We are pleased to announce the ARH Employee Benefits program for 2013!

ARH’s overall benefit plan has long been recognized and valued because of its comprehensive and competitive offerings in the market. In addition to an array of traditional benefits such as health care, dental care and retirement, ARH also offers a wide range of family-friendly benefits to attract, support and retain talented employees that help ARH remain a premier healthcare system.

Our benefit program in 2013 will include a Medical and Pharmacy Program, three Dental Plans to choose from, Cancer Insurance, Critical Illness coverage, Accident coverage, Vision Insurance, Voluntary Basic Life Insurance, Dependent Life Insurance, Supplemental Life Insurance on yourself, spouse, or dependents and a Universal Life Insurance with Long Term Care plan. Our disability insurance offers most employees the choice of Voluntary Short Term and Long Term Disability coverage. Employees will continue to have the option of enrolling in the Flexible Spending Accounts, ARH Employees Federal Credit Union, and Direct Deposit. A 529 savings plan for college education will be introduced at a later date. More information will be forthcoming on this program.

This is not intended to give you complete knowledge of each policy, but to give you some information that might help you decide which options may be of interest to you. There are different levels of coverage available and options to cover yourself, your spouse, or your family. We will have consultants to meet one on one with all employees this year. For more details see carrier specific information.

ARH is happy to introduce Star Robbins & Company, located in London, KY, as the benefit consultants selected to help

with this year's Open Enrollment process. The consultants will work with employees to provide one on one enrollment

education and guidance, which will make it easier for employees to better understand their benefits.

We have worked closely with the Star Robbins team to provide individualized benefit understanding we feel may be

needed to support each of you during this decision making process. Because there are a few changes, ARH thought

having someone to explain the current benefits and what the changes will be would be helpful to all employees.

All ARH employees will need to re-enroll in their benefits in order to have coverage effective 01/01/2013. Employees will

also need to bring their dependents social security numbers with them in order to enroll their dependents in coverages.

Once Open Enrollment is completed, the Star Robbins team will deliver year round support to our facilities. A schedule will

be introduced at the beginning of 2013 which will allow ARH employees to access benefit guidance as well as support

throughout the year to help with qualified changes and filing claims through Allstate Benefits and Trustmark.

We are excited to be able to provide this type of guidance and support to our employees in 2013.

Appalachian Regional Healthcare continues to work to provide medical coverage at affordable rates to its employees. Basic coverage will remain the same for benefits with a few changes to the ARH Health Plan that need to be noted.

There are three tiers of coverage to help you manage your health:

Tier 1

Service is provided at ARH-owned or ARH-approved providers and offers the greatest level of coverage:

• Deductibles/out-of-pocket apply at ARH approved providers for basic services in a physician’s office.

• New this year -- co-pays in Tier 1 for services such as Advanced Imaging, surgeries, and ER

Tier 2

Service is not provided at local ARH hospital. Must use ARH approved Network-see schedule on the following pages.

Outpatient Surgery Centers approved in KY—see schedule on the following pages.

West Virginia members may use Highmark BCBS participating surgery centers in the members area.

Tier 3

Service is provided at local ARH hospitals, but member choose to go outside ARH to Humana/ChoiceCare Network (Kentucky Employees) or Mountain State BCBS (WV Employees).

Prescription Benefits

Medical coverage includes a prescription benefit which allows the member to pay a percentage of the drug cost or minimum co-pay, whichever is lower. Mandatory ARH program for maintenance drugs.

ARH does not provide coverage if the applicable

carrier’s network is not utilized.

There will be a spousal surcharge if your spouse has an alternative source of coverage through his

or her own employer and chooses not to enroll in that coverage.

Part-time employees will pay a flat premium, rather than the pro-rated percentage of plan cost.

Prescription Benefits

Dental Coverage Employees have three voluntary dental plans to choose from for their oral health.

USW employees have the option of choosing union dental benefits or you may apply for one of the ARH dental options. It is important to note that you can enroll in one or the other, but not have both coverages.

Once enrolled in a benefit with ARH it cannot be cancelled unless there is a qualifying event.

USW and SUN refer to your collective bargaining agreement for your current prescription benefits.

FSA - Flexible Spending Accounts Today, we need every dollar we get to help with family budgets. Flexible Spending Accounts are one way of providing additional cash flow. They are designed to allow you to take part of your taxable “wage” dollars and turn them into non-taxable “benefit” dollars, which eliminates the tax on income.

Two major areas that qualify for FSAs are out-of-pocket medical expenses and dependent care expenses. Medical expenses include deductibles, co-payments, dental care, eye care, etc. Dependent care expenses can be reimbursed for care of a child under age 13, or a disabled dependent in your care, while you and your spouse work.

ARH provides both of these accounts to our employees. If you incur medical or child care/ dependent expenses, you should consider taking advantage of this IRS benefit. The FSA allows you to save state, federal, and FICA taxes on out-of-pocket medical expenses.

The FSA program provides a Debit Card to all enrollees. Once activated, the Debit Card is loaded with the amount you elect to contribute to your Health Care FSA. Every time you use your Debit Card to pay for eligible health care expenses, corresponding deductions will be made from your card balance. BMS may ask for copies of your receipts so be sure to save them! BMS may need to verify that your expenses are eligible for reimbursement under IRS guidelines.

Health Care FSA ~ can set aside up to $2,500 per year to pay for qualified, out-of-pocket medical expenses

Examples of qualified expenses include, but are not limited to: Prescription drugs, doctor’s office co-pays, co-insurance, deductibles, orthodontist & dental fees, prescription glasses & contacts, etc.

Dependent Care FSA ~ can set aside up to $5,000 per calendar year to pay for qualified dependent care expenses

Examples of qualified expenses include, but are not limited to: Babysitter, before/after school programs, licensed nursery care, day

care, elderly care, summer day camp, etc.

You have until March 15, 2014 ~ to incur a claim for the 2013 plan year. You have until March 31, 2014 to submit your FSA claim for

reimbursement. Any funds left over in your account will be forfeited; there are no rollovers or refunds.

Vision Coverage

Vision coverage is offered on a voluntary basis. As with the dental, the USW can elect the union vision plan or the one ARH offers, but not both. The vision plan allows employees and their dependents to see in or out of network providers. Please note, however, that the costs may be higher out of network.

The plan offers coverage for exams, lenses, contact lenses, and frames. Members can also receive a discount on additional services including LASIK.

Disability income protection insurance provides a benefit for “long term” disability resulting from a covered injury or sickness. Benefits begin at the end of the elimination period and continue while you are disabled up to the maximum benefit duration.

Benefits include:

• 90 day elimination period

• 60% of covered earnings up to a monthly maximum

VALUE-ADDED SERVICE: Travel Assistance Service

Through your group coverage with Reliance Standard, you will now automatically receive travel assistance services provided by On Call International (On Call). On Call is a 24-hour, toll-free service that provides a comprehensive range of information, referral, coordination and arrangement services designed to respond to most medical care situations and many other emergencies you may encounter when you travel. On Call also offers pre-trip assistance including passport/visa requirements, foreign currency and weather information.

Voluntary Disability is an important part of your financial security. Disability is designed to help cover the immediate needs should someone become disabled due to accident or illness and be unable to work for a period of time.

ARH eligible employees may enroll during open enrollment. The benefit pays direct to you.

Short Term Disability Income Protection

Long Term Disability Income Protection (LTD)

Voluntary Basic Life, AD&D, and Dependent Life

Benefits include:

• 24-hour coverage-On and off the job.

• Pregnancy is covered the same as any illness.

• 60% of your weekly earnings up to $1,500 per week maximum

• Benefits begin on the 31st consecutive day of disability if due to an illness and 1st day if due to an accident.

• 13 week benefit duration

• 12/12 Pre-existing condition exclusion, including pregnancy

If you are pregnant at the time of enrollment, it will be considered a pre-existing condition and will not be covered.

No Evidence of Insurability (EOI) will be required.

Employees have the opportunity to purchase life insurance as follows:

• Employee. One times your salary up to a maximum of $100,000 (rounded to the next (higher) $1,000)

• Spouse. $5,000 or $10,000

• Child. 1 4 d a y s but less than 6 months: $500; 6 months to 25 years if full time student: $5,000

Employees must purchase coverage for themselves in order to cover dependents. Other features include:

• Accelerated Life Benefit. If your life expectancy is 12 months or less, you can receive a percentage of the benefit prior to your death.

• Accidental Death & Dismemberment Benefit (AD&D) (If employee elects Basic Life Insurance ARH will pay for the AD&D benefit)

• Bereavement Counseling Service - Reliance Standard offers access to a toll-free counseling service to all beneficiaries who experience the loss of a loved one. Professional counselors who are experienced with the human emotions associated with the death of a loved one are available to help those who want to reach out. These counseling services are available to you free of charge through Health Management Systems of America (HMSA). Access to HMSA’s network of legal and financial advisors is available to all beneficiaries through the same toll-free number. For confidential, personal assistance, please call 1-800-961-3007.

• Travel Assistance Service (See Information Under LTD)

Benefit Age Reduction Information

• At age 65, the original benefit is reduced to 65%.

• At age 70, the original benefit is reduced to 40%.

You may be able to convert this policy, please contact the carrier for additional information.

Supplemental Voluntary Life Insurance provides additional coverage at affordable group rates. Some individuals may want to purchase additional term life insurance. If this is of interest, the supplemental voluntary life insurance provides additional coverage at affordable group rates. You choose the amount of coverage that’s right for you.

Spouse and Dependent coverage is available if you participate in coverage on yourself.

The maximum amount of coverage for your spouse can be no more than 50% of your benefit amount (up to a maximum of $250,000).

Supplemental Voluntary Term Life Insurance

How does Guaranteed Universal Life work? Universal Life is permanent life

insurance that provides affordable guaranteed protection for your family. It gives you death benefits that you can depend on and policy payments that remain the same throughout the life of your policy. Most policies also endow at age 100, which means benefits are payable to you in full if you live to be 100. Universal Life also comes with an extra level of protection for long-term care services, combining two important benefits into one affordable product. Your benefits can be paid as a Death Benefit, as Living Benefits for long-term care, or as a combination of both.

How are your benefits paid? Your benefits can be paid as a Death Benefit,

as Living Benefits for long-term care, or as a combination of both.

Death Benefit - Most people buy life insurance for the financial security of the death

benefit. A death benefit puts money in your family’s hands quickly when they need it most. It’s money they can use any way they want to help cover short- and long-term expenses.

Living Benefits - Long-term care can be expensive. Living Benefits make it easy to

accelerate the death benefit to help pay for home healthcare, assisted living, adult day care or nursing home services, should you or your covered spouse ever need them.

Features you’ll appreciate

Lifelong protection – Provides coverage that will last your lifetime.

Family coverage – Apply for your spouse even if you choose not to participate.

Terminal Illness Benefit – Accelerates up to 75 percent of your death benefit if

your doctor determines your life expectancy is 24 months or less.

Guaranteed renewable – Guaranteed coverage, as long as your premiums are

paid. Your premium may change if the premium for all policies in your class changes.

Separately priced benefits:

Waiver of premium – Waives policy payments if your doctor determines you are

totally disabled.

EZ Value – Automatically raises your benefits to keep pace with your increasing

needs, without additional underwriting.

•Employee. You may purchase up to $500,000 of coverage in $10,000 increments.

•Spouse. You may purchase half of your amount of elected coverage up to $250,000 in increments of $10,000.

•Child. You may purchase, for eligible children to age 19 (or 25 if full-time student): $1,000 from 14 days to 6 months; $10,000 from 6 months through age 19 (or 25 if full-time student).

Guarantee Issue for newly eligible is $200K on employee and $25K on spouse.

Other features include:

deciding between treatment options to managing every day needs to maintaining financial stability. Critical Illness insurance can help. It can help you manage your illness, your way. Critical Illness insurance pays a lump-sum cash benefit, upon the diagnosis of a covered illness, to help ease not just your financial worries, but your emotional ones too.

What’s covered?

Heart attack Stroke Invasive cancer (excludes most skin cancer) Renal (kidney) failure Blindness Occupational HIV ALS (Lou Gehrig’s disease) Transplant of a major organ Paralysis of at least two limbs

Subsequent condition benefit with a recurring condition – Pays a lump-sum cash payment when you are diagnosed with any and every

covered condition included in your policy. There are no limits to the number of payouts for each insured family member and no reduction in pay-outs for later diagnosed conditions. (Coronary artery bypass and carcinoma in situ are limited to one payout for each condition, which will not re-duce any subsequent benefits.) Benefits are also payable for recurrence of the same covered condition previously paid under the contract, except for cancer, carcinoma in situ or coronary artery bypass.

Health Screening Benefit - To help you stay well, the Health Screening Benefit

pays the cost of one screening test or immunization per calendar year ($100 maximum).

limits are up to $20,000 during initial offering ONLY!

Guaranteed renewable – Guaranteed active coverage for life, as long as premiums

Level premiums – Enjoy rates that don’t increase because of age.

Hospitalization/treatment – Not required to collect benefit. You receive benefit

payment after diagnosis.

Best Doctors® – Receive the one-on-one support of Best Doctors, a leader in connect-

ing you to the medical information you may need for covered conditions.

ARH employees are being offered a “one time” guaranteed issue option on certain voluntary benefits during this enrollment. Guaranteed issue means that individuals with prior medical conditions can obtain coverage without going through extensive medical underwriting. NEW Accident Coverage

We do not like to think about someone in our family facing treatment for cancer. However, the American Cancer Society estimates one-half of all men and one-third of all women will be diagnosed with cancer in their lifetime. You may not be able to prevent cancer, but you can guard against the financial burden that can come with treating this illness and allow yourself to concentrate on combating the disease.

Would you be able to survive financially while being treated for cancer?

The cost of cancer is not limited to the expensive treatments. There is also a cost for travel expenses, missed time at work, and other non-medical expenses your health insurance might not cover.

Cancer coverage is available for the individual and the family. All benefits are paid directly to you unless assigned to a doctor or hospital. These benefits are also paid in addition to any other coverage you have in place.

In addition to cancer, the Allstate Benefits policy has benefits for 29 additional diseases, including muscular dystrophy, multiple sclerosis, and meningitis. It also has a benefit for Intensive Care stays for any reason you are in an ICU unit should that rider be elected. Flexible coverage types built into one plan provides excellent, affordable benefits.

protection on or off the job”

An accident can wreak havoc on your savings if you’re not prepared. That’s why there’s accident insurance. It gives you a cushion to help cover medical expenses and living costs when you get hurt unexpectedly.

Medical fees up to $500 will be reimbursed in the event of an accident requiring medical care. These fees are limited to physician fees, X-rays, emergency room services, and repair to natural teeth if diagnosed by the dentist to have been caused by an injury.

There is a benefit of $50 for each visit to a doctor, outside of the hospital, for any reason, two times per year for individual and four times per year for family. Even regular check ups are included.

For each covered person, the policy will pay a one-time benefit of $1,000 per event when the covered person is admitted for an overnight stay in the hospital as a result of an injury.

The policy pays a $200 per day benefit for hospital confinement with a maximum of 90 days per injury. This amount increases to $400 per day if admitted to intensive care.

The benefits of this policy include $40,000 accidental death insurance. You may also be paid up to $40,000 if an accident results in dismemberment and up to $4,000 for a fracture. Only dismemberment and fractures listed in the policy schedule are covered.

The Accident and Cancer coverage is provided by Limited Benefit Insurance. It is also Supplemental - it pays in addition to any other insurance.
