    What this booklet is aboutMost people will experience mental health problems at some

    point in their life. Hartlepools Beautiful Minds is a partner-ship project comprised of a range of organisations working

    across Hartlepool that deliver mental health services. Theproject is dedicated to promoting good mental health. This

    booklet is designed to give residents of Hartlepoolknowledge on mental health services that can offer support

    Healthwatch

    Healthwatch is the new independent consumer champion for both health

    and social care. It has two distinct forms Local Healthwatch at local level,and Healthwatch England at national level. The aim of local Healthwatchwill be to give citizens and communities a stronger voice to influence andchallenge how health and social care services are provided within theirlocality. Local Healthwatch will also provide or signpost people toinformation to help them make choices about health and care services.

    Local Healthwatch will also:

    Have a seat on the local Health and Wellbeing Board ensuring that

    the views and experiences of patients, carers and other service users

    are taken in to account within key local strategies such as the Joint

    Strategic Needs Assessment (JSNA).

    Enable people to share their views and concerns about local health

    and social care services and help to build a picture of where services

    are doing well and where they can be improved.

    Be able alert Healthwatch England or Care Quality Commission

    (CQC) where appropriate, to concerns about specific care providers and

    other health and social care issues.

    Provide people with information about their choices and what to do

    when things go wrong

    Give authoritative evidence based feedback to organisations re-

    sponsible for commissioning or delivering local health and social care

    services.

    Help and support Clinical Commissioning Groups (CCGs) to make

    sure that local services really are designed to meet citizens needs.

    Be inclusive and reflect the diversity of the community it serves.

    Who the Service is forThe service is for adults aged 18 years + presenting with an acute mental healthneed that may require admission to a mental health unit without the potential in-

    volvement of an intensive home treatment team.

    Being referredThe Crisis Team can be accessed by the following:

    Substance Misuse Services.

    Primary Care Health Professionals including GPs

    Accident and Emergency Departments/Acute Hospitals

    Social Services/Emergency Duty Team

    Police/Forensic Medical Examiner working in custody suites

    Improved Access to Psychological Therapies (IAPT)

    Self ReferralsThose individuals who are currently open to secondary mental health servicesand their respective families and/or carers can contact the team during out ofhours for additional support or advice.

    Description of ServiceThe service provides assessment and intensive home treatment in a range of

    settings and offers an alternative to inpatient care. The service also responds toservice users in a mental health crisis with deteriorating mental health thatrequires a change in management and increased support.

    The Crisis Resolution and Intensive Home Treatment Team is multi discipli-nary. It comprises medical, nursing, social care, occupational therapy, supportand administration staff. These are available 24 hours a day every day.

    Who the Service is for

    DISC is a service appropriate for individuals over 18 years old whose drugand/or alcohol use is problematic, living in the Hartlepool area and who wishto make positive lifestyle changes.Being referred

    Description of ServiceDISC provides what is known as wrap-around support to adults withsubstance misuse issues. The project supports those people who want tostop, reduce or control and recover from their drug or alcohol use. We helppeople with; psycho

    -social interventions, community reintegration, pre-

    scribed treatment and detoxification, signposting for further treatmentaround a range of social issues, access to residential rehabilitation, peereducation and support programmes, counselling services and structuredprogrammes with group work and one to one sessions. Service users areat the heart of our work within DISC, and we adopt a client centred attitude.Our workers are innovative and proactive in their approach and workingstyles. The project works in partnership with other key professionals under

    one roof within a multi-agency setting.

    Who the Service is forAdults 18+ with a diagnosed mental health need who are a resident of

    Hartlepool.Being referredWe accept referrals from a variety of agencies. This includes statutoryservices, GPs and other agencies that work with adults who have diagnosedmental health needs. For more information or to request a referral formplease contact Choices for Life.

    Description of ServiceChoices for Life is an innovative day service for adults with mental healthneeds that puts participants at the heart of their decisions. As part ofCreative Support, a nationwide not for profit provider of personalisedservices we encourage and support project members to access a broadrange of in house sessions, one-to-one support and community basedactivities and events. We work in partnership with a range of servicesacross Hartlepool.

    At the centre of our service is our social enterprise, Creative Caf, acommunity hub open to the general public that sells delicious homecooked food using locally sourced ingredients. The Caf also has anexciting selling and exhibition space and offers voluntary opportunitiesallowing individuals to get work experience, learn new skills and gain

    qualifications.

    Creative Support:Hartlepool Personalised Services

    Description of ServiceCreative Support provides bespoke packages of support to peoplewith Direct payments and other forms of self-directed support funding.We can support you to: Get out and about on a one-to-one basis,Budget effectively, Take part in group or one-to-one day or eveningactivities and trips, Access benefits, Pay utility bills, Go shopping,

    Attend hospital or other important appointments, Access appropriatehousing, Develop coping strategies, Achieve any other identified in

    your support plan. We also provide traditional domiciliary care servicesto people aged 18 or over. This includes help in the home with tasksthat you might find difficult, such as preparing meals, administeringmedication and other aspects of personal care.

    All of our services are fully compliant in all CQC essential standards.We offer access to advice 24 hours a day 365 days a year via our outof hours on call service and operate a safeguarding report centre with

    a seniorpractitioner who is on duty 24 hours a day, 365 days a year.

    Hartlepool Carers

    Description of ServiceHartlepool Carers can offer support and information on issues con-nected with your caring role:

    Information and advice on local services, benefits, financial andhousing problems, employment, education and training

    Counselling and emotional support

    Health and social care

    Complementary therapies, including massage, reiki, reflexology

    One to one support

    Volunteer help with dog walking, gardening, DIY, sitting services,shopping etc.

    Group social activities

    Advocacy and representation

    A dedicated young carers team also offering a 'Think Family' support

    service.

    Who is the Service ForHartlepool carers support and information to carers in their role.

    A generally accepted definition of the term Carer is as follows:

    A carer spends a significant proportion of their life providing unpaid support to fami-

    ly, friends or neighbours. This could be caring for a relative, partner or friend who isill, frail, disabled or has mental health or substance misuse

    problems.

    Description of ServiceThe service provides assessments and short-term treat-

    ment of up to six weeks for people with mental health needsthat cannot be met within primary care. The aim of theseassessments is to determine whether you would benefitfrom secondary mental health services and to signpost orrefer you to an appropriateservice.

    Who the Service is forThe service is for people who may require secondary mental health

    services such as treatment from the affective or psychosis teams orspecialist services such as autism assessments. Following referralfrom your GP, the service will assess you and identify the mostsuitable service to meet your needs.

    Being referredTo be referred for assessment by the service, a referral from yourGP is required. Following referral, an assessment will usually be ar-ranged to take place within 2 weeks.

    Hartlepool and East Durham Mind

    Who the Service is forThe In Mind service is appropriate for anyone who is experiencing emotional

    distress such as anxiety, stress and depression.

    Being referredThe In Mind service has a GP referral system established. We also acceptself referrals. Organisations can request a referral from and refer their clientsto the service.

    Description of Service

    In Mind: Emotional Wellbeing and Psychological Service.We offer one to one Counselling and Psychological Therapy such as:Cognitive Behavioural Therapy (CBT), Interpersonal Therapy (IPT),Solution Focused Brief Therapy (SFBT), Couples Counselling andBereavement Counselling. We offer one to one private and confidentialcounselling to help people overcome difficulties such as anxiety, stress,depression, eating disorders, self harming, and anger management. We

    provide support and teach new techniques and strategies such as:relaxation skills, sleep management, communication skills, assertivenessskills. We work towards improving self confidence, self esteem andwellbeing.

    In accordance with NICE guidelines all staff work professionally andcollaboratively with each client to develop individual personalisedrecovery plans. It also provides people with the skills and the practical andemotional support they may require to improve their capacity to overcomeproblems and difficulties to better manage their daily lives.

    Lifeline Project Families and CarersSupport Service

    Who the Service is for

    The service is for family members/carers or any person who isaffected by the substance misuse (drugs or alcohol) of anotherperson including young people.

    Being referredService users can self-refer into the service or be referred from anyorganization they are currently working with e.g. social services

    Description of ServiceThis service offers family support and counselling to thosepeople affected by the substance misuse of another person.

    This includes family mediation, one to one counselling andcouples counselling. Family support includes family activities,educational groups including mental health awareness anddrug and alcohol awareness sessions, parent and child ses-sions as well as trips out. We also run the Community Rein-forcement and Family Therapy group (CRAFT) supporting fami-ly members to encourage change in the substance user.
