ADDENDUM # 3 Date of Issue: April 08, 2020 Date of Opening: April 14, 2020 (Revised per Addendum#2) 1. The Call for Bids is amended as follows:
Revise the second and third paragraphs to read: Sealed bids for the project designated above will be received by Kitsap County Department of Public Works before the time and date indicated above, at which time they will be opened and publicly read aloud. Due to the COVID-19 pandemic, the Public Works building is closed to the public. Prospective bidders will not be allowed inside the Public Works Building. Bids delivered in person or by private carrier (UPS, Federal Express, etc.) will be received by staff from Kitsap County Department of Public Works between the hours of 10:30 AM and 11:00 AM. Bids will be received in person or by private carrier (UPS, Federal Express, etc.) at: Kitsap County Department of Public Works At the front entrance of the Public Works Building, located at 507 Austin Ave Between the hours of 10:30 AM and 11:00 AM Port Orchard, Washington. Bids will be opened and publicly read aloud in the Commissioner Chambers where social distancing protocol will be strictly adhered to. Access to the Commissioner Chambers, located across the street from the Public Works Building, will only be provided during the bid opening.

1-05.15 Method of Serving Notice The second paragraph is revised to read:

All correspondence from the Contractor shall be directed to the Engineer. All correspondence from the Contractor constituting any notification, notice of protest, notice of dispute, or other correspondence constituting notification required to be furnished under the Contract, shall be in letter format and delivered either via mail delivery service to the Engineer's office or delivered as an attachment to an email, as described below.
Notices delivered by email shall be sent to all addresses on a mutually agreed upon list of addresses for the Contractor or the Contracting Agency. The list shall be agreed upon prior to issuance of the “Notice to Proceed with Construction” and shall be maintained by the Engineer. An email is deemed to be received by the end of the next business day following the date and time as recorded by the device used by the sender from which the email was sent. Email notifications shall have a subject line containing at least the project name and the word Notification (Bay Shore and Washington – Notification). Attachments over 10 megabytes are not allowed. For files over 10 megabytes, a link to a secure file transfer service shall be provided. Confirmation of receipt of email notification shall be provided by the recipient no later than one business day following receipt of the email notification.
1-07.1 Laws to be Observed Supplement this section with the following: COVID-19 Health and Safety Plan (CHSP) In response to COVID-19, the Contractor shall prepare a project specific COVID-19 health and safety plan (CHSP) in conformance with Section 1-08.4(13) of these special provisions. A copy of the CHSP developed by the Contractor shall be submitted to the Engineer as a Type 2 Working Drawing. 1-08.4 Preconstruction Work Phase 1-08.4(1) Activities Supplement this section by adding the following activity:
12. COVID-19 Health and Safety Plan (CHSP) 1-08.4(11) Notice to Proceed and Prosecution of Work Supplement this section by adding the following: No work shall be performed on the site until a COVID-19 Health and Safety Plan (CHSP) has been prepared in conformance with Section 1-08.4(13) of

these special provisions. Add the following new section: 1-08.4(13) COVID-19 Health and Safety Plan (CHSP). COVID-19 Health and Safety Plan (CHSP) The Contractor shall prepare a project specific COVID-19 health and safety plan (CHSP). The CHSP shall be prepared and submitted as a Type 2 working drawing prior to beginning physical Work. The Contractor shall update and resubmit the CHSP as the work progresses and new activities appear on the look ahead schedule required under Section 1-08.3(2)D. If the conditions change on the project, or a particular activity, the Contractor shall update and resubmit the CHSP. Work on any activity shall cease if conditions prevent full compliance with the CHSP. The CHSP shall address the health and safety of all people associated with the project including Contractor’s employees, Kitsap County staff, Kitsap County representatives, project staff, subcontractors, suppliers and anyone on the project site, staging areas, or yards. The plan shall contain the following minimum elements:
1. The CHSP shall identify all standards, guidance, publications, and sources on which it is based. Those standards may include references to OHSA, WISHA, and CDC publications that are current at the time the CHSP is prepared.
2. The CHSP shall identify a responsible individual from the Contractor who is responsible for implementation of the CHSP. The individual(s) contact information shall be listed in the CHSP.
3. The CHSP shall specifically identify the project for which it is applicable, and if applicable, shall address project work areas outside the project limits such as staging areas or yards.
4. The CHSP shall identify the PPE and administrative and engineered controls necessary to maintain a safe site. This includes but is not limited to: sanitation resources, screening stations, safety briefings, controlling access, and personal protective equipment (PPE) needed to protect workers from COVID-19.
5. The CHSP shall identify measures for screening and managing workers or visitors to areas identified in the CHSP. The plan shall include procedures should a person exhibit symptoms of

COVID-19.
6. The CHSP shall identify how the plan will be updated as new work activities are added with each Weekly Look-Ahead schedule. The CHSP updates shall identify the number of workers, crews, work tasks, and the degree of congestion or confinement workers will experience for the work activities in the Weekly Look-Ahead schedule.
7. The CHSP shall include how the Contractor will ensure everyone on the site has been trained on the CHSP requirements. This includes subcontractors, suppliers, and anyone on the project site.
COVID-19 Health and Safety Plan (CHSP) Inspection The Contractor shall grant full and unrestricted access to the Engineer for CHSP Inspections. The Engineer (or designee) will conduct periodic compliance inspections on the project site, staging areas, or yards to verify that any ongoing work activity is following the CHSP plan. If the Engineer becomes aware of a noncompliance incident either through a site inspection or other means, the Contractor will be notified immediately (within 1 hour). The Contractor will be given 24 hours to either remedy the noncompliance incident or halt the associated work activity. If the Contractor fails to comply within 24 hour of receiving the Engineer’s notification, the Engineer may suspend Work. The Contractor must satisfy the Engineer that the noncompliance incident has been corrected before the suspension will end. 7-05.2 Materials Revise the paragraph starting with “Exterior Manhole . “ to read: Exterior Manhole coating shall be Tnemec 46H-413 Hi-Build Tneme-Tar or approved equal and shall only be applied to manholes in Schedule B for the sanitary sewer system. 7-05.5 Payment The pay item for “Manhole ___In. Diam. Type___, per each”, is revised to read: The unit contract price for “Manhole __In. Diam. Type __”, per each shall include the PVC connection boots for sanitary manholes, forming and pouring custom bases in microtunnel limits as necessary, exterior coating for sewer manholes, crushed surfacing top course foundation, excavation, backfill and compaction, primer and joint wrap as described in the plans and these special provisions and the manhole base extension as described in the plans. Specialty fabrication for low cover stormwater manholes as noted in the plans

shall be considered incidental to this item. 7-17.2 Materials Delete the last paragraph of this section: All metal products and fitting components (e.g. bolts, gaskets, etc) are to be of domestic fabrication and construction. 7-20.2(5) Casing Pipe Revise the first paragraph of this section to read: New welded steel pipe, unlined and uncoated, meeting the requirements of ASTM A36 or ASTM A283 Grade A or B. Yield strength of the material: 35,000 pounds per square inch, minimum. Welding procedures used to fabricate steel casings shall be consistent with the provisions of AWS D1.1 and AWWA C206.
3. The Contract Plans are amended as follows: A. Drawing Number DS01 (sheet 42).
This drawing is re-issued as the titles for three of the details are missing. Detail 3 – Cleanout Detail 22 - Standard County Manhole Lid Detail MV-5 – Rubber Boot Manhole Insert

B. Drawing Numbers C01 (sheet 60) through C10 (sheet 69). Vaned grates are referenced to WSDOT Standard Plan B-30.30 in the general “Roadway Notes”, note 5. Revise note to read: Vaned Grates are to match Kitsap County Stormwater Standard East Jordan Ironworks Model 00770000 5B01 18”x24” with Kitsap County lettering, or approved equal. Revised sheets will not be issued for this note revision.
4. Questions have been asked by prospective bidders. Attached, and part of this
addendum, is a Questions and Answers document dated April 7, 2020.

Questions & Answers – April 7, 2020
The County has received Questions from prospective bidders and below are the County’s responding answers:

Question: Are the anchor bolts for the pedestrian and street lighting poles to be stainless steel or galvanized per WSDOT standards?
Answer: Galvanized per WSDOT standards
Question: The trenchless installation procedures listed for this project are by a slurry face Micro Tunnel Boring Machine, however would other methods of installation be considered?
Answer: As verbally provided at the Pre-Bid meeting, the County will consider a VECP as provided for in the WSDOT/APWA Std Specifications.
Question: Would pilot tube be a considered method of installation for the
trenchless portion of this work? Answer: We are open to pilot tube technology as long as the installation method
can achieve the tolerances shown in the special provisions and the Contractor has a history completing projects with similar tolerances.
Question: In the dewatering specifications 2-13.2(3) section D & F. It discusses
both approved drilling methods and that soil sampling is required for pumped wells, recharge wells and on every 5th well point. As the approved drilling methods in section D are not typically used to do the soil sampling in section F. Can a hollow stem auger be used to drill the hole and soil sample. Then high pressure jetted water be used to flush the hole after extraction of the auger to remove any fines. Thus flushing the hole to create a quality water well?

Answer: The drilling methods and sampling requirements provided in the contract documents are correct and shall be used. The request to use flight augers and perform post-drilling jetting of the hole in lieu of the specified drilling methods is not approved.
Question: Does this project have a UDBE requirement? Answer: No Question: On drawing DS03 sheet 33 the details for the different wells show a
concrete layer on the top. Typically temporary wells would not have a concrete seal at the top as they will be removed. Is this an oversite and no concrete is required?
Answer: Although the wells are temporary, they require concrete around the monument/locking cap over the wells. The wells need to be sufficiently robust to withstand the traffic and work going on around them and to be protected from damage throughout the course of the work.
Question: Is a sonic drill rig allowed to install the pumped wells and recharge
wells? Answer: Sonic drilling would be acceptable for drilling the dewatering wells and
recharge wells. For sonic drilling of the wells, the requirements remain to prevent bottom heave and to maintain water head greater than the piezometric head present in the aquifers inside the cased holes during drilling, soil removal from the drilled holes, and well construction.
Question: The Storm assembly on drawing D02, sheet 17 shows a gasket. The
storm assemblies have (2) 1” pick holes and do not have a gasket. Please let us know if a gasket will be required.
Answer: Gasket is only required on assemblies provided for sanitary sewers. No gasket is required for storm assemblies.
Question: Will the castings for this job require WSDOT inspection and
stamping? Answer: We typically require WSDOT inspection and stamping, regardless of
funding source, however, with COVID-19 pandemic and WSDOTs limited availability for fabric inspections, we will make other arrangements if necessary. We do expect to have castings inspected and preferably stamped.

Question: The project calls for a 30” SEWER Kitsap Logo cover, which not commonly produced?
Answer: Kitsap logo on 30" lids for Schedule B, sanitary sewer, will not be required but must be marked "SEWER". Additionally, open pick holes in 30" lids must be provided with rubber plugs.
Question: 26” Kitsap logo cover comes in a 4” frame that has been used in the
county for years. – This project is calling for 6” frames that would require a new assembly.
Answer: No 6" frames for 26" and 30" lids are required, only 4" frames. See Detail 22/DS01. The sewer Easement Clean-out detail (detail PD-13/DS01) calls for the top of the lid to be 6" above finish grade in non-paved/graveled areas, which could possibly be interpreted as a 6" frame. The frame and lid shall be as required in Detail 22/DS01, thus a 4" frame.
Question: In the “Roadway Notes”, Note 5, on sheets C01-C10, calls for vaned
grates per WSDOT B-30.30 (20x24 assembly). Kitsap County typically doesn’t use a 20x24 assembly, only the 18x24 assembly.
Answer: Kitsap County will not require that a 20x24 assembly have Kitsap County lettering. It is the County’s intent to use 18x24 assembly in lieu of 20x24 assembly.
