1. The ‘Various Health Checkup Offers’ are arranged exclusively for the ‘Bharat Diamond Bourse’ Members, Associates, Staff & their family.

2. Checkup requires Single Blood Sample and Urine Sample & ECG as per package chosen.

3. Maintaining 10 – 12 hours Complete Fast is Compulsory (i.e., No Food Item, Tea, Coffee, Milk or excessive Water) & preferably avoid stress/exercise before giving blood.

4. Camp Venue : Old Members Relations Office, Next to Blossom Restaurant, ‘G’ Tower, Ground Floor, BDB.

5. Camp Timing : 08.30 AM to 11.00 AM

6. Camp Day & Date : Choose from any of the following dates

Duly filled in overleaf Registration Form needs to be submitted at the Members Relations Center, GE, Ground Floor of Bharat Diamond Bourse on working days between 12.00PM to 05.00PM before Friday, 15th April 2016 along with payments (which can be made by Cash or by Cheque in favour of “Thyrocare –Charniroad”).

8. Kindly note, against registration, you will be provided with an acknowledgement slip along with required sterilized urine bottles and medical history forms which are to be submitted duly filled at the time of Blood collection.

9. Members are requested to note that it is mandatory to handover Acknowledgement Receipt at the time of Blood Collection where you will be provided your named Barcoded vials for the Blood Collection ensuring no errors on part of wrong blood in a wrong vial.

10. Members can also choose to organize the camp at the convenience at their office with no extra cost if the group size is more than 10 members. Contact on phone numbers mentioned in point # 13. And fix the appointment for suitable dates.

11. Members can also avail Home Collection Facility by paying additional Rs.200/- per visit within Mumbai City.

12. Home collection will be arranged as follows: a) The appointment will be given 1 day before visit to your house b) Thyrocare will call and fix a suitable appointment c) In event that the day given is unsuitable another appointment will be made at a later date

(again 1 day in advance) d) Suitable appointment will be given on First–come-First serve basis and location grouping e) The home collection drive will continue till all registered participants are serviced f) Collection times are allotted between 08:00 am to 11:00 am.
