    Group Life Insurance

    Group life insurance provided through USAble in the amount of 1 X Annual Salary to a max of$150,000 and a $15,000 death benefit in the Major Medical coverage.These life insurance benefits cover the employee only at no charge to employee. (taxable benefit)

    Short Term DisabilityOn the first day of incapacitation due to accident not covered by other income (eighth day of

    disability due to sickness, excluding maternity) this policy will cover the employeeby paying a disabilitybenefit of 60% of the employees monthly salary each month (up to a maximum of $1,500 per week) for 13weeks (until the Long Term Disability waiting period is over). Employee salary is bonused up for the

    premium with amount added to salary and then deducted so that payroll taxes are applied. Therefore, whenthe benefit is utilized, it will not be taxable.

    Long Term Disability

    After 90 days of incapacitation, when the Short Term Disability policy coverage ends, this policy willcover the employeeby paying a disability benefit of 60% of the employees monthly salary each month (upto a maximum of $5,000 per month). Employee salary is bonused up for the premium with amount addedto salary and then deducted so that payroll taxes are applied. Therefore, when the benefit is utilized, it willnot be taxable.

    Additional Portable Insurance Coverage

    Additional coverage can be purchased at the employees expense for up to an additional $100,000.00 of Life and AD&D with supplemental coverages for spouse and

    children up to $25,000.00 and $10,000.00, respectively;

    up to an additional $100,000 of AD&D with supplemental coverages for spouse and children up to$50,000.00 or $60,000.00 and $15,000.00 or $20,000.00, respectively, depending upon the planchosen;

    cancer insurance including 20 other specified diseases supplement.401-K Plan

    Provides immediate eligibility for the employee to save for retirement. There is a 5 year vestingschedule on all matching contributions paid by the employer. The employer will match your first 6 % of

    contributions per pay period at 50%. Therefore if you contribute 6% of salary to the plan each pay period

    you will receive a company contribution of 3% on top of that. The maximum match by the company is 3%per pay period.

    Vesting applies only to employer contributions. At the completion of 6 years of service the employer matchwill be 100% vested.
