• Please register horse and rider information before July 30 by MAIL. • All registrations received after July 30th or done the day of show will be subject to a $10 fee. • Please include a check payable to BTA/BOLT with your entry form. It will not be cashed until after the show. • Current negative Coggins required (within 12 months for MA; 6 months for out of state) • Check www.btabolt.org for more information.

I certify that this horse/pony and rider are eligible for the classes entered and are subject to the rules and regulations as stated for the 2014 BTA/BOLT Dressage and Horse Show. I enter the above horse/pony and rider at my own risk. I agree to make no claims against the show, BTA/BOLT, its members, volunteers or committees, or the Moore family for loss or injury to person, animals or property while on the show grounds. By signing this release, you agree to let BTA/BOLT use your and/or your child's photo in any BTA/BOLT promotion material, including its website.
