Building Bridges: A Practical Approach to Root Cause Analysis Are your patient safety events reoccurring? Do you see the same harmful events affecting your patient again and again? Do your corrective action plans include only terminating the employee and educating the staff who remain? If the answer is yes to any of these questions, you may want to consider the ECRI Institute educational program “Building Bridges: A Practical Approach to Root Cause Analysis”. The goal of the program is to build the bridge between theory and practice, creating a culture of patient safety that supports the identification of sustainable solutions that prevent harm to patients. The program includes the ECRI Institute team patient safety analysts onsite for two days presenting our Building Bridges educational program. In a two-day format, courses are provided by our ECRI faculty as on Systems Analysis, Facilitation, Event Investigation and Management, Identifying Root Causes and Developing and Monitoring Prevention Actions. Throughout the first day, participants have the opportunity to develop a post training action plan to document take home learnings that they can transfer back to the work place. On day #2 participants have the opportunity to apply the learned theory to realistic case studies of actual patient safety events. The program includes continuing educational credits physician and nurses. All attendees receive certificates of attendance. Recommended attendees The attendees should include leadership, clinical directors, supervisors and mid-level management. Departments that should consider registering staff include quality, patient safety, risk management, nursing and medical staff. The organization should consider including those who facilitate and attend Root Cause Analysis at their facilities. Pricing available upon request and dependent upon number of attendees.
