PPE must be provided, used, and maintained. PPE must be adequate. All PPE must be of safe design and

construction. Employer must assess the workplace to

determine hazards. Damaged or defective equipment must not be

used. Employers must provide training.

Prior to Entering a Confined Space

All personnel have been trained

Proper permit has been issued

Appropriate PPE is in place and utilized

OSHA’s procedures are in effect
