Insurer name- for Each insurance company one forms should be submitted
• Correct Policy no should be mentioned after verifying the original policy documents.
• Minor Policy will get attached with proposer’s e IA account.

Application form should be completed in all the respects.
Please ensure you receive clear copies of the KYC proofs along with the completely filled and signed application form.
All supporting documents should be self attested by applicant and All self attested documents should be stamped as “Original seen and verified”.
Avoid corrections and over writings , if any , it should be counter signed by the applicant.
Mail ID should not be used for multiple eIA account opening forms
Please ensure legible hand writing in application form, Usage capital letters
Would be the ideal way of form filling.
Don’t accept submit incomplete applications or incomplete
documentation
Don’t accept supporting documents with out self attestation and Original seen and verified.
Don’t collect illegible copies of supporting documents
