OBJECTIVES: At the completion of this CME activity, participants will be able to: INTRODUCTION: Older adults are increasingly living with and managing multiple chronic conditions. The self-management of illness occurs in a social and political context in which the responsibility for health has shifted from the State to the individual, who is expected to be an active consumer of health care. Self-care ability is a crucial health resource in older people and may be a decisive factor for older people managing daily life in their own homes. This conference aims to address therapies which can be used with and by older clients to improve their physical, cognitive, and mental functioning and help maintain their independence. Family Physicians, internists, geriatricians, psychiatrists, nurses, counselors, social workers, nursing home administrators, psychologists, physician assistants, physical and occupational therapists, pharmacists, residents and students are encouraged to attend. The University of Toledo is accredited by ACCME to provide continuing medical education for physicians.
