GENERAL INSTRUCTIONS

1. Filing

File electronically via Access Key by attaching the application to the Cover page. Access Key allows secure transmissions to the Commission and also authenticates your identity. Therefore, a signature is not required when using Access Key. Applicants who file their application in this manner are not required to submit a hard copy of the application and its related documents.

2. Confidentiality requests

At your request, portions of the application may be treated as confidential, pursuant to section 20 of the CRTC Rules of Procedure.

Provide a service area map that reflects the boundaries of the service area in one of the following formats:

• standard image (e.g., GIF, JPEG, TIFF, BMP) • PDF file format • GIS-compatible file format, electronically or on a CD-ROM

A map provided in a standard image or PDF file format must be in a scale that readily permits precise identification of the boundaries of the service area and depicts the location in relation to its surrounding areas. This map must provide a level of detail comparable to that contained in the paper map previously required by the Commission (1:50,000 NTS topographical map).

A map provided in a GIS-compatible file format must be created using a map datum of NAD83. The Commission will only accept the following GIS-compatible file extensions:

• MapInfo format (.tab) • MapInfo Interchange format (.mid/.mif)

Licensees are encouraged to post their service area maps on their respective websites.

6. SERVICE INFORMATION - REGIONAL CABLE LICENCE 6.1 Please indicate the location of the licensed areas, as indicated on the current licence of each

undertaking that will be part of the proposed regional cable licence and provide the coordinates of the local head-ends, if applicable. Bell Canada has included lists of secondary locations in the surrounding areas. The licensed area is as per the maps provided. Bell Canada’s regional BDU in Quebec is served by a centralized head-end in Toronto, with related facilities located in Central Offices in each Quebec serving area as indicated.

Community channel 6.6 For licensees that elect to distribute a community channel, outline how you will be operating fully in

accordance with section 27 of the Broadcasting Distribution Regulations, for each undertaking forming part of the proposed regional cable licence. Bell Canada’s community channel manager is fully conversant with the provisions of s.27 of the Broadcasting Distribution Regulations (as amended) and the Commission’s Community Television Policy as set out in Broadcasting Regulatory Policy CRTC 2010-622. As part of periodic reviews of Bell’s community channel, each element of the Regulations and policy will be reviewed against practices in the prior period, and corrective actions, if any, will be undertaken for future periods.

For licensees that provide community programming in the greater Toronto, Montreal and Vancouver areas, set out or update your plans and commitments as to how you will reflect the various geographic communities within your licensed areas in these urban centres. Bell will maintain records of the various communities represented, and attempt to balance

the interest of various communities within Toronto and Montréal by reaching out to under-represented communities, when and where identified. This will present certain challenges for Bell because, unlike the incumbent, it does not have a long history of serving greater Toronto and greater Montreal, and its community channel will be in start-up mode, and have limited resources in comparison to the incumbent’s. Nevertheless, Bell will attempt to achieve a balance over the term of its licence.

Accessibility 6.8

The Broadcasting Act states that "programming accessible by disabled persons should be provided within the Canadian broadcasting system as resources become available for the purpose." Describe the steps you are taking to make your service more accessible to people who are blind or visually impaired and to people who are deaf or hearing impaired, in particular in the following areas:

a)

customer service, for example billing, customer information, or dealing with complaints; Bell Canada has redesigned Bell.ca using WCAG 2.0 accessibility principles. In the Accessibility services section of Bell.ca, customers with disabilities can now browse Bell’s products and services by five key disability types: Physical (mobility and dexterity), Speech, Hearing, Vision and Cognitive. Bell Canada is also taking steps to ensure that all customer-facing employees have a basic understanding of accessible customer-service. Upon request, Bell Canada offers bills, bill inserts and more in alternate formats. Channel listings are available online. For service, general accessibility questions or complaints, customers with disabilities can contact the Accessibility Services call centre or can ask to be directed to the Accessibility Services call centre at anytime. Bell Canada is currently taking steps to increase public awareness of the Accessibility Services call centre and the accessible products and services offered by Bell Canada.

Under the Deferral Obligation to improve products and services for persons with disabilities, Bell Canada partnered with IBM to perform a product assessment of Bell FIBE TV. We are currently reviewing the assessment and opportunities for improvements to TV products and services for persons with disabilities that can be made under the Deferral Obligation. Bell Canada is also engaged in ongoing dialogue with Microsoft, the software provider for our IPTV service, to determine the feasibility of possible improvements. Any enhancements that Bell Canada requests will need to be assessed against Microsoft’s global development priorities.
