Under the service agreement, employees would be allotted an APS Card withseparate login id & password. He/she could directly logon to our websitewww.apshealth4U.com & enter his/her login id & password. Through this, theemployee would be able to access the list of hospitals, diagnostic centers, clinics,nursing homes & chemist stores. After selecting a hospital/ diagnostic centre/private clinic, medical store; type of OPD/IPD facility needed is to be selected fromthe list of facilities. The employee just need to click on the search menu, he would

be able to get the list of hospitals/ diagnostic centers/ nursing homes/ privateclinics/ chemist stores empanelled with us.

On- Line Appointment FacilityEmployees would be able to fix an appointment with the health facilitators by

just logging in at www.apshealth4u.com & enter their APS login id & password. Thisservice would enable employees to get the details of appointment with date & timespecified rather than wasting time by searching health facilitators & visiting therefor getting appointment for treatment.

Customer Care Helpline

The employee could also directly call to our customer care number & couldbook the appointment as per his/her needs & save his/her time & energy to get ahassle free healthcare facility.

Auto E-mail / SMS ServiceAfter the request for appointment is made by the employee at the desired

Hospital /Private OPD Doctors / Diagnostic centre /Nursing home/ Clinic / Chemiststore, he/she would be receiving an auto generated SMS / E-mail from APSHealth4U Pvt. Ltd. mentioning the status of appointment with time & date details. Incase appointment is unavailable on the date/time requested by employee he/shecould again login & put up a fresh request for some other date/time.

Employee Walk In The employee can any time walk in to our empanelled list of Health

Facilitators carrying his APS Health card & avail huge discounts on

treatment/medicines/ surgical instruments purchased.

APS Health4U CardUnder the services we would be offering Health card (on line) to each

employee, which would be containing the name of employee, his employeenumber, APS ID, gender & the corporate logo. By using this card at our empanelledhealth facilitators employee could not only avail huge discounts for himself/ herselfbut also for his/ her family members hassle free prior-appointment facility withPrivate OPD Doctors (Hospital)/Diagnostic centre /medical practitioner.Employee

just needs to carry the card along with him every time he visits our empanelledhealth facilitators. Rebate on medicines & surgical instruments are also availablewhenever employee presents Health card to any of the Chemist store empanelledwith us.

Online Health IndexEmployees could directly check the details of their health check up history by

using the online facility (applicable to the treatment undertake with prior

appointment only). Details may include - Type of test undertaken, date of visit,name of the doctor, treatment expenses, number of visits to the health facilitatorsetc.

Special Value added Services customized for NDPL only

For better utilization of health services and facilities an organization should havepanoramic view about the trend of utilization of services. Sometimes it has beenobserved that these welfare services are being misutilized. APS can certainlyidentify the trend and help the organization in controlling the health budget. Ourmodus operendi would be as:

As all NDPL prescribing doctors are maintaining the carbon copy, APS will

collect these prescriptions from the prescribing doctors/dispensaries next day

of treatment.

These prescriptions would be scand/viewed by our specialized panel of

Doctors to find/observe the trend of treatmentif any.This analysis will provide

management for maintaining data base employee-wise, ailment-wise,frequency of visit wise, drugs /diagnostic investigation prescribed and its

appropriateness /relevance etc.

We will collect all the reimbursement paper & tests reports also from NDPL

office for providing you the full MIS.

We will provide the MIS on weekly basis giving the details about the

particular of treatment

taken by the employees ,the outstanding liability of diagnostic centre,

hospitals, chemist s shop (if credit any) and also reimbursement bills raisedby employees will be analysed on real time basis.

APS will provide NDPL the preferred List of Hospital /Diagnostics centre /Best

Doctors/ chemist shop (Discount) on CGHS rates.

The CGHS rates for consultation are too low from current market rates. This

puts your employees in low priority list and they have to wait for a longer

periods. Our doctors, who are acclaimed professional, will not agree for

consultation on the rate of CGHS. We are having arrangement for online

booking and speeding the services, therefore, we suggest an upward revision

for consultation fee that can mutually be discussed.

Commercial:For all these services we will charge Rs 500/- only per employee (includingdependents) per annum and service tax as applicable.
