CHANGES TO THE PROJECT MANUAL

1. Section 004100- Bid Form

a. Under Section 004100, delete the Bid Form (3 pages) as included in the Project Manual. Replace withthe revised Bid Form (3pages) as included in this Addendum. Note that the revision to the Bid Formis the addition of a line for the Bidder’s price for Alternate No. 2 as described on the Bid Form.

a. Asbestos removal / abatement (including, but not limited to, the existing vinyl tile throughout the Naveand the area of the new restroom.

7. Performance and Payment Bonds to be furnished by the General Contractor are indicated in the Notice toBidders as part of the Base Bid.

8. Subcontractors and Products List Requirements - refer to Section 00 4333 of the Project Manual for descriptionof Subs & Products List to be submitted as follows:a. Each General Contractor bidder is to submit with their bid one copy of the Prime Subcontractors and

Products List. b. Each bidder, shall furnish one copy of the Complete Subcontractors and Products List to the

Architect’s Office by 9:00 a.m. on the morning following the bid opening. (Fax or email submissionis acceptable.) The selected Contractors shall then submit a fully completed, typed list with allsubcontractors and products listed for inclusion with the contract.

9. Substitutions will be considered if written request is made at least 10 calendar days prior to the bid date andsubsequently approved by the Architect in an Addendum. a. These requests must be received by the Architect no later than the end of Friday, May 8, 2015.b. Utilize the substitution forms included in Division 0 Section “Project Forms.”

10. Requests for clarification or interpretation of the Bid Documents must be made in writing and be receivedby the Architect at least 10 calendar days prior to the bid date .a. Architect must receive such request not later than the end of Friday, M ay 8, 2015.

11. If there is a discrepancy between Drawings and Specifications not clarified before bidding, the better qualityor greater quantity of Work shall be provided per the Supplementary Conditions - Specification Section 00 7300.

12. Review Scope of the Work.

a. Work at the existing Church Building:i. Excavation at the north and east exterior faces of the Church Building to install new

waterproofing at the face of the foundation wall.ii. Installation of perimeter foundation drainage system at the excavated building faces.iii. Backfill of excavated areas with drainage fill.iv. Repair of bearing ends of existing wood floor joists beneath the Nave.v. Selective excavation and removal of soil within the crawlspace beneath the Nave floor

framing.vi. Installation of new vapor barrier across soil surface of the crawlspace.vii. New concrete footing beneath existing brick foundation wall below the Nave’s center aisle.viii. New forced-air HVAC system throughout the Basement and crawlspace to serve the

Basement, crawlspace, and the Nave above.ix. Construction of a new handicapped-accessible restroom at the northwest corner of the Nave,

and associated demolition and plumbing work.

x. New sheet linoleum flooring throughout the Nave and associated new underlayment.xi. Selective removal of the existing wood wainscot along the north interior wall of the Nave.xii. The General Contractor should anticipate that lead paint will be encountered in the

Church Building. (1) The General Contractor will be required to be or have under contract to them

an EPA-licensed renovator for lead paint as required by the requirements ofthe State of Indiana.

(2) The General Contractor should provide necessary testing to confirm thepresence and levels of lead in paints.

(3) The Bidder will not be allowed to exclude this Work.

(4) The intent regarding Lead Paint is as follows:(a) The General Contractor is to include within their bid any and all

testing for lead paint.

(b) The General Contractor is to assume the presence of lead paint andbe prepared within their bid amount to properly and legally deal withit.

13. Review of Alternates: Refer to Specification Section 01 2300 “Alternates” for a complete description.

a. Add Alternate No. 1: i. Add all labor and material required for the painting of the existing exterior perimeter

limestone at grade, as shown in Elevation 3/A402.

(1) Base Bid: No work at the existing exterior perimeter limestone.(2) Alternate: Add the proper preparation and painting of the existing exterior perimeter

limestone at grade (as described in Elevation 3/A402).

14. Contractor’s Use of Premises and Facilities.

a. The Parish will vacate the Church Building. i. The Parish is to be allowed access to the Sacristy and Balcony as their needs dictate.

b. The other buildings on the Parish’ site will continue to function throughout the entire period ofconstruction. The General Contractor will be required to coordinate and schedule all constructionactivities and deliveries so as not to conflict with the Parish’s PreSchool and other activities.

A. Remove existing electrical wall heater in Vestibule 100 complete along with all associated controls, as clouded.

B. Add to Restroom 103, wall mounted exhaust fan (EF-1) within existing window opening, as clouded.

C. Revise return and outside air ductwork as clouded. 1.02 Sheet M-601: Detail and Schedules

A. Revised EWH-1 on Electric Unit Heater Schedule, as clouded. B. Revised Typical HVAC System Section, as clouded. C. Add Fan Schedule to include exhaust fan (EF-1), as clouded.

1.03 Sheet P-200: Plumbing Floor Plans A. Revise Keynote 5 to reference as follows: “Connect 4” sanitary to existing

sanitary sewer in alley. Information available indicates invert of sewer at connection point to be 651.1’. Aerial topography suggests alley elevation of 651.5’. Field verify location via excavation. Connect per Detail 1/P-501. Compact subgrade and patch alley pavement to match existing pavement section (unknown at this time).”

1.04 Sheet E-200: Electrical Floor Plans A. Remove all wiring and devices feeding existing electric unit heater in Vestibule

100. B. Revise wire size and circuit for EWH-1 in Vestibule 100, as clouded. C. Remove complete junction boxes and associated obsolete raceway complete in
