1. Medication Reconciliation: The VA has implemented a system-wide approach to reconciling medications for all veterans in primary care. All veterans receive a copy of their medication list at the time of their primary care visit, and are prompted to review it with their doctor to ensure accuracy and to make any changes necessary. Discrepancies between medical record medication lists and patient's actual medication use at home have been widely documented, and cited as a common reason for adverse drug events. We are studying the impact of this simple system change on physician behavior and patient behavior. There are several "spinoff" projects that would lend themselves nicely to a resident research project in this area.

2. Poorly controlled diabetes: The VA tracks many clinical measures carefully and places major emphasis on diabetes. Each month, primary care providers get a list of their diabetic patients with an A1C>9.0. This population is at especially high risk for diabetes-related complications, and yet providers express frustration that the A1C>9 cohort represent a population that is not responsive to most typical interventions. Often, they are simply labeled as "non compliant". In order to develop more effective interventions for the poorly controlled diabetic patients, we need to understand more about them. The first step in this process would be to do a detailed record review of this cohort. What is their burden of comorbidity, degree of chronic pain, and comorbid psychiatric disease? What is their adherence rate to medications, and what barriers to they face to adhere to such regimens. How many are on insulin, and how many who are not are simply unwilling to consider using insulin? Such information will form the basis of a new initiative to improve diabetes care at the VA for this challenging cohort of high risk patients.

1. We are starting a chronic pain management clinic within Chase and are planning to study different aspects of this intervention. One of the questions is can a brief, patient-centered ‘tool kit’ of pain management strategies improve pain levels/functionality compared to usual care? Residents could help design the intervention, determine what outcomes are relevant/measurable and help analyze results.

1. We are currently working on the Women Veteran's Cohort Study which is a VA HSR&D funded project that is investigating gender disparities in male and female veterans of Iraq and Afghanistan in terms of medical/mental health outcomes, VA care and utilization and costs. We have a national database of all the VA enrolled vets from Iraq and Afghanistan and currently have planned papers/projects on overall description of the populations by gender, cost disparities, pain, obesity, stress and coping mechanisms, and reproductive health.. We would welcome resident involvement on any number of subprojects.

2. I have data from a national survey of menopause and hormone therapy use experiences among women veterans. I can provide this data for any resident who may be interested in a project looking at characteristics of patients who choose to use or continue using hormone therapy despite its known adverse effects.

1. We will be having an elective on tropical medicine for Yale medical students and residents staring September10th. I would like to know if this course accomplishes its objectives and what we need to do to make it better.

2. We have started a clinical rotation at Mulago and have sent more than 120 faculty, residents, and medical students since August of 2006. I would like to put some sort of biometrics in place at Mulago to assess the impact of this program on medical education of Ugandan colleagues and patient care and if it is possible to see if this program has any impact on American participants.

3. 30% of patients at Mulago die during hospitalization and another 30% die within 60 days of discharge. Most of them are very young. I would like to know how they die. Why they die? How much patients at Mulago know about their diseases. How much are Ugandan colleagues sharing the medical information with patients? What is the doctor patient relationship at Mulago? What is the reaction of the relatives and friends of dying patients to the terminal events? How has the community adapted to the grief?

4. What is an ideal International Elective for American medical students or American medical residents? What are the components of such an elective?

5. Anemia is one of the 10 top killer diseases in Mulago among adults.

This is a common problem in Mulago. I would like to look at the burden of the problem among inpatients, lab investigations done and their usefulness, management options and out come, and how we can decrease mortality related to anemia.

6. Rheumatic diseases are poorly characterized in Mulago.

This is mainly common among adult patients and carries a high morbidity.

I am not sure if OA or RHA or infective arthritides is the main problem. Is obesity a major growing factor? Are other CT diseases common?

Most pts managed on NSAIDS almost alone.

How can we improve care of patients with rheumatic diseases at Mulago?

Most pts are seen in the emergency setting, and management and assessment remains suboptimal.

I have helped residents and colleagues in the writing of scientific abstracts and Grand Rounds to clarify the thinking process and the basic message.

1. Currently working on a project looking at HIV drug resistance patterns in opioid dependent, HIV+ patients on opioid agonist treatment. Project is in pilot data collection phase. We expect to complete pilot data collection by 3/09 and plan on starting a larger data collection once data analysis is complete.

2. Currently working on development of a clinical program to start Hepatitis C treatment in an opioid treatment center. Once the clinical program is up and running, we will have the capacity to test many research questions including an analysis of on-site (at the methadone clinic) vs. off-site (liver clinic) HCV treatment in this population.

3. Currently working on development of a substance abuse curriculum within the Yale Primary Care, Med-Peds and Traditional Medicine Residency programs. Many potential opportunities for resident scholarship including a needs assessment within the outpatient clinic (currently underway by Felix Cabrera), pre-post knowledge/attitudes/behaviors survey once implementation of curriculum is underway, etc.
