WHAT IS BEING JUDGED?
Applicants will be assessed on their overall standard of achievement and potential, taking all the information provided in this nomination form into consideration. Three finalists will be selected for each category from whom one will be announced as the winner. Judges are selected by Master Builders and no correspondence will be entered into, as the decision of the judging panel will be final.
Please complete each section of this entry form. Address all criteria and where there is insufficient space attach any additional information. Please include photocopies of testimonials and/or awards, trade results and trade certificates.

AWARDS OVERVIEW
Applicants will be assessed on their overall standard of achievement, including; Work ethic Personal skills Trade skills Relationship with fellow workers and contractors Ability to work unsupervised Ability to think laterally Level of competency Academic results and awards Non-academic awards, prizes or achievements Work undertaken for the community or other work experience
All forms must be fully completed, signed and received by 10 August 2017. Master Builders reserves the right to refuse any entry. All apprentices must be or have been an apprentice between 1 January 2017 and 3 November 2017. The judges must be satisfied that the entrant meets the criteria established for the awards. Apprentices must be registered with the WA Department of Training & Workforce Development. All documents submitted remain the property of Master Builders. Judges decisions are final. Judging will be carried out by a panel of judges, comprising of industry representatives. Judging will be based on written submissions, face to face interview and/or telephone interview.

Use this simple checklist to ensure all of the entry requirements have been met, prior to submission. A completed entry form. A description of the apprentice that addresses the judging criteria (Max 1pg per criteria). Supporting documentation (Eg. copies of results, awards & experience references). Please provide one (1) photo on CD of your apprentice - one head shot at a resolution of 300dpi, image size of 210mm x 297mm in jpeg format. The photo will be displayed at the awards night, so please provide a respectable image.
The top Apprentice from each category will compete for the WA Master Builders Apprentice of the Year Award. Presentations will be held on Friday, 3 November 2017 at the Perth Convention and Exhibition Centre.

Please indicate the category for which the candidate is being nominated by marking the applicable box/es below. Apprentices can be nominated for more than one category.
Direct Indenture: The apprentice must be or have been indentured to an employer in the 2017 calendar year who is a current Master Builders member (builder or sub-contractor).
Bricklaying: Candidates undertaking a Bricklaying or Bricklaying (Housing) apprenticeship in the 2017 calendar year. Candidates may be from a Group Scheme or directly indentured to an employer.
Carpentry: Candidates undertaking a Carpentry, Joinery or Carpentry and Joinery apprenticeship in the 2017 calendar year. Candidates may be from a Group Scheme or directly indentured to an employer.
Wall and Floor Tiling: Candidates undertaking a Wall and Floor Tiling or a Tile-laying apprenticeship in the 2017 calendar year. Candidates may be from a Group Scheme or directly indentured to an employer.
Electrotechnology (Electrician): Candidates undertaking an apprenticeship in Electrotechnology. Candidates may be from a Group Scheme or directly indentured to an employer during the 2017 calendar year.
Best Female Building and Construction Apprentice: Female apprentice who is employed in a construction trade by any employer or by a Group Scheme during the 2017 calendar year.
Best Indigenous Building and Construction Apprentice: Indigenous apprentices in any construction trade. Candidates may be from a Group Scheme or directly indentured to an employer during the 2017 calendar year.

The winner of the 2017 Master Builders Apprentice of the Year Award automatically receives a nomination into the Master Builders Australia Apprentice of the Year Award.
This award will be presented at the Master Builders Australia National Awards on 25 November, 2017. Master Builders is keen to promote the training of all apprentices through its annual Apprentice Awards.
