activities

4. Click the magnifying glass icon.

5. Click Register for the selected activity.

b. Click Register

c. Enter your Username and Password, and click Sign In.

1. Of you do not remember your Username or Password, click the Forgot password? link.

2. If you are not a registered user, click Create an account to enter your account information and receive your Username and Password.

d. You may be prompted to enter additional registration information.

e. Enter the access code provided at the end of the session in the Access Code field.

f . Click Submit to complete the enrollment.

4. Once enrolled, the activity completion pathway automatically displays.

5. Review the Activity Overview tab, scroll to bottom and click Continue.

a. You may be required to check a confirmation box that you have read the information.

6. Click the Launch button to complete an evaluation on the course. This step is required to claim credit. Once

complete, click Continue.

7. Claim your credit by clicking the Claim button for the appropriate accreditation.

a. You may qualify for more than one credit type based on your user profile.

b. ACCME, ANCC, and CAPCE allow claiming of variable credit. You will be given the option to claim all

or part of the activity credit if applicable to your user profile.

8. Click Continue to generate your certificate.

9. Click the certificate link to print or save it.

10. Click Close. The activity is stored under Completed Activities.
