Welcome to the OZONE! Thank you for your interest in After School Care at Cheer Omega. We are so
excited to offer an after school program that is truly out of this world! Students in our program will
enjoy daily organized gym activities, homework time, snack time, games, optional tumbling classes and
discounts on additional tumbling. Our mission at Cheer Omega is to enrich the lives of the children and
their families through After School Care, Tumbling, and Competitive Cheerleading. Our goal each day is
to teach your children the value of kindness, hard work, integrity, leadership, self-confidence, positivity,
and a love for learning. Each and every family is important to us and our program. We can’t wait to
welcome you to Our Family!

Pricing Information:
Full Week Price (Pickup M-F): $60 per week
Daily Rate (Price per day): $15 per day
All students enrolled in 3 or more days a week receive a FREE Tumbling class!
Any additional tumble or stunt classes 10% off for all After School Students!
15% discount for all 2015-2016 Cheer Omega All Star Athletes
Payment Information:
Pay by Month Option: Pay in advance by the 1st of each new month
Weekly Payment Option: Payment due by Monday of each new week
Cheer Omega accepts payment in the form of Credit Card, Cash, or Check
All Families must have Autopay on file, but may opt to use a different form of payment each week.
Any payments not received by Wednesday of each new week are subject to a $10 late fee
If your child does not attend school on their scheduled pickup day due to illness, early check out,
vacation, or any other reason, families must let Cheer Omega know by 1pm each day or are subject
to a $10 inconvenience fee.

FAQ’S
Q: What will my children do each day? A: Day to Day operations may vary slightly, but here is what a typical day in Cheer Omega’s After School program looks like:
1:45pm-2:45pm: Pick up from Schools (Pickup time varies dependent on your child’s school)
2:00pm-2:45pm: Arrival Time/Homework
2:45pm-3:30pm: Open Gym Time
3:30pm-3:45pm: Snack Provided every day (Please list any known allergies on form below)
3:45pm-4:30pm: Additional Homework time/ Quiet time (Reading, Homework, Board Games)
4:30pm-6:00pm: Group Games, Movies, and more!
6:00pm: Pickup time Please note: All students enrolled in three or more days of after school will receive a free tumbling class! Tumbling classes are Monday-Friday from 3:30pm-4:30pm or 4:30pm-5:30pm dependent on your day and time selection and your child’s tumbling level. Q: Who are the Instructors and drivers? A: Our After School program will be managed and overseen by a Certified Escambia county school teacher, Ashley Monn. We believe that utilizing Ashley’s talents as a teacher will only further to benefit our after school program’s success, as well as each child’s academic success. Casey Jo Benson will serve as the Head counselor and Director for the program. If you have any questions or concerns, please see Casey Jo, first and foremost. Casey Jo will be responsible for following through with the day to day operations of our After School program. Sarah Harrell will also serve an important role in our program and will act as Casey Jo’s Assistant Director, as well as taking on Director responsibilities in Casey Jo’s absence. Other counselors consist of coaches, Ryan Dockens, Kevin Allen, and China Farmer. All of our counselors and coaches have been background checked to work with your children and provide transportation to Cheer Omega from your child’s school each day. Q: What ages can attend the After School program? A: Kindergarten-5th grade students can enroll in Cheer Omega’s After School program. We have also picked up certain middle schools if enough interest is shown. Q: I have a 5 year old Kindergartener in a booster seat, can she attend? A: Absolutely! Safety is our #1 priority at Cheer Omega. We have two booster seats available for use and will provide one for any child who needs to use a booster during their transportation from school to Cheer Omega. Q: Can you provide pickup to schools that aren’t on your pickup list? A: We take requests for schools to add to our pickup route and dependent on how many children need pickup and distance to the school we will consider adding additional schools to our route. Generally at least two children are needed for us to add a school to our driving route, but we have made exceptions. Q: What time do I need to pick up my child and how do I add family to their Check out list? A: Pick up time is 6:00pm. However, parents are able to pick up children as early as their schedule needs. Each child’s parent will fill out a Check Out list at the start of the school year and only authorized adults on that list will be able to pick up your child each day. If you are late to pick up your child or want to utilize extended stay hours, you can do so for $5 each day that you need to pick up your child between 6:00pm-6:30pm (This doesn’t include All Star athletes who have practice after this time.) Q: This sounds great! How do I sign up? A: Please fill out this packet and turn in or email to Cheer Omega prior to your child’s first day of pick up.
Cheer Omega will offer day camps on most days that school is out of session. These camps will be available at a discounted rate for everyone in our OZONE After School Program! Please preregister in order to receive the reduced rate.

EVERYONE is required to submit credit card/debit card information and to be on auto-pay. It is your responsibility to inform the office of any changes to this card. Although families are not required to pay by using the Autopay option each week, a credit card is required to be on file to participate in OZONE After School Program. Payment each week is available via Credit card (Auto or self pay), Cash, or Check. If payment is not received by Wednesday each week (Not including Holidays), I Authorize Cheer Omega to run my Card on File. If the card declines or has incorrect information, I understand that a late fee of $10 will be added to my account for each week I am late on making my After School Payment to Cheer Omega. All Autopay Weekly payments will be taken on Monday of each new week, All Autopay Monthly payments will be taken at the 1st of each month (August will be taken upon the 1st day of pick up)

Participation in physical activities at CHEER OMEGA, LLC presents some risks. I understand the nature of this Activity and that I am
qualified, in good health, and in proper physical condition to participate in such Activity. I acknowledge that if I believe event conditions
are unsafe, I will immediately discontinue participation in the activity. I fully understand that this Activity involves risks of serious bodily
injury, including permanent disability, paralysis and death, which may be caused by my own actions, or inactions, those of others
participating in the event, the conditions in which the event takes placements, or the negligence of the “releases” named below; and
that there may be other risks either not known to me or not readily foreseeable at this time; and I fully accept and assume all such risks
and all responsibility for losses, cost, and damages I incur as a result of my participation in the Activity.
I hereby release, discharge, and covenant not to sue Cheer Omega, LLC, its respective administrators, directors, agents, officers,
volunteers, and employees, other participants, any sponsors, advertisers, and, if applicable, owners and lessors of premises on which
the Activity takes placement, (each considered one of the “RELEASEES” herein) from all liability, claims, demands, losses, or damages,
on my account caused or alleged to be caused in whole or in part by the negligence of the “releases” or otherwise, including negligent
rescue operations and future agree that if, despite this release, waiver of liability, and assumption of risk I, or anyone on my behalf,
makes a claim against any of the Releases, I will indemnify, save, and hold harmless each of the Releases from any loss, liability, damage,
or cost, which any may incur as the result of such claim.
I have read the RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT, understand that I have given
up substantial rights by signing it and have signed it freely and without any inducement or assurance of any nature and intend it to be a
complete and unconditional release of all liability to the greatest extent allowed by law and agree that if any portion of this agreement
is held to be invalid the balance, notwithstanding, shall continue in full force and effect.
