EXECUTIVE SUMMARY
A summary of findings is provided below. It should be recognized that details were not included orfully developed in this section, and the report must be read in its entirety for a comprehensiveunderstanding of the items contained herein.
Terracon Consultants, Inc. (Terracon) conducted an asbestos survey for the Louisiana Departmentof Transportation and Development (LDOTD) of the vacant residential structure located at 103Company Canal Road in Bourg, Louisiana. It is our understanding that LDOTD is planning todemolish the structure. The purpose of this survey was to identify and sample suspect asbestos-containing materials (ACM) and provide information regarding the identity, location, condition andapproximate quantities of ACM in building components.
The survey was conducted on March 20, 2018, by Mr. Michael V. Milano, a Louisiana Departmentof Environmental Quality (LDEQ) certified Asbestos Inspector, in general accordance with thesampling protocols established in Environmental Protection Agency (EPA) 40 Code of FederalRegulations (CFR) 763. Thirty-three (33) bulk samples were collected from eleven (11)homogeneous areas of suspect ACM. Asbestos in concentrations greater than one percent (>1%)were identified in the following sampled materials:
n Fiber Backing associated with Tan Pebbled Sheet Flooringn White Exterior Wall Siding
Asbestos-containing fiber backing in the bathroom closet is considered friable Regulated AsbestosContaining Material (RACM). According to LDEQ and EPA NESHAP regulations, friable ACM isconsidered RACM and is required to be abated prior to demolition.
As the results of this survey indicated the structure contains RACM, all section of the AAC-2 Formmust be completed and submitted to LDEQ prior to demolition activities and an Asbestos DisposalVerification Form (ADVF) requested. Upon proper notification the Department will issue an ADVFto provide approval to begin renovation or demolition activities and to insure that the ACM isremoved and disposed of properly. The ADVF must be onsite during all RACM renovation ordemolition activities. A copy of the most current form approved by the Department can be found athttp://www.deq.louisiana.gov.
According to LDEQ and EPA NESHAP regulations, Category II non-friable ACM is any material,excluding Category I non-friable ACM, that when dry, cannot be crumbled, pulverized, or reducedto powder by hand pressure. Category II non-friable ACM that has a high probability of becomingor has become crumbled, pulverized, or reduced to powder by the forced expected to act on thematerial in the course of demolition operations are considered Regulated Asbestos ContainingMaterials (RACM) and are required to be abated prior to demolition.

Asbestos was identified in concentrations of less than 1% in the ceiling tile adhesive; therefore, thismaterial is not considered ACM. However, the OSHA Asbestos standard for construction (29 CFR1926.1101) regulates workplace exposure to asbestos regardless of concentration. The OSHAstandard requires that employee exposure to airborne asbestos fibers be maintained below 0.1asbestos fibers per cubic centimeter of air (0.1 f/cc). The disturbance of this material has thepotential to result in the release of airborne asbestos fibers.
Terracon recommends that the identified ACM be removed by a State of Louisiana licensedasbestos abatement contractor prior to demolition. LDEQ requires an Asbestos AbatementNotification Form AAC-2 be submitted prior to demolition.
A summary of the classification, condition and approximate quantity of each identified ACM ispresented in Appendix A. Laboratory analytical reports are included in Appendix C.

1.0 INTRODUCTION
Terracon Consultants, Inc. (Terracon) conducted an asbestos survey for the Louisiana Departmentof Transportation and Development (LDOTD) of the vacant residential structure located at 103Company Canal Road in Bourg, Louisiana. The survey was conducted on March 20, 2018, by Mr.Michael V. Milano, a LDEQ certified Asbestos Inspector, in accordance with the Task Order Number6. Interior and exterior building components were surveyed and homogeneous areas of suspectasbestos-containing materials (ACM) were visually identified and documented. Suspect ACMsamples were collected in general accordance with the sampling protocols outlined inEnvironmental Protection Agency (EPA) 40 Code of Federal Regulations (CFR) 763, AsbestosHazard Emergency Response Act, (AHERA). Samples were delivered to an accredited laboratoryfor analysis by polarized light microscopy (PLM).
1.1 Project Objective
We understand this asbestos survey was requested due to the planned demolition of the structure.EPA regulation 40 CFR 61, Subpart M, National Emission Standards for Hazardous Air Pollutants(NESHAP) and Louisiana Administrative Code (LAC) Title 33, Part III, Section 5151, prohibits therelease of asbestos fibers to the atmosphere during renovation or demolition activities. The EPANESHAP and LDEQ regulations requires that potentially regulated asbestos-containing buildingmaterials be identified, classified and quantified prior to planned disturbances or demolitionactivities.
2.0 BUILDING DESCRIPTION
The structure is a vacant, one-story, approximately 855 square-foot residential building with anunknown construction date. The building is a wood-framed structure constructed on a slab-on-grade concrete foundation, and topped with a gabled metal roof. The exterior walls are finishedwith vinyl siding over asbestos-containing cement board siding. Doors are wooden with woodframes and windows are aluminum framed. Interior floors are vinyl floor tiles installed throughoutthe house on sheets of plywood with a vapor barrier underneath. Interior walls are non-insulatedwood panels, and the ceilings are acoustical ceiling tiles adhered to wooden panels. Drywallgypsum wallboards were used to construct the HVAC closet in the northeast bedroom and thewalls of the stairwell to the attic.

3.0 FIELD ACTIVITIES
The survey was conducted by Mr. Michael V. Milano, a LDEQ certified Asbestos Inspector. A copyof Mr. Milano’s Asbestos Inspector certificate is attached in Appendix D. The survey was conductedin general accordance with the sample collection protocols established in EPA regulation 40 CFR763, AHERA. A summary of survey activities is provided below.
3.1 Visual Assessment
Our survey activities began with visual observation of the interior and exterior of the structure toidentify homogeneous areas of suspect ACM. A homogeneous area consists of building materialsthat appear similar throughout in terms of color, texture with consideration of the date of application.The interior and exterior assessment was conducted throughout visually accessible areas of thestructure. Building materials identified as concrete, glass, wood, masonry, metal or rubber were notconsidered suspect ACM.
3.2 Physical Assessment
A physical assessment of each homogeneous area of suspect ACM was conducted to assess thefriability and condition of the materials. A friable material is defined by the EPA as a material whichcan be crumbled, pulverized or reduced to powder by hand pressure when dry. Friability wasassessed by physically touching suspect materials.
3.3 Sample Collection
Based on results of the visual observation, bulk samples of suspect ACM were collected in generalaccordance with AHERA sampling protocols. Random samples of suspect materials were collectedin each homogeneous area. The inspector collected bulk samples using wet methods as applicableto reduce the potential for fiber release. Samples were placed in sealable containers and labeledwith unique sample numbers using an indelible marker. Thirty-three (33) bulk samples werecollected from eleven (11) homogeneous areas of suspect ACM.
3.4 Sample Analysis
Bulk samples were submitted under chain of custody to Moody Labs of Farmers Branch, Texas foranalysis by PLM with dispersion staining techniques per EPA methodology (40 CFR 763, SubpartE). The percentage of asbestos, where applicable, was determined by microscopic visualestimation or point counting. CEI is accredited under the National Voluntary LaboratoryAccreditation Program (NVLAP Accreditation No. 102056-0) and Louisiana EnvironmentalLaboratory Accreditation Program (LELAP Certificate Number 04089).

4.0 REGULATORY OVERVIEW
The asbestos NESHAP (40 CFR Part 61, Subpart M) regulates asbestos fiber emissions andasbestos waste disposal practices. The asbestos NESHAP regulation also requires theidentification and classification of existing ACM according to friability prior to demolition orrenovation activity. Friable ACM is a material containing more than 1% asbestos that, when dry,can be crumbled, pulverized or reduced to powder by hand pressure.
The asbestos NESHAP regulation classifies ACM as either Regulated Asbestos Containing Material(RACM), Category I non-friable ACM or Category II non-friable ACM. RACM includes all friableACM, along with Category I and Category II non-friable ACM that has become friable, will be or hasbeen subjected to sanding, grinding, cutting or abrading, or ACM that has a high probability ofbecoming or has become crumbled, pulverized, or reduced to powder in the course of renovationor demolition activity. All friable ACM is considered regulated asbestos containing material (RACM).Category I non-friable ACM are exclusively asbestos-containing packings, gaskets, resilient floorcoverings, resilient floor covering mastics and asphalt roofing products that contain more than 1%asbestos. Category II non-friable ACM are all other non-friable materials other than Category I non-friable ACM that contain more than 1% asbestos. Category II non-friable ACM generally includesbut is not limited to cementitious material such as: cement pipes, cement siding, cement panels,glazing, mortar and grouts.
The State of Louisiana has established Chapter 27 of the ERC (LAC 33:III.Chapter 27) to regulatethe identification, management, and abatement of ACM in schools and state buildings. Chapter 27requires any asbestos-related activity in a school or state building to be performed by an individualor company accredited by the State of Louisiana, through the LDEQ. An asbestos-related activityconsists of the disturbance (whether intentional or unintentional) or abatement of ACM, theperformance of asbestos surveys, the development of management plans and response actions,asbestos project design, the collection or analysis of asbestos samples, monitoring for airborneasbestos or any other activity required to be accredited under Louisiana Department ofEnvironmental Quality (LDEQ) Chapter 27 Appendix A.
In non-state, non-school buildings, the State of Louisiana sets forth emission standards for asbestosunder Chapter 51 of the ERC (LAC 33:III.Chapter 51). Per Chapter 51 Section P, the followingactivities must be performed by accredited individuals: asbestos surveys, asbestos abatement,monitoring for airborne asbestos, and project design.
The Louisiana Air Quality Regulations (LAC 33:III.Chapter 51, Subchapter M) require that aninspection be conducted by a person currently accredited as an LDEQ asbestos inspector. LDEQrequires a notification by submitting either an AAC-2 (a) form or AAC-2 (b) form. An AAC-2 (a)form is required when requesting Asbestos Disposal Verification Forms (ADVF) for AsbestosContaminated Debris Activities (ACDA), Demolition, Renovation, and/or Response Actionprojects where Regulated Asbestos Containing Material (RACM) is present, or assumed to bepresent, above the established thresholds or as otherwise required by LAC 33:III.5151.F.1. TheAAC-2 (a) form must be either postmarked or hand delivered to the Department at least 10

working days prior to the scheduled dates of asbestos removal. An AAC-2 (b) form is requiredwhen greater than 64 square feet of Vinyl Asbestos Tile (VAT) is removed without the intent ofmaking it RACM, or when lab analysis of properly sampled materials indicates that no ACM ispresent; that ACM present is not RACM and will not be made RACM by the demolition; or that allRACM present is less than established thresholds. The established thresholds per LAC33:III.5151.F.1 include the combined amount of RACM less than 60 linear feet on pipes, 64 squarefeet on other facility components or 27 cubic feet of facility where length or area could not bemeasured previously. A Form AAC-2 (b) must be postmarked or hand delivered to the Departmentat least 5 working days prior to the scheduled date of asbestos removal or 3 working days if theremoval only includes resilient floor covering per LAC 33:III.5151.F.2.c.
Any individual or company contracted to perform a demolition or renovation activity which disturbsRACM must be recognized by the Louisiana Licensing Board for Contractors to perform asbestosabatement.
The United States Occupational Safety and Health Administration (USOSHA) asbestos standardfor construction (29 CFR 1926.1101) regulates workplace exposure to asbestos. The USOSHAstandard requires that employee exposure to airborne asbestos must not exceed 0.1 fibers percubic centimeter of air (0.1 f/cc) as an eight hour time weighted average (TWA) and not exceed 1.0fibers per cubic centimeter of air (1.0 f/cc) over a 30 minute time period known as an excursion limit(EL). The TWA and EL are known as USOSHA’s asbestos permissible exposure limits (PELs). TheUSOSHA standard classifies construction and maintenance activities which could disturb ACM, andspecifies work practices and precautions which employers must follow when engaging in each classof regulated work.
5.0 FINDINGS AND RECOMMENDATIONS
Asbestos in concentrations greater than one percent (>1%) were identified in the following sampledmaterials:

6.0 GENERAL COMMENTS
This asbestos survey was conducted in a manner consistent with the level of care and skill ordinarilyexercised by members of the profession currently practicing under similar conditions in the samelocale. The results, findings, conclusions and recommendations expressed in this report are basedon conditions observed during our survey of the structure. The information contained in this reportis relevant to the date on which this survey was performed, and should not be relied upon torepresent conditions at a later date. This report has been prepared on behalf of and exclusively foruse by the LDOTD for specific application to their project as discussed. This report is not a biddingdocument. Contractors or consultants reviewing this report must draw their own conclusionsregarding further investigation or remediation deemed necessary. Terracon does not warrant thework of regulatory agencies, laboratories or other third parties supplying information which mayhave been used in the preparation of this report. No warranty, expressed or implied is made.

Notification of Demolition and Renovation Form and Asbestos Contaminated Debris Form(AAC-2a)
General Information and Instructions
1. Demolition or Renovation involving Regulated Asbestos Containing Material (RACM)After proper notification is received, the Department issues an Asbestos Disposal Verification Form(ADVF) to provide approval to begin renovation and to ensure that the ACM removed is disposed ofproperly. The ADVF must be onsite during all RACM renovation or demolition activities.
The AAC-2 form must be either postmarked or hand delivered to the Department at least 10 working daysprior to the scheduled dates of asbestos removal (Section VIII). The completed AAC-2 form should besubmitted to the Department at the address at the top of the AAC-2 form.
Note: The AAC-2 form MUST be typed, properly completed with an original signature, and beaccompanied by the appropriate fee. The AAC-2 Form MUST be submitted to LDEQ on the most currentform approved by the Department, and can be located on the Asbestos web site athttp://www.deq.louisiana.gov/portal/tabid/2883/Default.aspx. Modified forms will not be accepted.
A. Notification of Demolition and Renovation Form, AAC-2 Instructions
1. Box at the top of the Form: No. of ADVFs Requested - One ADVF is required per load of AsbestosContaining Waste Material (ACWM) being transported. Exception: Residential or Commercialdemolition structures financed by FEMA as part of Hurricane projects may use the procedures noted in:
§ Addendum to ADVF Procedure for Residential ACWM Sample Addendum (Implemented 3/29/2006; Revised 5/31/07)
§ Addendum to ADVF Procedure "ADVF 1" for Commercial ACWM (9/27/07)
which is located on the Demolition website at: http://www.deq.louisiana.gov/portal/tabid/2885/Default.aspx.

3. Check if the Notice is an Emergency and complete the information in Section XIV: Emergenciesare defined in LAC 33:III.5151.B Definitions:
Emergency Demolition/Renovation Operation—a demolition or renovation operation that was not planned but results from asudden unexpected event that, if not immediately attended to, presents a safety or public health hazard, is necessary to protectequipment from damage, or is necessary to avoid imposing an unreasonable financial burden. This term includes operationsnecessitated by nonroutine failures of equipment.
4. Check if this notification is a revision of a previous notification. If it is a revision, please note thenumber of the most recent ADVF issued for the project. Highlite or mark in some manner all changedsections on the AAC2. Note that none of the Types of Notifications in Section I may be used on the

Original: The first notification of the particular removal activity.
Additional: Request additional ADVF(s) for a particular activity. Document the number of theoriginal or first ADVF number issued in which the additional ADVF(s) are necessary.
Annual: Any individual asbestos renovation and/or demolition operation necessitated solely bythe need for the repair or maintenance of facility components, and involves a total of less
(Annual Maintenance) than 1 cubic yard of RACM per operation. Note the Total Volume in Section V as anestimate only. Storage bin size may be used for an estimate.
Disposal Only If the asbestos is for disposal only, check the box noted as “ Disposal Only” .
II. Type of Operation - Check the appropriate selection:Demo: The wrecking or taking out of any load-supporting structural member of a facility
together with any related handling operations or the intentional burning of any facility.Note: Intentional burning of any facility that contains any type of asbestos is strictlyprohibited.
Reno: Altering a facility or one or more facility components in any way, including the strippingor removal of RACM from a facility component that is to be returned or remain in place.
RACM Demo: Entire structure treated as RACM
ACDA: Asbestos Containing Debris Activities
Response Action: RACM operations for schools or State buildings
Ordered: Demolition is ordered by a local or state government agency.
III. Facility Description: Enter the following information for the facility where the actual activity will take place:
Facility Name: Legal name of the facility.Project Designer: LDEQ Accreditation no. of the Project Designer is required only for schools and
State buildings. Leave blank if the facility is not a school or state building.Physical Address: The physical address where the activity will take place. Specific directions
to the site, should the address prove inadequate. (i.e. Two miles past Highway X on the right.) Include the city, state, zip code and parish where the facility is located. No P.O. Box Numbers are accepted.
Owner Name: Include contact name, mailing address, phone number, and email
Site Location: Building name or number, equipment or component name (i.e. Bldg 2-A, Boiler 1) wherethe work is being done.
Telephone No: Telephone no. of the facility where the asbestos activity will take place.

Building Size: Estimate in square meters or square feet.No. of Floors: Enter the no. of floors including the basement or ground level floors.Age in Years: The approximate age of the facility.Present Use: Describe the current primary use of the facility.Prior Use: Describe the former primary use of the facility
IV. Determination of Asbestos Present? Check “known or assumed” or “determined by inspection”. Include: Inspection Date (MM/DD/YY) Month/Day/Year; Inspectors Name; LDEQ Inspector Accreditation number; and Procedure used to identify the asbestos material, including analytical method. (“Assumed” or “Known” to be asbestos containing is acceptable.)
If the material is “known” or “assumed” to be asbestos and the inspection information is not known,check that box in lieu of the inspection information.
V. Approximate Amount of Asbestos, including:Check if removal times will be during:
Business Hours, After Hours, Weekend, or Holidays.ACM to be Removed:
Describe the RACM, and Category I or II to be removed. Describe thematerial, i.e. thermal system insulation (TSI), ceiling, transite siding, vinylasbestos tile (VAT), etc.
Unit of Measurement: For Pipes, note linear footage; and for Surface Material, note square footage.
The “Total Volume of RACM” in cubic yards is mandatory. Note the total amount of asbestoscontaining waste material in cubic yards for the entire project.
Non-Regulated ACM Not to Be Removed Prior to Demolition:Enter information on any ACM left in the building during demolition. Note: Only Category IACM that is in good condition, and will remain in good condition by the forces expected to actupon the material during demolition, may remain in the structure prior to demolition. For adefinition of Category I and/or Category II non-friable Asbestos Containing Material, see theLouisiana Air Quality regulations, LAC 33:III.Chapter 51. Subchapter M athttp://www.deq.louisiana.gov/portal/tabid/1674/Default.aspx.
VI. Asbestos Removal Contractor Information:Enter the licensed asbestos removal contractor’s name; State AI number; the licensed asbestosremoval contractor’s license number issued by the La. State Licensing Board for Contractors; andthe name, LDEQ accreditation no, and expiration date of the Supervisor/ Contractor in charge of theproject.
Enter the licensed asbestos removal contractor’s mailing address, including city, state, and zip code;a contact name, and telephone, fax number, and email address for the contact person.
VII. Other Operator/Demolition Contractor:If another operator/ contractor is handling demolition activities (whether the structure containsasbestos or not), such as a general contractor, or any other person who leases, controls or supervises

VIII. Scheduled Dates of Asbestos Removal:Enter the dates (Month/Day/Year) for asbestos removal work. Asbestos removal work includes anyactivity which dislodges or disturbs asbestos materials, including site preparation.
IX. Scheduled Dates of Demolition:Enter the dates for beginning and ending the demolition or renovation activity. For renovations,Sections IX and X may be (and usually are) the same.
X. Solid Waste Transporter for RACM/ACD to the Landfill:Enter the name, address, contact person, Solid Waste (SW) Transporter no, and telephone no. of thecompany responsible for transporting the asbestos waste from the removal site to the waste disposalsite. Please contact Sheryl Grimmer of the LDEQ Permit Support Services Division, Notificationsand Accreditations Section at 225-219-1665 to apply for a SW transporter number.
XI. Solid Waste Transporter for RACM/ACD to a Designated Site for Storage:Enter the name, address, contact person, Solid Waste Transporter no, telephone no. of the companyresponsible for transporting the asbestos waste from the removal site to a designated area prior totransporting to the asbestos waste disposal site. Please contact Sheryl Grimmer of the LDEQ PermitSupport Services Division, Notifications and Accreditations Section at 225-219-1665 to apply for aSolid Waste transporter number.
XII. Asbestos Waste Disposal Site (RAL) for RACM/ACD:Identify the asbestos waste disposal site, including the complete name, location, telephone number,and contact person of the facility. The disposal site must be permitted by the LDEQ, WasteDivision and recognized by the LDEQ, Permit Support Services Division. A listing of approvedsites can be found at http://www.deq.louisiana.gov/portal/tabid/2883/Default.aspx under theheading, Louisiana Recognized Asbestos Landfills.
XIII. Government Ordered Demolition Information:Provide the name of the responsible official, title and agency, authority under which the order wasissued, the dates of the order and the dates the ordered demolition will begin. Attach a copy of theGovernment Order to the AAC-2 form.
XIV. Emergency Renovation Information:Provide the date and time of the emergency, a description of the sudden unexpected event; and adescription of unsafe conditions, equipment damage, or financial burden resulting from the event.The information should be detailed enough for DEQ to evaluate whether a renovation falls withinthe emergency exception.
The Louisiana Air Quality regulations note emergencies as:
1- a sudden, unexpected event that necessitated the renovation; 2- an unsafe condition, or condition that would cause equipment damage; or
3- an unreasonable financial burden.

For emergencies, verbal telephone notification must be made immediately, but in no case later thanfour hours after learning of the incident. If additional space is needed, write the information on aseparate sheet of paper and attach it to the AAC-2 form. The Department will determine whetherthe project is a true emergency, and will enforce the ten (10) day notification period if the project isnot deemed an emergency.
XV. Description of Planned Demolition, Renovation, Response Action, or ACDA Work Method(s) tobe Used: Describe the demolition or renovation techniques to be used, and the areas and types offacility components that will be affected. (eg. Adequately wet material, use leak-tight bags, etc.)
XVI. Comments: Provide additional comments relevant to this notification.
XVII. Certification of an Asbestos Renovation or Demolition Activity: If the activity involves Regulated ACM, read and certify by signature if the statement is accurate.
