AR Office is Closed for Summer
and will re-open on Monday, August 1st
Pre-Registration for ALL students in the AR Office
8/1-8/12 8:30am-3:30pm
• Please give yourself at least an hour to complete forms.
• Parents/Guardians MUST accompany students as some forms require
both parent and student signature
• You must complete Pre-Registration by 8/12
Friday, August 12th is also the deadline for NEW students to turn in:
1. Copy of Birth Certificate
2. Proof of residence (copy of utility bill)
3. Up-to-date Immunization Record
4. Interdistrict Attendance Permit (if you live outside of FUHS District)

Attendance-IMPORTANT! 1. If your student is absent for ANY reason during ANY part of a school day, the Attendance Office MUST
receive a phone call or a note from the parent or guardian.
•Absences for illness, medical appointments, bereavement or school-sponsored activities are excused.
•All other absences are unexcused
2. Calls or notes must include Date, Time & Reason for the absence
3. Absence calls or notes go to Samantha Broadstock at 476-4203 in the Attendance Office.
4. ANY time a student arrives late or leaves early for ANY reason, they MUST check in/out from the
Attendance Office. If you are late, ALWAYS check in at the office first BEFORE going to class.

Notebook for each class Pencils & Pens Graph Paper (Algebra 1 or 2)
TI-83 Graphing Calculator for Algebra 1 or Geometry Binder with 5-8 sections for class handouts
Composition Book
Lockers PE Lockers can ONLY be used during PE Class, NOT on a day-to-day basis. Student lockers for
continuous use are in the halls or bathrooms of the AT Building. You can find an empty locker in the AT
Building, put your lock on it, and register it with the Attendance Office.
Chromebooks Google Chromebooks will be issued to all students once they have turned in a Chromebook Agreement
signed by their parent/guardian. Students will be able to pick up their Chromebooks on Monday, August
15th. Chromebook insurance can be purchased in the Attendance Office and coverage starts at $40 (cash
or check) for one year with a $0 deductible. Purchase of the insurance is STRONGLY encouraged as
uninsured repairs of damage to the Chromebook can range from $129-$355 depending on the severity of
damage. Students are also encouraged to purchase a protective case for their Chromebooks, as
Chromebooks that are stored in backpacks with books often end up with cracked screens. Insurance
covers accidental damage and theft. Chargers are considered accessories and are not covered by insurance,
so students are financially responsible if they damage their charger.
FUHS Bus Schedule Bus schedules are available in the AR Office once you have turned in all pre-registration paperwork.

College Textbooks for CR Fall Classes (Returning Students Only) AR pays for all required CR Textbooks for students enrolled in college classes.
Students are required to create their own list of required books. Students can do this
using the CR Bookstore web site at
www.bkstr.com/colloftheredwoodsstore/shop/textbooks-and-course-materials
Students must turn in the following to the AR Office by Monday, August 15th:
1. Copy of their CR Schedule
2. Completed CR Book List Form (Form is on AR Web Site under ‘Students’ tab
Don’t list recommended textbooks and please leave the Rent/Purchase field blank.
School Photos-Required for Student I.D. Card Tina’s Photography is coming Monday, August 15th to take school photos. ALL
STUDENTS are required to be photographed for school records and I.D. cards.
No purchase is necessary. However, photos can be ordered if you wish. Prices
and packages are on the enclosed flyer. Please bring cash or check if you want to
purchase pictures.
Academic Advising Academic Advising is provided by Alison Hodges. If you need to contact her, she can be reached by
email at [email protected] or 476-4584. Please be patient as her schedule is very heavy during
the first weeks of school. Add/Drop Request forms are available in the AR Office.
Meals 1. Students may bring a lunch to eat in the Student Success Center (SSC) or the CR Cafeteria.
2. Guidelines for students who purchase a lunch (or bring their own) to eat in the CR Cafeteria:
•Be polite and courteous, clean up after eating, and leave table & chairs in place.
•CR Cafeteria hours are 10:00am-1:30pm when CR is NOT in session and CLOSED on FRIDAYS.
•CR Cafeteria Fall semester hours are 7:30am-7:00pm Mon-Thurs & 7:30am-5:00pm on Fridays.
3. Free or Reduced Meals
• Returning students who received free or reduced meals may continue through September 12th only.
•New & returning students must qualify through a new application after September 12th.
•Free or Reduced Meals are served at the SSC.
Interdistrict Agreement Required If you do NOT live in the Fortuna Union H.S. District you MUST file a new Interdistrict
Agreement form for the 2015-2016 school year in YOUR district office by Friday, August 12th.
