Annual Report Form

PURPOSE

The purpose of the Annual Report is to:

Multiple Programs within the Same Facility NAEYC Accreditation is granted to the overall program, and will not be granted to individual classes within a program. Throughout the NAEYC Accreditation process, all eligible classes a program serves must be reported and may be observed during a site visit. This includes classes within the program that operate during the summer and as after-school care.

A class can only be excluded from a center's NAEYC Accreditation only if it is part of a separate program that has a separate public identity. A program pursuing NAEYC Accreditation must notify NAEYC of all separate programs that operate within its facility and be able to demonstrate a separate a separate budget, administration, and license.

Section 2: PAYMENT INFORMATION

This form will not be processed until all applicable fees are paid in full. If payment is not received in full within 90 days, NAEYC reserves the right to withdraw the program from the accreditation process.

All fees are nonrefundable.

The accreditation fee is calculated based on the number of children enrolled in a program at the time this form is submitted. If the number of children changes for a defined portion of the year (for example, altered program operation during the summer), the program should report the number of children that typically applies throughout the majority of the school year. Reference the following rules to determine the number of children that determines the accreditation fee for your program.

Each child is only counted once. Each child of eligible age (birth through kindergarten) that is part of an eligible class is counted. All eligible classes MUST be included in

your program’s NAEYC Accreditation. Programs with Before & After-Care should only be included if program has NAEYC School Age Accreditation. For programs with hourly care or drop-in care classes in which the total number of children enrolled in the class exceeds the maximum

licensing capacity of the class, only the maximum licensing capacity of the class is counted.

To be eligible for NAEYC Accreditation, programs that are eligible to be licensed or regulated in their jurisdiction must in fact be licensed or regulated or must be the in process of becoming licensed/regulated. Programs that are license-exempt and not otherwise regulated must voluntarily seek licensure. Administrative Staff Qualifications Option Guide

Use the guide below as a reference to indicate the qualifications of the administrative staff member(s) within your program. One person may meet the qualifications for both the Operational Administrator and the Pedagogical Administrator (one person meets A or B AND C or D), or two separate individuals may meet as either the Operational Administrator or the Pedagogical Administrator (one person meets A or B and a second person meets C or D). Operational Administrator: The administrator responsible for the operational leadership of the program, including functions such as business leadership, facilities management, legal and fiscal management, marketing, and use of technology. This administrator may have a title such as director, site manager, program manager or principal. Pedagogical Administrator: The administrator who provides program leadership in instructional matters, such as developmentally appropriate practice, curriculum and instruction, child assessment; and inclusion practices. This administrator may have a title such as director, assistant director, curriculum coordinator, or master teacher. Option If the Operational Administrator has this

10B.16 Evidence of licensing/regulatory status

Show that your program and your facility has a license to operate, or is regulated by an applicable public regulatory system. Your license or regulatory documentation indicates your program is in good standing with your regulatory y.

Evidence needed to meet this assessment item may vary from program to program. A fully valid and current license completely meets the assessment item and no additional evidence is needed to support “good standing” (see definition below). If there is some question about license status, programs must present additional evidence to support “good standing”. Programs that are license-exempt and not otherwise regulated need to execute a NAEYC “License-Exempt Acknowledgement Form” in order to meet this item. Such programs do not need to offer evidence of good standing, because they cannot. Regulated: Programs that are not licensed by a state government but are under the regulation of a public agency, such as Department of Defense or a board of education. Good standing: The program has a fully valid and current license or certification. Suspensions or revocations of licensing or regulatory statuses are NOT accepted as evidence of good standing. Examples of acceptable licensing/regulatory evidence: Full and current license or regulatory certificate, correspondence from licensing or regulatory body showing good standing or licensing extension, web link to state public licensing reports of program status, NAEYC “License-Exempt Acknowledgement Form”.

Required Documents for the Site Visit

The items below are required, and are assessed in the Program Portfolio. Failure of these items will cause a program to fail the entire assessment. Self-assess your current documentation to assure that each bullet point is addressed. If you are unsure whether your documentation is sufficient, call your NAEYC Program Support Specialist for assistance.

1B.10 Critical part of guidance/discipline policy Show that your guidance/discipline policy does not include any circumstances when it is permissible for staff to use any form of physical punishment, psychological abuse, or coercion when disciplining a child. Appropriate use of restraint for safety reasons is permissible.

Examples of physical punishment: shaking, hitting, spanking, paddling, slapping, jerking, squeezing, kicking, biting, pinching, excessive tickling, and pulling arms, hair, or ears; requiring a child to remain inactive for a long period of time. Examples of psychological abuse: shaming, name-calling, ridiculing, humiliation, sarcasm, cursing at, making threats; frightening a child; ostracism, withholding affection. Examples of coercion: rough handling (shoving, pulling, pushing, grasping of any body part); forcing a child to sit down, lie down, or stay down, except when restraint is necessary to protect the child or others from harm; physically forcing a child to perform an action (such as eating or cleaning up).

are not eligible to submit via email.

NAEYC accepts the postmark date as the submission date. NAEYC recommends that programs obtain written confirmation of receipt of all forms sent to NAEYC P.O. Boxes. NAEYC is not able to sign for materials that are delivered to a P.O. Box. Copy this form for your program’s records before submission. NAEYC will not return this form to the program.

Responsibility: To update NAEYC of any major changes related to the program, eligibility requirements, and critical incidents that may impact a program's quality.

Responsibility: To adhere to selected timelines for submission deadlines to avoid any penalty or lapse in accreditation.

Responsibility: To submit all applicable fees and forms to purse or maintain NAEYC Accreditation and avoid withdrawal from the accreditation process.
