WHAT IS AN EXECUTIVE SUPPLEMENTAL MEDICAL PLAN?

BeniComp Select is a fully insured, supplemental, group executive medical expense reimbursement insurance policy. It allows employers to reimburse their key employees for medical expenses not otherwise covered by health insurance.

Additional Premium Paid claims + 10% administration premium

Accidental Death Benefit equal to annual

TAX ADVANTAGES AND BENEFITS

• Charges for the treatment of infertilityup to 10% of annual maximum percalendar year

It is strongly recommended that interested parties seek the advice of tax counsel when considering adopting an Insured Medical Supplemental Plan.

COVERED BENEFITS

Generally, if an expense is medically necessary and qualifies under Section 213 of the Internal Revenue Code, it would be eligible for reimbursement under this plan. Individuals must be under the care of a legally qualified physician to receive reimbursement.

Elective procedures that are not medically necessary are not covered by BeniComp Select.

GETTING STARTEDFill out an Application OnlineFill out an online application form and provide a payment of $250 per employee per year. The payment will be processed once your group has been approved.

Upload your Summary Plan Description (SPD)An SPD upload button is provided within the online application if the participants have employer-sponsored health insurance.

a. If any potential participants are individually insured, they must attach their individual health plan’s SPD to

their online enrollment card.

b. If any potential participants have Medicare, they must attach a copy of their Medicare card and their

spouse’s Medicare card, if applicable, to their online enrollment card. Potential participants on Medicare

must have Parts A and B and an additional supplement plan, such as Medicare Part D or F.

Provide us with a list of participantsSend us a list of the participants joining the plan along with their email addresses. These emails will be used to send participants their online enrollment cards.

Potential participants fill out an Enrollment Card and EFT FormEach potential participant will receive an email to fill out an online Enrollment Card and EFT Form (optional).

Submit the Business Associate Agreement Once BeniComp reviews and approves the application, an online Business Associate Agreement will be submitted to the employer contact to sign.
