Educational objectives:

1. To understand current thinking about dry eye disease, and recognize that Meibomian gland dysfunction is the primary cause of dry eye syndrome.

2. To recognize dysphotopsias after cataract surgery, and to understand the natural history and potential treatments so appropriate recommendations can be made to affected patients

3. To understand the optics of the new extended-depth-of-focus IOLs, and how these IOLs differ from multifocal and monofocal IOLs

4. To be familiar with current patient reported outcomes (PROs) of LASIK, and the implication of these PROs for patient education prior to LASIK

5. To understand the state of development of small incision lenticule extraction, and its potential advantages and disadvantages.

Who cares, if they are asymptomatic…?

• Answer:

Because these conditions can impact:

 IOL calculations

Inaccurate keratometry can lead to wrong IOL power

LRI or Toric IOL axis and/or magnitude

 Inaccurate keratometry

 Inaccurate topography

Fluctuating vision post-op, especially with PC-IOLs

Irregular Topography from DES

Challenges in Dry Eye Classification

DES Before Tx After Tx • Patients often present with conflicting signs

Low Schirmers (< 5 mm) with a high TBUT (> 7 seconds)

Evidence of staining, but normal Schirmers & TBUT

• Symptoms alone are insufficient to determine severity

• Existing signs show poor correlation to disease severity

Schirmer strips, TBUT, staining have low specificity

MGD Treatment

• Warm compress (with eyelid scrubs?)

• Antibiotic

Systemic tetracycline/doxycycline

Azithromycin

• Immunosuppressive

Restasis

• Topical corticosteroids

• Dietary Supplements

Omega-3

• Tear substitutes

Warm Compresses Thicken the Oil Layer

Warm Compresses

Goto, E. et al (2002). Two weeks of warm

compress treatment:

 Reduced tear film evaporation

 Improved dry-eye symptoms

 Reduced rose Bengal staining

 Improved tear film break-up time

Take Home Messages

DES is no longer a specific diagnosis

 “Aqueous tear deficiency” (ATD)

“Lipid tear deficiency” (LTD) or “MGD”

 Or both

 Routinely express Meibomian glands

Recognize lack of expressions as MGD

Try to resolve central SPK before referring the patient

for surgery

Treatment Ladder Evaluation of Dysphotopsias

MGD ATD Hot compresses Artificial tears

 Omega-3 supplements Punctal plugs (permanent)

 Doxycycline Restasis

 Topical Steroid Xiidra
