The Quality Plan presented herein covers the Remedial Activi­

ties as set forth in Section 4.4 Tailings Piles of the Uravan Reme­

dial Action Plan (RAP) , and as described in the Final Plans and

specifications for Remedial Activities for Tailings Piles (Spec­

ification No. S-404-1), hereinafter referred to as the Specifica­

tions.

The Quality Plan meets the requirements set forth in the RAP in

section 4.4.5 Quality Control/Quality Assurance, Monitoring Per­

formance Evaluation and Inspection, and in Section A4.4 Tailings

Piles in Addendum A.

The Quality Plan consists of the following:

(1) Quality control/Quality Assurance (QC/QA).

1.0 SCOPE OF QUALITY CONTROL/QUALITY ASSURANCE

1.1 OC/OA Included

QC/QA shall be provided for the following:

(1) construction of trenches and sumps for dewatering surface liq­

uids by excavation of the existing tailings for dewatering.

(2) Placement and compaction of contaminated materials on top of

the Tailings Piles.

(3) Placement of riprap on the rockfill buttress on the Tailings

Piles.

(4) Placement of the reclamation cover on the Tailings Piles.

(5) Construction of the drainage ditches and channels .

(6) Installation of monitoring devices .

(7) Submission of Documents and Reports.

The Quality Control Officer or his designated representative,

hereinafter referred to as the QC Officer, shall verify that the

Work is performed in accordance with the specifications as approved

by the State. The key items for Quality Control are listed in the

following sections. Approval of the construction covered in each of

the following sections shall be documented in writing by the QC

Officer in accordance with the Quality Control Procedures listed in

this Quality Plan.

2.2 Dewatering surface Liguids on the Tailings Piles

2.2.1 Trenches, Sumps and Drainage systems

The QC Officer shall verify the following:

(1) Grades are maintained to direct surface water to the back of

the Tailings Piles to a point of collection.

(2) Temporary trenches are excavated and sumps are constructed to

provide surface drainage, reduce ponding and facilitate the

removal of surface water.

(3) Collected surface water from the top of the Tailings Piles is

pumped to the New Evaporation Ponds or the lined Existing Club

Ranch Ponds if ready.

Quality control Procedure QC-10 Observation of Excavation shall

be used.

2.2.2 Compliance Report

A compliance Report shall be prepared for dewatering surface

liquids on the Tailings Piles.

2.3 Placement of contaminated Materials on Tailings Piles

2.3.l Placement

The QC Officer shall verify the following:

(1) Only materials designated for placement on the Tailings Piles

are placed on the Tailings Piles.

(2) Size of scrap placed on Tailings Piles 1 and 2 has a maximum

dimension of 10 feet and a maximum volume of 30 cubic feet.

(3) Scrap having a hollow volume greater than 5 cubic feet is re­

quired in volume by at least 70 percent prior to placement on

Tailings Piles 1 and 2.

(4) Scrap is spread across the Designated Disposal Areas to avoid

nesting and to reduce the volume of voids.

(5) Tailings, neutralized sludge, and/or soils are placed over and

into the scrap in sufficient amount to fill the voids between

the large pieces and the volume within the hollow pieces

recognizing that some voids will remain as provided for in the

Specifications.

(6) The first layer of tailings, neutralized sludge and/or soils

does not exceed S feet in thickness. Subsequent layers do not

exceed 2 feet except for isolated scrap protrusions.

(7) The upper 2 feet of the first layer, and the subsequent layers

of tailings, neutralized sludge and/or soils are compacted to

at least 90 percent of standard Proctor maximum density.

(8) The contaminated materials are placed to contain drainage on

top of the Tailings Piles at all times and where possible, the

working surface is sloped toward the back of the Tailings Piles

at a minimum slope of 2%.

(9) The contaminated materials are placed to achieve the final

Tailings Piles configuration with S(H):l(V} slopes and the tops

of the Tailings Piles are sloped at 3 percent towards the back

of the Tailings Piles.

2.3.2 Quality control Procedures

The QC Officer shall use the following Quality Control Proce­

dures. for Quality Control of the placement of contaminated materi­

als.

The frequency of the Quality Control Tests shall be as follows:

(1) The frequency of the field density and moisture tests shall be

not less than one for each 1,000 cubic yards of soils, tailings

or neutralized sludge placed.

(2) There shall be at least one field density and moisture test for

each layer of material placed.

(3) Frequency of laboratory standard compaction tests shall be such

that the maximum densities are determined for the range of

materials being placed on the Tailings Piles. In addition, the

frequency of laboratory compaction tests shall be not less than

one test for each 10,000 cubic yards of materials placed on the

Tailings Piles.

2.3.4 Compliance Report

A Compliance Report shall be prepared for the placement of

contaminated materials on top of the Tailings Piles.

2.4 Riprap Cover for Rockfill Buttress

The QC Officer shall verify the following :

(1) The riprap from the Burbank Quarry placed meets the require­

ments of Type c Riprap having a maximum size of 48 inches, a

0 50 size of 24 to 36 inches, and a 0 15 size of 3 to 18 inches.

(2) Riprap cover conforms to the lines and grades shown on Drawing

Nos. P-404-3 and P-404-4 of the Specifications.

(3) The riprap is hauled and placed in a single lift thickness in

such a manner to minimize segregation.

(4) The riprap areas receive at least two passes of a D-7 dozer or

equivalent in order to key the rock for stability.

(5) Temporary access roads on the rockfill buttress are regraded,

compacted and covered with the required thickness of Type c

Riprap to conform to the riprap cover in place.

2.4.2 Quality Control Procedures

The QC Officer shall use the following Quality Control Proce­

dures for Quality Control for placement of riprap on the rockfill

buttress.

2.4.3 Frequency of Quality control Observations

The frequency of Quality Control Observations shall be as fol­

lows:

(1) Each load of riprap shall be visually examined by the loader

operator before leaving the Burbank Quarry. The examination

shall be performed by comparing the rock size distribution in

each truckload against a calibrated reference stockpile in

accordance with Quality control Procedure QC-8.

(2) Periodic site inspections shall be made for the riprap place-

ment on the rockfill buttress. Areas of riprap which show an

accumulation of riprap sizes less than the minimum 050 size

shall be removed and blended with subsequent truckloads until

the materials visually conform with the specified gradation.

2.4.2 Compliance Report

A Compliance Report shall be prepared for the placement of

riprap on the Tailings Piles rockfill buttress.

Reclamation cover for Existing 3(H):l(V) Slopes

QC/QA shall be provided for the major items of construction of

the Reclamation Cover on the existing J(H) : l(V) Tailings Piles

slopes. The final cover shall be constructed in accordance with the

Specification including Drawing No. P-404-3.

2.5 . 1 Placement

The QC Officer shall verify the following:

(1) The existing layer of random fill is regraded to a uniform

thickness as practicable and that adjustments in subsequent

random fill thicknesses are made to ensure that the total

thickness of random fill is 1.5 meters or as otherwise speci­

fied.

(2) The existing layer of random fill is compacted to at least 95%

of the standard Proctor maximum density in the areas where the

dry density is less than this specification.

(3) Clayey soil material having at least 40 percent material pass­

ing the No. 200 sieve, a maximum size of 12 inches, a liquid

1imit of at least 20 percent, and a plasticity index of at

least 10 percent is removed from an approved borrow source and

placed as a clayey cover over the existing random fill.

(4) The clayey soil is placed in horizontal compacted lifts of

1-foot or less in thickness and compacted to at least 95% of

standard Proctor maximum density.

(5) The compacted thickness of the clayey soil layer is at least

o.9 meters or as otherwise specified.

(6) Random Fill is inorganic soils having a maximum size of 12

inches and is obtained from an approved Borrow source and

placed in a cover layer over the clayey soil.

(7) The Random Fill is placed in horizontal compacted lifts not

exceeding 1-foot in thickness and compacted to at least 95

percent of the standard Proctor maximum density.

(8) The thickness of the existing and newly placed random fill

totals at least 1.5 meters or as otherwise specified.

(9) Riprap excavated from the Burbank Quarry or other approved

Borrow source having a maximum size of 24 inches, a 0 50 size of

12 to 18 inches, and a D15 size of 3/4 inch to 3 inches

(Type B), is placed over the Random Fill.

(10) The Riprap is placed in a single lift in such a manner as to

minimize segregation, and receives at least two passes of a D-7

Dozer or equivalent in order to key the rock for stability.

(11) Accumulation of riprap sizes less than the minimum 050 size is

removed and blended with subsequently delivered loads to meet

the desired gradation.

(12) The riprap layer is at least 0.6 meter thick.

2.5.2 Quality control Procedures

The QC Officer shall use the following Quality Control Proce­

dures for Quality Control of the placement of the reclamation cover

The frequency of Quality control tests and observations shall

be as follows:

(1) The frequency of the field density and moisture tests shall be

not less than one test for each 500 cubic yards of placed fill

material (random fill or clayey cover}.

(2) There shall be at least one field density and moisture test for

each lift of fill material placed .

(3) Gradations and Atterberg limits of compacted fill materials

shall be performed at a frequency of not less than each 1,000

cubic yards of placed clayey cover or each 5,000 cubic yards of

compacted earthfill.

(4} For fill materials where moisture content tests are correlated

with optimum moisture content as determined by the standard

Proctor compaction tests (ASTM D-698), frequency of moisture

content tests of compacted fill material shall not be less than

one test for each 500 cubic yards of compacted fill material.

(5) For fill materials where field density test of compacted fill

materials are correlated with laboratory maximum density as

determined by the standard Proctor compaction test {ASTM

D-698), frequency of field density testing for compacted fill

material shall not be less than one test for each 500 cubic

yards of compacted fill material.

(6) Frequency of laboratory standard Proctor compaction tests shall

be such that maximum densities are determined for the range of

materials being placed in the fill; however, frequency of com­

paction tests shall not be less than one test for each 5,000

cubic yards of compacted fill material.

(7) Each load of riprap shall be visually examined by the loader

operator before leaving the Burbank Quarry. The examination

shall be performed by comparing the rock size distribution in

each truckload against a calibrated reference stockpiled in

accordance with Quality Control Procedure QC-8.

{8) Periodic site inspections shall be made for the riprap place-

ment on the random fill. Areas of riprap which show an accumu-

lation of riprap sizes less than the minimum 0 50 size shall be

removed and blended with subsequent truckloads until the mate­

rials visually conform with the specified gradation.

2.5.4 Compliance Report

A compliance Report shall be prepared for the placement of the

reclamation cover on the existing 3(H) :l{V) slopes of the Tailings

Piles .

2.6 Reclamation Cover for S{Hl:l{Vl Slopes and Top

2.6.l Placement of Reclamation cover

The QC Officer shall verify the following:

(1) The reclamation cover is constructed in accordance with the

Specifications including Drawing Nos. P-404-3 and P-404-4.

(2) During placement of the contaminated materials, an interim

cover of random fill is placed to protect the contaminated

material from runoff on the S(H):l(V) slopes. No more than 5

vertical feet of contaminated material is exposed along the

Tailings Piles crests at any one time.

(3) The interim cover consists of inorganic soils with a maximum

particle size of 12 inches and is placed in a 1-foot compacted

lift and compacted by at least two passes of a D-7 Dozer or

equivalent; if the interim cover is to be considered part of

the final cover, the soil shall be compacted in accordance with

the Random Fill Requirements to at least 95 percent of standard

Proctor maximum density, and Quality Control testing shall be

conducted on the same scheduled frequency as for the random

fill. The thickness of the random fill to be subsequently

placed shall be reduced by a thickness equal to the thickness

of the interim fill compacted in accordance with Random Fill

requirements outlined above.

(4) The clayey soil layer is placed over the interim cover or con­

taminated material on the 5(H):l(V) slopes and over the contam­

shall have at least 40 percent material passing the No. 200

sieve, a maximum size of 12 inches, a liquid limit of at least

20 percent, and a plasticity index of at least 10 percent and

shall be removed from an approved Borrow Source .

(5) The clayey soil is placed in horizontal compacted lifts not to

exceed 1-foot in thickness, and compacted to at least 95 per­

cent of standard Proctor maximum density.

(6) The clayey cover is a minimum of 0.9 meter thick or as other­

wise specified.

(7) Random Fill is inorganic soils having a maximum size of 12

inches and is remoyed from an approved Borrow source and placed

in a cover layer over the clayey soil.

(8) The Random Fill is placed in horizontal compacted lifts not

exceeding 1-foot in thickness and compacted to at least 95

3 to 6 inches, and a 0 15 size of 3/4 inch to No. 8 sieve size

(Type A) is placed over the Random Fill.

(11) The Riprap is placed in a single lift in such a manner as to

minimize segregation, and receives at least two passes of a D-7

Dozer or equivalent in order to key the rock for stability.

(12} Accumulation of riprap sizes less than the minimum 0 50 size is

removed and blended with subsequently delivered loads to meet

the desired gradation.

(13) The riprap layer is at least 0.3 meter thick.

The frequency of Quality Control Tests and Observations shall

(5) For fill materials where field density test of compacted fill

materials are correlated with laboratory maximum density as

determined by the standard Proctor compaction test (ASTM

0-698), frequency of field density testing for compacted fill

material shall not be less than one test for each 500 cubic

yards of compacted fill material.

(6) Frequency of laboratory standard Proctor compaction tests shall

be such that maximum densities are determined for the range of

materials being placed in the fill; however, frequency of com­

paction tests shall not be less than one test for each 5,000

cubic yards of compacted fill material.

(7) Each load of riprap shall be visually examined by the loader

operator before leaving the Burbank Quarry. The examination

shall be performed by comparing the rock size distribution in

each truckload against a calibrated reference stockpiled in

accordance with Quality Control Procedure QC-a.

(8) Periodic site inspections shall be made for the riprap place­

ment on the random fill. Areas of riprap which show an accumu­

lation of riprap sizes less than the minimum 050 size shall be

removed and blended with subsequent truckloads until the mate­

rials visually conform with the specified gradation.

2.7 surface Runoff Control Facilities

2.7.1 Channels and Ditches

Drainage control shall be provided during placement of contami­

nated materials on top of the Tailings Piles and construction of the

reclamation cover over the tailings and contaminated materials. The

QC Officer shall verify the following:

(1) The rockfill buttress and existing Tailings Pile cover is main­

tained and regraded as necessary.

(2) The interim cover over contaminated materials is placed as

described in Part II section 2.6.

(3) Drainage on top of the contaminated materials on top of the

Tailings Piles is maintained by providing a minimum 2 percent

slope towards the sumps at the back of the Tailings Piles.

(4) Following precipitation events, the specified freeboard is

established by continuously pumping utilizing the existing pump

system following the storm event. Freeboard shall be measured

from the water elevation to the crest of the Tailings Piles.

PART III MONITORING

1.0 SCOPE OF MONITORING

The monitoring program for the Club Mesa Tailings Piles is

designed to provide information on the phreatic levels in the pile,

to ascertain settlement rates and amounts, provide data on stability

and erosion of the protective cover, and to determine the rate and

impact of cliff retreat.

Monitoring of the Tailings Piles shall include:

(1) surface liquids dewatering, quantities and quality

(2) Piezometer levels .

(3) Toe drain effluent rates, quantities and quality.

(4) Siltation in runoff collection channels.

(5) Monitoring of the rockfill buttress, side slope cover, top

cover and drainage diversion channels and ditches.

decline of seepage over time. Monitoring of the following instru-

The piezometers will monitor the change in fluid levels in the

piles at the crest of the rockfill buttress, on the embankment face,

and on the retirement crest.

Data collected from the piezometers will be used to aid in

estimating Tailings Pile settlement (consolidation) and in determin­

ing when the toe drain system can be abandoned. Additionally, the

monitoring of the piezometers should be used to determine if the

Remedial Plan is operating as proposed. When all necessary informa­

tion has been obtained, the piezometers will be properly plugged

with a bentonite slurry.

expectations and performance predictions. The periodic evaluation

shall be comprehensive and shall include discussion of the interre­

lationship between the monitoring data and the predicted performance

of the Tailings Piles with respect to short and long term pile sta­

bility, cover integrity, load/settlement, dewatering and seepage.

Based on a comprehensive assessment, "action" and "concern" levels

shall be specified and discussed in each evaluation. "Concern"

level shall mean that monitoring data is reflecting Tailings Pile

performance which is grossly inconsistent with expectations and may

lead to environmental damage. "Action" level means action must be

taken immediately to mitigate the problem reflected by the monitor­

ing·data. contingencies shall be included in this evaluation where

appropriate.

1.4.2 Synthetic Liner Material

The QC Officer shall verify the following:

(1) The synthetic liners are manufactured of new, first quality products designed and manufactured specifically for the purpose of liquid containment. The QC Officer shall visually docwnent that the materials are new.

(2) The liner materials are free of holes, blisters, undispersed raw materials or contamination by foreign matter. Any such defect shall be repaired using the extrudate welding technique in accordance with the manufacturer's recommendations.

(3) Certification from the Liner Manufacturer documenting that the liner meets all physical property specifications for the intended application has been received and approved.

Vacuum Box Testing - All field seams are inspected for unbonded areas by applying a vacuum to a soaped section of seam. This nondestructive test is performed in accordance with Quality Control Procedure QC-19. Repairs - All defective field seams as detected by both destructive and nondestructive tests are marked and repaired in accordance with manufacturer's recommendations.

Placement of Contaminated Materials on the Repository

The QC Officer shall verify the following:

Only materials designated for placement on the repository are placed on the repository.

(2) Size of scrap placed on the repository has a maximum dimension of 10 feet and a maximum volume of 30 cubic feet.

(3) Scrap having a hollow volume of greater than 5 cubic feet is reduced in volume by at least 70 percent prior to placement on the repository.

(4) Scrap is spread across the repository to avoid nesting and to reduce the volume of voids. No scrap is placed in the first two (2) foot lift or layer of contaminated material.

(5) Tailings, neutralized sludge, soils and/or any other designated waste forms are placed over and into the scrap in sufficient amount to fill the voids between the large pieces and the volume within the hollow pieces, recognizing that some voids will remain as provided for in the Specifications.

(6) The first layer of tailings, neutralized sludge, soil and/or any other designated waste forms does not exceed 2 feet in thickness. Subsequent layers do not exceed 2 feet in thickness except for isolated scrap protrusions.

The QC Officer shall verify the following:

(l) The reclamation cover is constructed in accordance with the Specifications including Drawings P-404-8, P-404-9 and P-404-10.

The clay soiVradon barrier is placed over the contaminated material on the 5(H):l(V) slopes and over the contaminated materials on top of the repository. The clay soil material shall meet the requirements of Specification P-404-1. Part VII. Section 5.0. Article 5.1 for radon barrier/clay fill be in accordance with the specified and shall be removed from an approved Borrow Source.

The clay soil/radon barrier is placed in horizontal compacted lifts not to exceed 6 inches in thickness and compacted to at least 95 percent of Standard Proctor Maximum Density at a moisture content between optimum and 4 percent over optimum.

The clay soiV radon barrier cover is a minimum of 18 inches thick or as otherwise specified.

Following a seasonal shutdown or period of prolonged exposure to inclement weather or freezing temperatures. the last lift of clay material previously placed and accepted is visually examined and tested in accordance with Specification _ S-404-1, Part VII. Section 5.0, Article 5.1.3

Random filVfrost barrier material is comprised of inorganic soils having a Unified Soils Classification System classification of CL and/or CH, maximum size of .2. inches and is removed from an approved Borrow Source and placed in a cover layer over the clay soil radon barrier.

APPENDIX A (7) The random fill/frost barrier material is placed in horizontal compacted lifts not

exceeding 1 foot in thickness and compacted to at least 95 percent of the Standard Proctor Maximum Density at a moisture content above minus 2 percent of optimum.

(8) The random fill frost barrier is at least 4.5 feet thick or as otherwise specified.

(9) Riprap excavated from the Burbank Quarry or other approved Borrow Source or stockpile having a gradation as specified on the revised project Drawings is placed over the random fill frost barrier.

(10) The riprap is placed in a single lift in such a manner so as to minimize segregation and receives at least two passes of a D-7 Dozer or equivalent.

{11) Accumulation ofriprap sizes less than the minimum DS0 size is removed and blended with subsequently delivered loads to meet the desired gradation.

(12) The riprap layer is at least I foot thick.

(13) The frequency of Quality Control Tests shall be in accordance with the Specifications for reclamation cover construction.

4.0 Surface Runoff Control Facilities

The QC Officer shall verify the following:

(1) The exposed rockfill buttresses and Tailings Pile surfaces are regraded during repository construction as necessary to prevent gullies from forming.

(2) Drainage on top of the contaminated material on the repository is maintained by providing a minimum 1 percent slope towards the back of the repository.

(3) Following precipitation events, collected runoff is continuously pumped from the top surface of the repository so that the water is drawn down as soon as is practicable. Under no circumstance will collected water be allowed to pond on the top surface of the repository for more than 10 days after stonn events.

(4) Pennanent Groin Channels TP2 and TP3 and diversion ditches B-1 and B-2 are constructed at the locations and to the lines and grades shown on Drawings P-404-8 and P-404-10.

(5) Permanent Channels and ditches are lined with the specified Type of riprap where excavated or constructed in soils in accordance with the Specifications.

6.0 Monitoring Devices

Monitoring devices shall be installed in and/or modified adjacent to the repository to monitor the performance of the repository or existing Tailings Piles as appropriate during and after construction. The QC Officer shall verify the following:

(I) Existing piezometers presently located on Tailings Piles 2 and 3 are extended as necessary and maintained utilizing similar materials as shown on Drawing P-404-7.

(2) New piezometers are installed as shown on Drawing P-404-7 at locations to be determined in the field. The location of new piezometers are shown on the "As­Built" Drawings

(3) Surface movement monuments are installed as shown on Drawing P-404-7 at locations to be determined in the field during repository construction. The location of movement monuments are shown on the "As- Built" Drawings.

The QC Officer shall ensure that monitoring devices are installed in accordance with the Specifications through periodic inspection of the construction.
