II. SELF-DETERMINATION OF ELIGIBILITY

Before completing the rest of this application, please answer the questions in this section, which are designed to assist the firm with determining whether the minimum requirements have been met to be eligible for certification. Please complete Parts A, and B below to find out if you meet the requirements of the Certified Commissioning Firm (CCF) Program.

This section is included for the sole purpose of helping to make a reasonable determination of whether you should proceed with this application. The firm is not guaranteed admission to candidacy solely on the basis of the answers to the questions in this section. The BCCB will evaluate your entire application and make a final determination of your eligibility. The decision of the BCCB will be final.

Part A. General Qualifying Criteria

1. Does the firm employ a minimum of one full-time Certified Commissioning Professional (CCP)?

2. Has the firm been providing commissioning services for a minimum of 24 months with no current bankruptcy filings or commissioning contract terminations for cause?

Part B. Commissioning Project Experience and References

Has your firm completed—as the lead commissioning firm, involved from project design through end of construction—three or more qualifying commissioning projects for new construction, existing building, or major renovations of buildings with a cumulative total of at least 150,000 square feet and having a construction value of at least $30 million? (See the details about qualifying projects and references below before responding.)

Qualifying Commissioning Projects:

Select at least three (3) projects for new construction, existing building, or major renovations on which the firm served as the lead commissioning firm from project design through end of construction. The buildings, considered together, must total at least 150,000 square feet in size and have a total construction value of at least $30 million. If necessary, you may add additional projects to meet the square footage and dollar value minimums.

A qualifying commissioning project is defined as a project where the firm has served as commissioning provider firm and performed the Essential Work Scope items for each project. Please note that in order to count as one (1) qualified project all Essential Work Scope items must have been performed. Some projects may not contain all Essential Work Scope items; therefore applicants may choose to combine projects, no more than two, for a single qualifying project. When combined, the lesser of the two square footage and construction

values will be used toward the square footage and construction value minimums. When combined, these two separate projects must include all Essential Work Scope items and a separate client reference is required for each. The minimums listed above for square footage and construction value must be met for each of the Essential Work Scope items cumulatively across the three or more projects. For each qualifying project the Project Experience Form (Appendix A) and Client Reference Form (Appendix B) must be completed and returned to the BCCB.

Existing building commissioning experience may be submitted as a qualifying project provided it meets the Essential Scope. The intent is that the firm also has some experience in new building commissioning. New building commissioning experience will likely be required in order to meet the square footage and construction value requirements of the submitted projects. Regardless of new or existing building experience, the applicant may submit as many projects as necessary to achieve these requirements.

Projects Submitted by CCP: If the on-staff CCP submitted projects and references while employed at the applying firm, those projects and references may be used to complete this application requirement. A minimum of one project must have been completed within the last 24 months. For example if the CCP was certified more than two year ago two of the projects from that application can be used and one recent project and reference must be submitted. CCP materials need not be resubmitted; project names should be referenced on page 6 of this application.

Definition of Major Renovation: A major renovation may include, but is not limited to, the replacement of the building envelope, lighting system, HVAC system, and other systems that have substantial impact on energy usage. It is a renovation that costs more than 25% of the building’s replacement value.

Client References:

Obtain a client reference for each of the qualifying projects submitted via the Project Experience Form (Appendix A). If the firm worked on more than one qualifying project for the same client, you must obtain a reference from a different person for each project. At least one of the client references must be a direct representative of the facility owner. Each reference must complete a Client Reference Form (Appendix B) and return it to the BCCB. You must attach a copy of Appendix A to Appendix B and send it to your reference who will verify the information provided on Appendix A. References must submit their form to the BCCB directly. Please enter the firm’s name on the top line of the form before giving it to your references. You have the option of entering an application deadline on your reference form. This date is irrelevant to the BCCB but has been included if you believe your reference will be more responsive if given a deadline.

You are responsible for distributing the forms to your references and ensuring that they are received by the BCCB. Please check with your references or with the BCCB to verify that the forms have been submitted.

X. BCA Essential Attributes of Building Commissioning

The BCA and BCCB consider the BCA Essential Attributes fundamental to effective building commissioning. Please read these carefully prior to moving on to the Affirmations of the Applicant section.

1. The commissioning authority (CxA) is in charge of the commissioning process and makes the final recommendations to the owner regarding functional performance of the commissioned building systems.

2. The CxA is an objective, independent advocate of the owner. If the CxA's firm has other project responsibilities, or is not under direct contract to the owner, a conflict of interest exists. Wherever this occurs, the CxA discloses, in writing, the nature of the conflict and the means by which the conflict shall be managed.

3. In addition to having good written and verbal communication skills, the CxA has current engineering knowledge, and extensive and recent hands-on field experience regarding:

a. Building systems commissioningb. The physical principles of building systems performance and interactionc. Building systems start-up, balancing, testing and troubleshootingd. Operation and maintenance procedurese. The building design and construction process

4. For each project, the commissioning purpose and scope are clearly defined in the CxA contract.5. The CxA recommends the commissioning roles and scope for all members of the design and construction teams be

clearly defined in: a. Each design consultant's contractb. The construction manager's contractc. General Conditions of the Specificationsd. Each division of the specifications covering work to be commissionede. The specifications for each system and component for which the suppliers' support is required

6. Each project is commissioned in accordance with a written commissioning plan that is updated as the project progresses. The commissioning plan:

a. Identifies the systems to be commissionedb. Defines the scope of the commissioning processc. Defines commissioning roles and lines of communications for each member of the project teamd. Estimates the commissioning schedule

7. On new building commissioning projects, the CxA reviews systems installation for commissioning-related issues throughout the construction period.

8. All commissioning activities and findings are documented as they occur. These reports are distributed as they are generated, and included in the final report.

9. The functional testing program objectively verifies that the building systems perform interactively in accordance with the project documents. Written, repeatable test procedures, prepared specifically for each project, are used to functionally test components and systems in all modes of operating conditions specified for testing. These tests are documented to clearly describe the individual systematic test procedures, the expected systems response or acceptance criteria for each procedure, the actual response or findings, and any pertinent discussion.

10.The CxA provides constructive input for the resolution of system deficiencies.11.Every commissioning project is documented with a commissioning report that includes:

a. An evaluation of the operating condition of the systems at the time of functional test completionb. Deficiencies that were discovered and the measures taken to correct themc. Uncorrected operational deficiencies that were accepted by the ownerd. Functional test procedures and resultse. Reports that document all commissioning field activities as they progressf. A description and estimated schedule of required deferred testing

XI. AFFIRMATIONS OF FIRM

Read each of the following statements carefully and have a firm principal or another individual with authority sign and date. By signing and dating below, you are affirming, on behalf of your firm, that each statement is true. If you cannot truthfully attest to all of the statements, your firm is not eligible to be certified. Please note that, should the BCCB determine that you have falsified any attestation, your application may be rejected or your certification may be revoked.

A. I certify that the information provided in this application is true, accurate, and verifiable to the best of my knowledge. Further, I agree to notify BCCB of any material change in the information provided in this application.

B. I affirm the firm has provided building commissioning professional services for a minimum period of twenty-four (24) months with no bankruptcy filings or commissioning contract terminations for cause.

C. I affirm that the firm currently employs, full-time, a minimum of one Certified Commissioning Professional (CCP) and understand that a CCP must be held on staff to maintain this certification. If the CCP leaves the organization, I understand it is the firm’s responsibility to notify the BCCB and provide information about how the firm will obtain a new CCP. The firm will not be eligible to renew the certification if a CCP is not on staff at time of renewal and if a CCP is not identified within one (1) year of the renewal date the certification will be revoked.

D. I certify that the firm has not been convicted by any court, licensing board, or registration board of violating the law in conjunction with the performance of the firm’s professional work.

E. I understand that, once the BCCB processes this application, the fee for this certification program is non-refundable.

F. I understand that I have twenty four (24) months from the date the BCCB receives this application to fulfill all application requirements, after that time a new application and fee will have to be submitted.

G. I understand that BCCB certification is valid for a period of one (1) year. Should the firm be granted certification, I understand that if the firm seeks recertification, it is the firm’s responsibility to demonstrate evidence of continued competence in the field of building commissioning.

H. I understand that BCCB reserves the right to verify any or all information on this application and that any incorrect or misleading information may constitute grounds for rejection of the firm’s application, revocation of the firm’s CCF certification, or other disciplinary action. I authorize BCCB and their officers, directors, employees, agents, and assigned examiners to review this application to determine whether the firm has met BCCB’s standards for certification.

I. The firm indemnifies and holds harmless BCA, BCCB and their officers, directors, employees, agents, and assigned examiners from the decision made on my application as long as such decision was made in good faith and does not constitute gross negligence by BCA, BCCB or their officers, directors, employees, agents and/or assigned examiners.
