President 2016-17

In the summer of 1917, only a few months afterthe United States entered the first world war,Rotary held its eighth annual convention in Atlanta.Although many Rotarians at the time thought theconvention should be canceled, the Board ofDirectors ultimately agreed with Paul Harris that itshould continue as planned. In the midst of suchuncertainty and fear, Harris penned, as part of hisconvention greeting, some of the most-quotedwords in Rotary:

Individual effort when well directed can accomplishmuch, but the greatest good must necessarilycome from the combined efforts of many men.Individual effort may be turned to individual needsbut combined effort should be dedicated to theservice of mankind. The power of combined effortknows no limitation.

Fittingly, it was at this convention that then-President Arch C. Klumph proposed a Rotaryendowment fund “for the purpose of doing good inthe world.” The power of combined effort wasjoined by a new power: that of combinedresources. It was a combination that has provedunstoppable and has been behind so much ofRotary’s work for the last 100 years. Today, it isdifficult to imagine Rotary without its Foundation. It was the Foundation that turned Rotary from anorganization of local clubs into an internationalforce for good with the power to change the world.

In this Rotary year, we are marking the centennialof our Rotary Foundation in the city where it allbegan: Atlanta. Our 108th Rotary InternationalConvention promises to be one of the most excitingyet, with inspiring speakers, great entertainment, and a wide array of breakout sessions to help youmove your Rotary service forward. And of course,we’ll be celebrating the Foundation’s centennial instyle.

Whether you’re a regular convention goer, haven’tbeen to one in a few years, or haven’t yetattended your first, the 2017 convention will be theone you won’t want to miss. Atlanta is a greatdestination in its own right, with great food, friendly people, and many local attractions toenjoy. But the real reason to come to theconvention is always the convention itself, and thepeople, ideas, inspiration, and friendship you’ll findthere. To learn more, and save money on

Message from theFoundation TrusteeChairSeptember 2016

Kalyan Banerjee

Trustee Chair 2016-17

A few months ago, I read a story in this magazineabout a man named Carl Sanders, a member of theRotary Club of Kenosha, Wis. Sanders haddeveloped a successful painting business despitethe fact that he could not read – a shameful secretthat he struggled to keep to himself.

This story surprised me a little. I tend to think ofilliteracy as a problem that mainly afflicts people inpoor countries, not U.S. Rotarians. But Sanders’situation is not so uncommon. Even in a wealthycountry like the United States, millions of peoplelack basic reading skills.

Sanders’ story had a happy ending. He shared hissecret with a fellow Rotarian, who steered him to alocal literacy program and encouraged him as hetackled his reading lessons.

Our Rotary Foundation wants to create more such

happy endings, and there is no shortage of peoplewho need them. Today, more than 750 millionadults are functionally illiterate globally.

In 2015-16, our Foundation awarded 146 globalgrants totaling $8.3 million to support basiceducation and literacy projects worldwide. Theseprojects vary considerably – from providingcomputers and school supplies in Ghana tosponsoring an after-school homework program inthe U.S. to developing a literacy and mentoringprogram for Roma girls in Bosnia, a project thataddresses the gender imbalance that exists inmany parts of the world.

In my country, Rotary has been on a literacymission for the past few years. India has apopulation of 1.2 billion and is about 75 percentliterate. Illiteracy occurs mainly in rural India, where most people live. So Rotary in India joinedhands with the government to eliminate illiteracy, especially among women, because literate womenraise literate families, ensuring a better future forall. Indeed, the numbers are staggering, and whenit is done, the impact could be incredible.

As we observe Basic Education and Literacy Monthin September, let’s think about the millions ofpeople whose chances for success remain blockedby illiteracy. Our Foundation is helping many ofthem, but with Rotarian support and involvement,we can do so much more.

Doug demonstrated his versatility not only at thewheel, but also in the galley: After a short cruisefrom Richmond to Ayala Cove on Angel Island,Doug gave the crew liberty while he prepared a**** grilled salmon lunch. Following chow-time,we set course through Raccoon Strait into thelarger bay, with fabulous Golden Gate Bridgeand Palace of Legion of Honor views, then aroundAlcatraz and back into Richmond Marina. A harborporpoise, cormorant and more than a few gullscompleted the wildlife visits. The six satisfiedsailors will likely collaborate on this adventure atthe Club's upcoming Culinary Classic November

Ironman triathlete MindaDentler challenges world toend polio

Minda Dentler becomes the first woman handcyclist to complete the 2.4-mile swim, 112-milecycle, and 26.2-mile marathon of the IronmanWorld Championship in Kailua-Kona, Hawaii, USA

I was born in 1978 in Mumbai to a domesticworker and single mother. At six months old, I wasparalyzed from the hips down by polio. Thechances of surviving in India until your 18thbirthday with a disability are very slim. My motherwas unable to care for me and left me at anorphanage. I don't remember much about my timethere because I was so young, but I know theconditions were primitive. I had no real hope thatmy life would become anything of note or that Iwould have the opportunity to be independent andovercome the burden of a very preventabledisease.

I didn't know it then, but several years after I wasborn, a revolution in the way the world approachedpolio prevention came to India. That revolution wasthe Global Polio Eradication Initiative, which hasreduced global polio cases by 99.9 percent since1988. But like millions of others in India, I neverreceived the two drops of oral vaccine that protectagainst the virus. In India, your health is vital toyour social and economic opportunities. If you arehealthy, you can get a job, and if you have a job,you can get married. Unfortunately, this simplepassage of convention seemed beyond my reach. But then my life changed irrevocably once more.

At age three, I was adopted by Bruce and AnnDentler and joined their family of two children andanother adopted son, from Korea. I moved toSpokane, Washington, USA, shortly after my thirdbirthday. Over the next few years, I underwent aseries of surgeries on my hips, legs, and back tostraighten my body, and I could eventually walkwith leg braces and crutches. My parents had thesame expectations of me as they did of my siblingsand set the tone that having a disability should notprevent me from doing whatever I wanted to do

with my life. I had to do the same chores and domy homework. It was a very happy childhood.

I loved to compete, so I threw myself into manyactivities, from debating at school to playing thepiano. I graduated from high school and moved toSeattle to study business at the University ofWashington. While in college, I interned at theWhite House and IBM. I studied abroad in Spainand backpacked through Europe by myself, wearing my leg braces and crutches. Upongraduating, I moved to New York City for amanagement consulting job. I pursued an MBA, gotmarried, and now work at a large multinationalinsurance company. Through my example, I hopepeople can see that a disability shouldn't hindersomeone from living a full and productive life.

While living in New York, I met Dick Traum, thefirst amputee to complete the New York CityMarathon in 1976. Dick later founded a nonprofit,Achilles International, which provides free trainingand support to help people with disabilitiesparticipate in sports. He gave me a hand cycle, which is a three-wheeled recumbent bicyclepropelled by the arms, and encouraged me to trainfor a marathon. This opened up a new world ofopportunity for me, and I completed the New YorkCity Marathon in my hand cycle in 2006.

My next challenge was thought to be impossiblefor a female wheelchair athlete: the IronmanTriathlon. I made the transition to triathlon andfinished my first Ironman in Louisville, Kentucky,USA, and qualified for the world championship inKona, Hawaii, in 2012.

The Ironman Triathlon requires a wheelchairathlete like me to swim 2.4 miles, hand cycle 112miles, and push a racing wheelchair 26.2 miles, allwithin tight time limits for each stage of thecourse. But at the Kona Ironman, I failed to makethe 10½-hour cutoff time for the cycling portion. Iwas disappointed, but I'd faced harder setbacksbefore. The failure steeled my determination, and Idecided to regroup and try again the next year.

By October 2013, I was back at the starting line forthe Kona Ironman in Hawaii for the second time. Iwas bidding to become the first woman handcyclist in history to finish the Ironman WorldChampionship. Just as my parents had insisted thatI complete the same chores as my siblings, theIronman event demanded that I complete thecourse within the same strict time limits as everyother able-bodied competitor. I had qualified forthe race and earned the right to compete on alevel playing field, but if I did complete the race, itwould mean something more than achieving

another personal goal.

Every stroke in the water and crank forward on myhand cycle were movements for those who couldnot lift limbs paralyzed by polio. With everyrotation of the wheels on my racing wheelchair, Iwas moving forward for the millions of poliosurvivors who would never get this opportunity. When I finally crossed the finish line 14 hours and39 minutes after I started, I was overwhelmed withjoy and excitement. It was a storybook ending andthe realization of a dream that seemed impossibleto achieve.

I'd followed Rotary's polio eradication efforts forsome time when I had the honor of being invited tospeak at a World Polio Day event in 2014. Sincethen, I've been one of Rotary's polio ambassadors, helping to raise awareness for the End Polio Nowcampaign. In this role, I was offered an opportunityto return to India for the first time since I was achild.

Last year I set off for the country where mostpeople said polio could never be eradicated. Butagainst the odds, one year after my first successfulIronman World Championship, India did eradicatepolio – despite the challenges of crowded slumswith poor sanitation, the second largest populationin the world, the weakened immune systems ofmillions living in poverty without propernourishment. Despite all this, Southeast Asia wascertified polio free in 2014.

The enormity of this achievement is clear if youconsider that less than a decade ago, Indiareported almost half of the world's new polio cases. But until the disease is eradicated everywhere, itcould easily return. So on my trip, I participated ina National Immunization Day, when 172 millionchildren through age five are immunized againstpolio.

One of the most memorable moments for me wasmeeting a polio survivor named Parveen at St.Stephen's Hospital in New Delhi. It was a starkreminder of a tale of two worlds. Here she was, thesame age as me, but we are living very differentlives. I was adopted and catapulted into a life ofprivilege. At age 37, Parveen is illiterate, withoutresources, and has been a burden on her family.

I do not want to see other children become victimsof polio and suffer the lifelong effects of apreventable disease. It was heartbreaking to me,and, as a mother, I want for her and all children inthe world, no matter their circumstances, to have achance at a healthy life. Rotary is changing theworld, one child and two drops of vaccine at a

time.

I've had good fortune at various stages of my life. Iwas adopted by a loving family after three years inan orphanage. I was given my first hand cycle byAchilles International in New York. I had thesupport of my family to push me across the finishline in Hawaii. But I hope readers realize that mystory is also one of empowerment and personalchoice.

Whether you are a polio survivor, a supporter ofthe polio eradication effort, or even someone whois surprised polio is still a threat – we all have animportant choice to make. We can choose to haveour children vaccinated and ensure that otherparents in our communities do the same. I knowwhat it is to miss out on this life-changing vaccine,as my childhood wasn't the same. In India, I alsomet Rukhsar Khatoon, the country's lastdocumented polio victim, and it made me realizethat when we finally do end polio, our work will notbe over.

There are 10 million to 20 million polio survivorsworldwide, and they need more than physicalrehabilitation. It will be another lifetime's work toensure that every polio survivor has access to agood education and to prevent stigmatization incommunities or the workplace because of aphysical disability. The least we can do in thepresent is to make the choice to prevent moreneedless suffering by vaccinating our children. Andsoon, our children, and their children, will live in aworld without polio. Just imagine.

To the surprise of some, today's Rotary is quitedifferent from the Rotary they joined years ago. New members identify more with theirengagement in Rotary projects than they do withany perceived image or status associated withmembership. Simply put, newer members, especially Millenials in particular are more doersthan joiners. Their Rotary identity is a function oftheir participation.

Following considerable, careful study of

"Psychographics" of members and non-members,Rotary International adjusted their recruitment andretention approach more toward participation, engagement of members, rather than merelybelonging.

Within the 62 clubs comprising District 5130's 6county span, Rotaract offers individuals with valuesparalleling Rotary to become active in their localcommunities as well as International opportunitieswith different levels of financial commitment. Twoclubs are active in Humbolt County and seven inSonoma. Rotaractors and Rotarians find veryfulfilling relationships through informal vocationalmentoring.

"Engaging members is the key to a successfulmembership". Wulff reminded everyone thatRotary has projects for all ages and skill levels. Young and older alike want relevance and valuein their commitment of time and resources. If theyfind those, they remain active Rotarians. Wulffreminded us The Rotary Foundation is one of onlythree in the world to be ranked the #1 non-profitPhilanthropic fund. It has the support of the Billand Melinda Gates Foundation in eradicating Polioworldwide.

Just how is Rotary visibly changing? Last year anew pin was introduced, combining both the wellrecognized Rotary Wheel with our name: "Rotary".We are encouraged to wear our pin and beprepared to tell why we are Rotarians: How are weengaged in our local, national and Internationalcommunities? What keeps us in Rotary?

Wulff concluded his address with his specialengagement with Rotary: Peace Initiatives. Describing war as the temporary repeal of reasonand virtue, Wulff spoke of Rotary International'spriority of promoting peace through it's PeaceScholars and Institutes. He urged us to collaboratewith other clubs in establishing a Place of Peace: Mark a place with a message, a plaque, a banner,in a park, create a garden. Build a place to helppeace grow!!

Surgeons from India bringrelief to underservedpatients in Rwanda

Hundreds of people gather in an open-air courtyardat University Central Hospital in Kigali, Rwanda.Men in suits, women in flowered dresses, evenprisoners in pink and orange gowns are waiting tofind out if they will receive medical care. Somehave no visible signs of injury. Others arrived oncrutches, with arms in slings, or with cathetersprotruding from their clothing. Several haveswollen, broken limbs: injuries that should havebeen mended long ago but were neglected becauseof the country’s long surgical-ward backlog, orsimply poverty.

Emmanuel Mugatyawe, 36, sits on the ground as afriend fills out his yellow admissions form. He hasbeen waiting two months for an operation to repaira broken leg – now infected – that he sustainedwhen a car plowed into his motorbike.

“These are not routine cases; there are very fewfresh injuries,” says Shashank Karvekar, anorthopedic surgeon and member of the Rotary Clubof Solapur, India, after he and his Rwandancolleague Joel Bikoroti examine several dozenpatients, scheduling many for surgery. Over thenext eight days, a team of 18 specialized doctors(12 of whom are Rotarians) will perform surgerieson 268 Rwandan patients, including procedures inorthopedics and urology. The trip, initiated byDistrict 3080 (India) and hosted by District 9150(Central Africa), is funded by The RotaryFoundation with support from the Rwandangovernment. It’s the fourth medical mission toRwanda that the two districts have organized since2012. This time, among the volunteers is K.R.Ravindran, the first sitting RI president to take part

in the mission.

A few buildings down on the University CentralHospital’s campus (referred to as CHUK), RajendraSaboo, 1991-92 Rotary International president, isbusy coordinating the last-minute logistics of themission. The 82-year-old from Chandigarh, India, has done this many times. After finishing a post-presidential term on the Board of Trustees, Sabooand his wife, Usha, began to look for ways toparticipate in the type of hands-on service theyhad long encouraged of their fellow Rotarians.

They wanted to help India, a country that oftenreceives outside assistance, make a stronger globalcontribution. It didn’t take long for Saboo to focuson medicine. He found that many local doctors hadtrained or worked in limited-resource settingssimilar to what they would find in Africa. “Ourdoctors are medically very strong,” Saboo explains.“And because India also does not haveinfrastructure of the highest level, they’ve learnedhow to innovate.”

Saboo’s first mission, to Uganda, took place in1998 and focused on cataract surgeries andcorrective operations to help disabled poliosurvivors. Organized with Rajiv Pradhan, apathologist and past governor of District 3130, itconsisted of doctors from Saboo’s district (3080)and Pradhan’s.

Today, Saboo recalls the mission as a life-alteringexperience – one so successful that the two soonarranged a trip to Ethiopia. That visit marked thestart of an 18-year partnership that has broughtmore than three dozen surgical missions to 12African countries, as well as Cambodia and six ofIndia’s least developed states. Over time, themissions have increased in frequency to four peryear, while adding specialties such as plasticsurgery, urology, and gynecology. Saboo has beenon almost every trip. “Raja Saboo is absolutely fullof energy,” says Pradhan. “He’s constantly thinkingof new ways to support medical missions. Even atthis age, he’s working 12 hours a day.”

Rwanda, a compact central African country withmountainous topography that often drawscomparisons to Switzerland, is perhaps best-knownfor its darkest moment: the slaughter of up to amillion citizens, mostly members of the Tutsiminority, in the 1994 genocide. Twenty-two yearslater, it’s one of the fastest-growing economies inAfrica. Kigali, its capital, is among the tidiest citieson the continent. Since 1994, life expectancy hasmore than doubled in Rwanda while maternal andchild mortality rates have fallen.

Rwanda still faces public health challenges, however. Access to surgery is among them.According to The Lancet, an estimated 5 billionpeople, including nine out of 10 residents of lower-and middle-income countries, do not have accessto “safe, affordable surgical and anesthesia carewhen needed.” In these countries, the Britishmedical journal notes, 143 million additionalsurgical procedures are needed every year.Although most Rwandans are covered by nationalhealth insurance, which gives them access to low-cost care, many people living in rural areas cannotafford to get to a public health facility. Moreover, surgery is only available in five of the country’spublic hospitals, and many patients must wait tobe referred from local health centers or district-level facilities.

Aside from a minority of patients who can affordprivate care, complex cases wind up at one of twopublic hospitals in Kigali: CHUK and RwandaMilitary Hospital, which also hosted doctors fromthe mission. A persistent shortage of surgeonsmeans there’s typically a long waiting list.According to Faustin Ntirenganya, who heads thedepartment of surgery at CHUK, the hospitalemploys just 10 surgeons and threeanesthesiologists – a staffing shortage that, attimes, means a backlog of up to 1,000 cases.Despite a growing number of surgical residents atRwanda’s national university, the lure of better-paying jobs abroad makes holding on to specialistsdifficult, Ntirenganya says. “Our biggest challengeis numbers,” he says. “Our limited team cannothandle the needs of the whole population.”

The Rotary mission helps meet the high demand. In four trips to Rwanda, Saboo’s teams haveconducted nearly 900 surgeries. For some patients, the mission represents a final chance. MichelBizimungu, who had been out of work sincerupturing a patellar tendon playing soccer lastOctober, was told his case could be handled only atRwanda’s top private hospital, at a price farbeyond his means as a cleaner. Then his case wasreferred to Asit Chidgupkar, an orthopedic surgeonand member of the Rotary Club of Solapur.Although Chidgupkar had never encountered thisspecific injury, and CHUK lacked some neededequipment, including biodegradable screws andsuture anchors, Chidgupkar devised a plan. Thenext day, in a four-hour procedure involving threeseparate incisions, he repaired Bizimungu’s knee.Chidgupkar called the procedure an “absoluteimprovisation.” (He later presented the case at anorthopedic conference in India, and he keeps intouch with Bizimungu, who updates himperiodically on his recovery.) “It’s one of my most

memorable cases,” he says.

The mission also provides training. Mission doctorsteach cutting-edge surgical techniques to localphysicians, medical students, and residents. Duringsurgery, the visiting doctors demonstratetechniques and learn from host country doctors.Bosco Mugabo, a fourth-year resident in surgery atthe University of Rwanda who assisted Chidgupkarwith Bizimungu’s operation, says the opportunitywas invaluable. “There are some tricks and hintsthat you don’t learn from school,” he says. “Youlearn them from a specific surgeon.”

With this in mind, Saboo worked with local healthauthorities to slightly modify the Rwanda mission. At a dinner in Kigali, he announced plans to invite10 Rwandan doctors to India for three-monthstints of training there – part of an effort to boostlocal capacity in a more sustainable manner. Thenext mission to Rwanda will also be smaller andfocus more on teaching two in-demand specialties:reconstructive urology and anesthesiology. Inaddition, 20 Rwandan children will undergo open-heart surgery in Saboo’s home city of Chandigarh. With travel funds from the Rwandan Ministry ofHealth, 30 Rwandan children have already receivedsuch operations there. According to EmmanuelRusingiza, one of only two pediatric cardiologists inRwanda, the country’s high rate of rheumatic heartdisease, which generally results from untreatedcases of strep throat, means the country has awaiting list of more than 150 children. “A bignumber of them are passing away,” he says. “It’s avery hard situation.”

As the mission in Kigali winds down, Saboo isalready looking forward to the next one. With moreIndian districts interested in sending doctors, andAfrican districts interested in hosting them, heexpects the number of trips to increase, even if hisown attendance becomes less frequent.

Many mission participants, both first-timers andveterans, say they plan to return, though itsometimes entails a significant personal andprofessional sacrifice. Karvekar, whose own sonunderwent heart surgery in India just days beforehe traveled to Kigali, is one of them. “I’d wantedto go on one of these trips for a while,” he says, noting that the mission was his longest absencefrom his family’s private clinic, where he’s the onlyorthopedic surgeon on staff. “There were a lot ofchallenging cases, but fortunately we were able todo them well and, I think, give the patients a goodresult.”

“It is totally a labor of love,” adds Saboo, speakingfor himself as well as the team of doctors. “When
