Emergency Medicine Foundation Program (EMFP) is a joint venture of Royal college of

Emergency Medicine (RCEM), UK and Rawalpindi Medical University (RMU), Pakistan. Main

objective of the foundation program is to improve the standards of Emergency Department by

providing quality training to emergency physician. Memorandum of Understanding was sign

between two institutes and the program was launched in July-August 2019 for the period of one

year. Training site was decided to be Emergency Department of Holy Family Hospital,

Rawalpindi. 14 supervisors were nominated from the faculty of RMU and were two train-the-

trainers workshops were conducted by RCEM. 28 trainees were inducted for one year training

program. Most of inductees were from RMU and allied hospital while others were from different

institutions or clinics.

EMFP curriculum for trainees and their assessment guidelines were published by RCEM.

Supervisor’s course guidelines and assessment methodologies were accessible. With the

launched of program, teaching and training was started. NHS eportfolio was activated for EMFP

trainees and supervisors and RCEM learning resources were accessible to trainees for updated

and integrated learning. Monthly webinars were conducted by RCEM for trainees and

supervisors to discuss their difficulty in the training and development.

28 trainees were inducted that worked in Emergency Department of Holy Family hospital,

Rawalpindi for one year training program.

Train The Trainer’s Workshops

Two train the trainers workshops were carried out by Royal College of Emergency Medicine to

make sure quality supervision of trainees. First workshop was on 18-19 June 2019 and

supervisors were trained about EMFP curriculum, educational & clinical supervision, eportfolio

and effective feedback mechanism. RCEM team consisted of Dr. Jason Long, Dr.Ayyaz Abbasi

and Ms.Emily Beet. Second workshop was conducted on 17-18 December 2019 with RCEM

compromises of Dr. Emma Fernandez, Dr.Amanda Farrow and Dr.Ayyaz Abbasi. RMU team gave

feedback over the training program and highlighted the deficient areas especially in WPBAs and

eportfolio.

EMFP Teaching & Training

Overall trainees were satisfied with teaching and training program. Teaching was in the form of

contact and clinical sessions. Core knowledge was delivered on weekly contact session while

professionalism, clinical skills and case handling were discussed in clinical sessions. Contact

sessions were conducted by senior RMU faculty and clinical program was carried Emergency

Patient safety

Patient safety is given prime importance in the emergency department, similarly each trainee

is also provided safe working environment. Identification of vulnerable patient is carried out

ED stay. EMFP trainees are bound to discuss each case with seniors fellows before discharge.

End of shift handover is mandatory step to ensure continuity of care. 24 hours consultant

supervision in ED is available for smooth and safe working.

Training Protocols

EMFP is an organized training program under direct supervision of its clinical and educational

supervisors. Trainees are inducted according to Rawalpindi Medical University induction

procedure i.e. advertisement, applications, scrutiny, selection, interview and induction.

Introduction to one year trained program and their aims & development plans are discussed in

orientation session. Trainees are monitored and accessed by EMFP supervisors. Trainee’s

capabilities are graded according to EPAs (Entrustable Professional l Activities). WPBA

(Formative and summative), DOPs are carried out throughout training period. Trainees have

mandatory educational supervisors meeting to discuss their concerns and update them for

their program and short comings. 360˚ degree evaluation is done by multi source feedback

mechanism. Structured training report and end of Programme report are generated annually

and discussed with each trainee about the performance.

Educational supervision

An educational contract signed by educational supervisor and trainee is necessary part of

documentation. This contract makes education supervisor responsible for ensuring his/her

trainees have clear understanding about the course, training methodology, developmental

needs and competencies to be achieved. Regular meetings ease the trainees to discuss their

concern with positive outcome.

Clinical supervision

24 hours clinical supervision is provided to EMFP trainee. All clinical supervisors have been

trained by RECM train the training workshops. Annual supervision schedule of CSs is provided

and is work in organized accordingly. Trainee’s clinical skills and professional development are

accessed and recorded according to competency level. Clinical supervisors provide positive

feedback during the daily WPBAs and identify the trainees that need extra support. Such cases

are discussed with respective education supervisor followed by ES meeting to sort out

trainee’s issues and difficulties. In case of difficulty for CS, ES and EMFP incharge look into the

matter to way out the challenge.

Working environment

Respectable and safe working environment provided to the trainee with access to life

essentials, learning facilities and contact details of their supervisors. Trainees are provided on

monthly bases for their monthly work rota in ED, core knowledge and skill development

activities along with their work place based assessment schedule.

Escalating trainees concerns

Trainees are divided in groups and their group leaders are chosen for better communication of

their mates. All the trainees have to access to RMU EMFP head office to bring any issue they

face while induction, training and during evaluation. Depending upon nature of issue,

appropriate feedback is given either individually as in group.

Supporting trainees

‘’ Trainee in difficulty’’ is one of the important concern that has been identified, discussed in

webinar and incorporated in RMU teaching. Clinical supervisors regularly monitor and access

trainees for their progress and identify their problems. Then ES meeting with trainee is aimed

to provide appropriate support and maintain quality training.

Management concerns

20% trainee and 16% of supervisors experience management issues during induction and

registration phase. Up to 10% faced problems in the 1st quarter, mainly clashes of EMFP session

with their work station duties at the same time. Some faced communication issues among

trainees and supervisors. 5% trainees faced difficulties in finding the classroom and their

respective supervisor. 10% trainees had financial problems and were happy with the financial

assistance provided by RMU. Support from management team was 100% with positive results.

RCEM Support

Trainees were satisfied with the support of RCEM in foundation program. They considered RCEM

learning resource and webinars were the best achieving 98% and 94 % satisfaction level

respectively. 93% trainees were happy with RCEM communication and 90% with technical

support. e-portfolio was most difficult task for both trainees & supervisors inspite of webinars

and hands on workshop

EPRP – End of Program Review Panel

EPRP is the final authority to decide for EMFP certification. Decision of awarding the certificate

depends upon the ES and CS annual report, multisource feedback, personal development plan,

trainee’s progression evidences and achieving desired EPA levels. The meeting is jointly

conducted by RCEM & RMU team and final outcome is declared.

Supervisor Appraisals

At end of year training, educational and clinical supervisors were reviewed for their professional

development and reflection on the supervisory skills.

These annual appraisal meetings were jointly conducted by RCEM & RMU team and final

outcome was declared as RCEM-EMFP educational or clinical supervisor. These meetings took

place online due to travel restriction. The scope of appraisal meeting focused mainly on

development as supervisors that with key components as
