ABOUTTHEPROGRAMTheAmericanPharmacistsAssociation(APhA)developedThePharmacistandPatient-CenteredDiabetesCareCertificateTrainingProgram,aneducationalexperiencedesignedtoequippharmacistswiththeknowledge,skills,andconfidenceneededtoprovideeffective,evidence-baseddiabetescare.Theprogramprovidescomprehensiveinstructionincurrentdiabetesconceptsandstandardsofcareandincorporatescasestudiesandhands-onskillstrainingfocusedonthesituationsmostlikelytobeencountered—aswellastheservicesmostneeded—incommunityandambulatorycarepracticesettings.Participantswillgainexperienceevaluatingandadjustingdrugtherapyregimensforpatientswithtype1andtype2diabetes,counselingpatientsaboutlifestyleinterventions,analyzingandinterpretingself-monitoringofbloodglucoseresults,andassessingtheoverallhealthstatusofpatientstoidentifyneededmonitoringandinterventions.THEGOALSOFTHISPROGRAMARETO:ThegoalofThePharmacistandPatient-CenteredDiabetesCareistoteachthepharmacisthowtointegratediabeteseducationandmanagementintopractice.Thisoverridinggoalincorporatesthegoalofpharmaceuticalcare,whichistoensurethatpatientsmakethebestuseoftheirmedicationsandachievethedesiredtherapeuticoutcomes.Specificprogramgoalsareto:
• Providecomprehensiveinstructiononcurrentstandardsofcareforpatientswithdiabetes.
• Increasepharmacists'abilitytoapplypharmacotherapeuticinformationandserveasthedrugtherapyexpertonthediabeteshealthcareteam.
• Refreshpharmacists'knowledgeofthepathophysiologyofdiabetesandtheacuteandlong-termcomplicationsofthedisease.
• Familiarizepharmacistswithimportantconceptsinnutrition,exercise,andweightcontrolthatcontributetooptimaldiabetescare.
• Providetrainingontheuseofdiabetes-relateddevicesandphysicalassessmentsinvolvedwithoptimaldiabetescare.
• Describebusinessopportunitiesandrolesforpharmacistsinimprovinghealthoutcomesforpatientswithdiabetes.
THEPROGRAMHASTWOCOMPONENTS:Onlineinteractiveselfstudywithassessment;andtheliveseminarwhichincludesdemonstratingcompetencyinbloodpressuretesting,self-injectiontechniques,diabeticfootexam,andbloodglucosetesting,followedupwithonlineevaluationandassessment.ACertificateofAchievementwillbeawardedtoparticipantswhosuccessfullycompleteallprogramcomponents.
TheAmericanPharmacistsAssociationisaccreditedbytheAccreditationCouncilforPharmacyEducationasaproviderofcontinuingpharmacyeducation.ThePharmacistandPatient-CenteredDiabetesCarecertificatetrainingprogramisapprovedforatotalof23.0contacthoursofcontinuingpharmacyeducation(CPE)credit(2.3CEUs).TheACPEUniversalActivityNumbers(UAN)forthisactivityarelistedbelow.
KEYLEARNINGOBJECTIVESOFTHELIVETRAININGSEMINARAtthecompletionofthisactivity,theparticipantwillbeableto:
• Evaluatetheoverallhealthstatusofpatientswithdiabetesintermsofrecommendedmonitoringandinterventions,andformulatestrategiesforclosinggapsincare.
• Proposemodificationstoapatient'sdrugtherapyregimenrootedinevidence-basedalgorithmsfordiabetesmanagement.• Recommenddietaryinterventionstosupportoptimalglycemiccontrolandweightloss(whenindicated)inpatientswithdiabetes.• Analyzeandinterpretapatient'sself-monitoringofbloodglucoseresultsandusetheresultstoidentifyneededchangesinthediabetes
managementplan.• Demonstratepropertechniqueformeasuringbloodpressure,administeringinjections,obtainingfingersticksamplesforbloodglucose
monitoring,operatingbloodglucosemeters,andperformingmonofilamentfoottesting.• Integratethevariedaspectsofcomprehensivediabetescareintoefficient,sensitive,respectfulpharmacist-patientinteractionsthat
supportoptimalpatientself-management.• Describewaysinwhichpharmacistscankeepabreastofnewdevelopmentsandtakeadvantageofprofessionalopportunitiesindiabetes
care.
