    001 / 2 General Notes

    - Bridge inventoried from south to north. This is a routine underwater inspection. Only the concrete abutments andchannel elements were observed during this inspection.

    **UNDERWATER INSPECTION TO ASSESS SCOUR/SUBMERGED SUBSTRUCTURE ELEMENTS ONLY -ELEMENT NOT RATED. THE CS RATING AND COMMENTS ARE FROM THE 04/15/2015 INSPECTIONREPORT.**

    Cracks- Longitudinal and transverse cracks to 1/16" wide occur along deck soffit and overhangs.

    Spalls- Spalls measuring 6' L x 1' W x 2" D, with exposed and corroding reinforcing steel occur at the southwest soffitnear Abutment 2- At approximately 14' to the west edge of deck, a spall is exhibited up to 1'6"L X 1'2" W X 3"D with exposed andcorroded rebar, with up to 25% section loss.- At approximately 4' to the west edge of the deck, incipient/open spalling is exhibited up to 5.2' L X 3.2'W X 3" Dwith exposed and corroded longitudinal bars, with up to 25% section loss.- At approximately 6' from from the east edge of the slab at soffit, spalling is exhibited to 1'6" W X 3'W X 2" Daround a drain hole with exposed and corroded rebar and up to 25% section loss (Photo 9)- Slab soffit exhibits intermittent areas of heavy scaling and moderate efflorescence leaching.

    - Spalling and delamination occur at several abandoned and paved over drain holes on the slab soffit.- Spalls measuring to 6' L x 1' W x 2" D, with exposed and heavily corroding reinforcing steel occur along southeassoffit near Abutment 2 (Photo 8)- Spalls measuring to 5' L x 1' L x 3" D, with exposed and heavily corroding reinforcing steel occur along westoverhang- Spalling measuring up to 25' L X 1'2"W X 4" D, exposed and corroded rebar is exhibited along the west edge withup to 25% section loss

    - Abutment 1 exhibits three vertical cracks that range in size from hairline to 1/32" wide with efflorescence thatextend from bottom of the arch down to the high water mark.- Abutment 2 exhibits five vertical cracks that range in size from hairline to 1/16" wide and extend from bottom of the arch down to the high water mark (refer to Photo 8).

    - Abutments 1 and 2 exhibit scaling typically 1/2" deep with maximum penetrations of up to 3/4" that extends from 3below arches down to the channel bottom (refer to Photo 7).
