Surgical & Prosthetic Development for Delayed Implant Placement in The Aesthetic Zone

Soft tissue management in cases of late implant placement is different than in cases of immediate implant placement where the soft tissue can be supported immediately after surgery with a temporary crown. After extraction or staged GBR the soft tissues collapsing and has to be constructed with the help of Mucogingival surgery and prosthetic soft tissue development.

The lecture will demonstrate in a structured manner the decision which lead to delate instead of immediate implant with the associated consequences and the following steps in the development of emergency profile abutment and crown to achieve an ideal soft tissue architecture. These cases need a team approach for ideal aesthetic /prosthetic implant placement and emergence profile development.

This program will cover:• Ideal implant placement according soft tissue quality-Biotype.• Emergence profile development for late loading.• Correlation between implant position/prosthetics &soft tissue outcome.

Step by Step demonstration of the Algorithm for soft tissue development (abutment and crown design) for late restoration will be demonstrated.

Soft Tissue Flexibility for Predictable Bone AugmentationDr. Marius Steigmann

Bone grafting procedure is part of implant dentistry today. To make these procedures predictably successful makes Learning the skills off soft tissue management absolutely necessary. One of the most frequent complication of these procedures is flap opening. To be able to cover different volumes of bone graft with the soft tissue, makes gain of flexibility for different quality’s/quantity’s /biotype of the soft tissues a necessary procedure. Different regions of the mandible and maxilla requires specific individualized manipulation of the flap (PRI, Split thickness Flap, MDP) for covering the graft and specific suturing techniques to maintain the flap closed.

The lecture will categorize the procedures to gain soft tissue flexibility according to soft tissue thickness/Biotype and the region which requires bone grafting.

Lateral vs. crestal approach in sinus elevation surgery: A revisited clinical visionTiziano Testori MD, DDS, FICD

Maxillary sinus elevation surgery has evolved during the last 25 years to a point where it is considered the most successful augmentation procedure performed today and the survival rate of implants placed in these grafts is as high or higher than that of implants placed in the non-grafted posterior maxilla. The sinus elevation procedure is still evolving due to the development of new surgical techniques and new biomimetic technologies, however, there is still no unanimous consensus on when to adopt the lateral or crestal approach. This lecture will address both evidence-based and the latest state-of-the-art techniques.

Maxillary sinus surgery complications: prevention, diagnosis and treatmentTiziano Testori MD, DDS, FICD

Sinus elevation surgery is nowadays a predictable therapeutic technique to restore the atrophic maxilla, however each surgical step may give rise to complications.

Anatomical limitations such as thickness of the lateral bony wall, the thickness of the Schneiderian membrane, the presence of septa and of unusual large vessels can influence the intraoperative complication rate.

Membrane perforations can be predictably resolved without jeopardizing the overall success rate.

Sinus complications may occur intra-operatively, in the early postoperative period, or in the late postoperative period, late complications may occur months or years after surgery and are due to improperly treated maxillary sinusitis.

The aim of the lecture is to give clinical recommendations on how to prevent, diagnose, and treat intra and post-operative infections.

Flap Design Incision and Suture in the Aesthetics Zone - Hands-OnDr. Marius Steigmann

Soft tissue management incision flap design and suture are of major importance in the aesthetic zone. Iatrogenics in the aesthetic zone lead to papilla loss, mid-facial recession at implant site and also at the adjacent teeth. For this reason, exact planning off the flap design, according to the anatomy of each patient, is necessary to avoid this soft tissue complication. Specific incision for each indication will be described. Different flap designs according to the anatomy will be shown and an overview of most suturing techniques necessary for the aesthetic zone.

Participants will exercise on pig jaws the most common Incision, Flap design and sutures for grafting in the aesthetic zone using specific instruments for soft tissue manipulation developed by Dr. Steigmann.

Dr. Marius SteigmannDr. Marius Steigmann graduated the school of Dental Medicine in Neumarkt in 1987. In 2005 Dr. Steigmann received his PHD Summa cum laude from University of Neumarkt.

Dr. Steigmann is Adjunct Clinical Associate Professor at the University of Michigan Dpt. of Periodontics, Adjunct Assistant Professor of Oral and Maxillofacial surgery at Boston University, Adjunct Assistant Professor at University of Pennsylvania Dpt. of Endodontics and Honorary Professor of the “Carol Davila” University Bucharest. He is a Diplomate of the ICOI

and a member of the board of the DGOI. He also received the medal of “Semmelweiss” Budapest University Dental School Dept. of Oral and Maxillofacial surgery. He is the founder and Scientific chairman of “Update Implantologie Heidelberg” 2002-2011 and the founder and director of the “Steigmann Institute”. Dr. Steigmann maintains a private practice in Neckargemünd, Germany.
