Change is as much a part of business as it is a part of life. From farming and manufacturing to education and technology, the ability to adapt positively to change is a requirement for success in any American industry. Nowhere is this more evident than in healthcare.

Within the last twelve months, the federal government enacted historic cuts to Medicare while at the same time implementing the quasi-public health insurance exchanges required under the Affordable Care Act. As a result, all healthcare providers have had to constantly rework, retool and redesign the way in which they deliver care. While many hospitals and healthcare systems are shrinking, I am proud to report that Hospital for Special Care (HSC) continues to grow.

Over the past year, HSC has launched new programs for both Parkinson’s disease and Chronic Obstructive Pulmonary Disease, and our outpatient Autism Center continues to grow. We have hired a new physician to oversee our Movement Disorders Clinic and more than a dozen other clinicians to work in our inpatient and outpatient programs. We were recognized as one of the Hartford Courant/Fox CT “Top Workplaces” for 2013. The Business Council of Fairfield County recognized HSC as a Gold award winner for our successful efforts in promoting a healthy workplace, and we received statewide recognition for our spotlight on autism in daily life with our Spectrum of Kindness initiative spectrumofkindness.org.

The most pervasive changes are in how and where care is delivered, which spurred HSC on to tremendous growth. In fact, the hospital now has served patients from more than 90% of the cities and towns across Connecticut. That growth required more space; earlier this month, we officially opened The Maximilian E. & Marion O. Hoffman Outpatient Center at our main campus in New Britain. This $5.9 Million project was constructed in less than one year and brings a new modernized design to the oldest part of the original 1941 hospital building. Simultaneously, construction continues on a $600,000 indoor riding arena for our Manes & Motions Therapeutic Riding Center in Middletown. The addition of an indoor riding arena opens the doors for improved and expanded services for our riders without seasonal interruption.

Though the landscape of Connecticut’s healthcare system continues to change, Hospital for Special Care remains committed to both our independence as an institution and our willingness to partner with every acute care hospital (and health system) in Connecticut and beyond. The service we provide our patients and the value we bring to the state of Connecticut will continue to be our hallmarks in the months, years and decades ahead.

Dear Friends,It should come as no surprise that the Hospital for Special Care is recognized locally, regionally and nationally as a center of excellence for our various programs in specialty care. This did not happen overnight or without challenges, but rather by nimbly responding to the changing needs of our community and the patient populations we serve without conforming to geographic borders.This year, we recognize our President & CEO, Dr. John Votto for his 25 years of service at HSC. In 1990, Dr. Votto joined HSC as Chief of Pulmonary Medicine. He was promoted to Vice President of Medical Affairs in 1992 and Co-President & CEO in 1997. Under Dr. Votto’s leadership, HSC has grown into the nation’s only specialty hospital serving adults and children across a spectrum of complex care and rehabilitation. Dr. Votto’s presence on Capitol Hill as a recognized authority on long-term acute care and his service as past president of the National Association of Long-Term Hospitals (NALTH) has significantly contributed to HSC garnering national recognition and respect. Locally, Dr. Votto continues to serve the community through his work with the VA Connecticut Healthcare System in Newington. Amongst his accolades, in October of 2013, I had the privilege of attending a ceremony in which Dr. Votto was honored with the Hartford Business Journal’s prestigious lifetime achievement award, in recognition of his career in healthcare. Please join me in congratulating Dr. Votto on his tireless service, success and well-deserved honor.I also wish to recognize the employees of HSC for their incredible service and dedication to our mission. The state of the healthcare environment is impacting how we provide care more than ever and our employees have embraced the need for more efficient care while maintaining superior patient outcomes and safety. With this in mind, HSC was recognized as one of the Hartford Courant/Fox CT “Top Workplaces” for 2013. The hospital board and senior management are proud of this recognition as it was an employee who nominated HSC. As we share inspirational stories of our patients, I continue to believe their success would not be possible without the dedication of our caregivers and support staff.That dedicated care will continue to grow, consistent with our strategy, as we open The Maximilian E. & Marion O. Hoffman Outpatient Center, allowing for greater neuromuscular, pulmonary and physical rehabilitation services. This project is the culmination of our five year “Hope Campaign” and could not have been completed without the generosity and support of our community, employees and friends. My sincere thanks and appreciation to all who participated in this project.With the support of a $3.6 Million grant from the State of Connecticut, the integration of the electronic medical records (EMR) system is on track. Serving as the foundation of a patient-centered specialty practice (PCSP) at our Autism Center, (and soon thereafter for other populations), this system will provide the program data critical to identifying the long-term value HSC provides the healthcare marketplace. This reinforces the message we continue to hear about the future of healthcare reimbursement regarding transparency, quality outcomes and the availability of data.As we face uncertainty and challenges ahead, HSC is well-positioned as an independent provider of a unique continuum of care that other hospitals and community groups value when referring patients. We will continue to look at ways to optimize strategy, programs and sustainability so HSC can better serve those individuals no other provider serves. In closing, I’d like to acknowledge four retiring board members for their extensive service to HSC: Todd Bower, Leslie C. Clark, Rita E. Grygus and Sister Mary Mark Pizzotti. Let me thank you and fellow board members for your support, commitment and stewardship. Together we move forward always focused on achieving our mission.

Stephanie Marquez is a natural, gifted athlete. Now, she is a World Champion born out of Hospital for Special Care’s Adaptive Sports Programs, coupled with her own inherent ability, determination, passion and perseverance for her sports. After her first year, Stephanie qualified and participated in the National Junior Disability Championships in 2007. In August 2013, Stephanie represented the United States in the Junior World Championships, earning four medals in five events, including the gold medal in discus.October 11, 2014

2013 GOALS• Provide exemplary complex care in seven areas

of specialization: Pulmonary Care, Acquired Brain Injury, Medically Complex Pediatrics, Neuromuscular Disorders (including ALS research and a newly-opened Parkinson’s Disease & Movement Disorders Center), Spinal Cord Injury, Comprehensive Heart Failure, and the diagnostic assessment, treatment and consultation for children and adolescents with Autism Spectrum Disorders.

• Deliver quality complex care and rehabilitation in one easily accessible outpatient setting by completing the $7.4 million Hope Campaign and construction of a new 11,950 square-foot facility.

• Having reached our fund-raising goal, commence construction of an indoor riding arena for Manes & Motions Therapeutic Riding Center, Inc., allowing the program to become year round. Manes & Motions offers a horseback riding program for individuals living with special needs, including children with Autism and Cerebral Palsy and veterans with Post Traumatic Stress Disorder and Mild Traumatic Brain Injury.

• Implement a $3.6 million electronic medical record in-kind grant made possible by the State of Connecticut, a necessary step towards certification as a patient-centered medical home.

• Manes & Motions Dinner & Auction “The Mane Event” at our new indoor riding arena on September 19, 2014.

• Support Team Special Care at the 2014 NU Hartford Marathon on October 11, 2014.

• Hospital for Special Care is the beneficiary of several third party events hosted by friends and supporters of the hospital.

RHANA’S JOURNEY Pure Sweetness and DeterminationSome journeys are planned, some just happen. Rhana’s journey started unexpectedly on December 15, 2012. A champion gymnast at the age of 12, Rhana was running to warm up for gymnastics like she had done a hundred times before but, something was different this day. Her arms grew heavy and she had trouble breathing. “We got a call from the gym that she was sick,” said Rhana’s father Robert Fleming. “We didn’t really know how sick. We didn’t really think it was catastrophic sick.” “I said ‘was there an accident?’ ‘Did she fall?’ and they said, ‘no, we were just doing warm-ups’,” her mother Cheryl Fleming added.Roughly one hour later Rhana was at the Emergency Room at Hasbro Children’s Hospital in Providence, Rhode Island. She arrived by ambulance on a body board with neck support. After a long, painful 3-hour wait on a stretcher in the emergency room her body was becoming paralyzed (during which she lost her ability to speak, began losing movement in her legs and her breathing was deteriorating rapidly - all while in excruciating pain). She was finally taken to be examined in the Emergency Room. She stopped breathing less than two minutes after they took her for examination. At that point she was in critical condition. She was intubated, paralyzed from the neck down and unable to breathe and in extreme pain, but fully aware of the surroundings with her eyes open. They admitted her to the Pediatric Intensive Care Unit. The doctors at Hasbro diagnosed her condition as Transverse Myelitis (TM). TM is an inflammation of the spinal cord which can affect everything from movement to swallowing to involuntary systems like breathing. Rhana had symptoms in all three areas. She was unable to move, needed a ventilator to breathe and was in constant pain.Once at the hospital, she started to respond to treatments about ten days later when she moved her right foot for the first time. Over the next few weeks came many milestones, each reported by her parents on their website rhanasjourney.com. In January 2013, Hasbro doctors recommended the Flemings consider Hospital for Special Care for Rhana’s rehabilitation. They drove over and reported on their website that “With many pools, gyms, library and media rooms it’s a place built to promote healing. It’s open and airy and most of all it is QUIET.” What made an even bigger impression of her parents, though, were the people they met here - both staff and patients. They noted that here Rhana would meet and talk with people who actually know what it is like to face the challenges that lay before her.

So, on January 25, 2013 Rhana arrived at HSC. Within days, she invited another patient (also 12 years old and in the sixth grade) in her room to watch the movie “Annie” as they ate chocolate chip cookies. These girls became great friends and to this day keep in touch via email and text messages. The next few months were filled with a lot of firsts, some setbacks and then some more progress. On February 4th, Rhana went off the vent for 45 minutes, the first time she had breathed on her own since this all started. That same week HSC got her an iPad and stylus so she could communicate and control her own choice of movies (“The Shining” was the first). A few days later she got her first electric wheelchair that she could control through a breathing tube. Within a few weeks she was routinely off the vent for 12-16 hours a day and long dormant muscles were coming back. In the middle of her challenges, Rhana gave us all proof of what she is capable of. Here is the story in her parents’ words:

Today my daughter showed me what it means to be selfless. There is a new patient in rehab. She didn’t want to do her PT, to get up and walk a few steps. Her mother and therapist were trying to coax her and Rhana wheeled up to her and said “You can do this. I’ll walk with you, you got this. We’ll do it together.” The girl got up. She walked many feet with Rhana right by her side, coaching and cheering her every step. I was amazed and so proud of my strong, beautiful daughter.

We all are.By early May, Rhana had been off the vent for as long as 30 hours, and on May 16th she was off the vent for good. By the end of June, Rhana returned home for the first time since December. She has returned to school three days a week while getting back involved with her friends at school and gymnastics. Rhana’s challenges continue but she goes about life undaunted by them. Whether it is the school dance or having her girlfriends over for a sleepover, Rhana’s determination and pure sweetness shine through. With a little help, she already walks out to get the mail, paints beautifully with her right foot and seems to be unencumbered by worry regarding her challenges.When Rhana and her parents agreed to come back to join us for the Annual Meeting, she had a long list of friends (both patients and staff), she wanted to see. And they all were just as excited to see her.

She enjoys helping each and every patient achieve personal and social wellness as they live with a chronic illness. Lorraine devotes her personal time to shop, alter and make clothing items so each patient looks their best to help their overall self-esteem. She encourages patients to take advantage of available therapeutic activities and helps each patient attend an activity that they enjoy. Her customer service exceeds the requirements of her job. She places her unit, peers and patients as number one. Lorraine has been the person who has made it through each snow storm to be there for the hospital community. Her actions set a positive tone for staff relationships and enhance the quality of life for patients.
