Q: What is the Physicians for a Healthy California CalVaxGrant program?

A: COVID-19 Provider practices may be eligible to receive up to $55,000 to support vaccination efforts through the new CalVaxGrant program, which is administered through Physicians for a Healthy California, in collaboration with the California Department of Public Health and the California Medical Association. Medical practices and independent pharmacies with 200 or fewer physicians are eligible to participate. The CalVaxGrant application cycle runs July 12 through August 13, 2021. Q: What are the guidelines for monitoring storage unit temperatures?

A: COVID-19 vaccine storage temperatures must be monitored using a digital data logger and recorded twice daily to help prevent the loss of vaccines and the potential need for revaccination of patients. Any out-of-range temperatures are temperature excursions and may need to be identified as waste. If a temperature excursion occurs, first complete the Report Temperature Excursion Worksheet to gather the information the vaccine manufacturer will need to determine whether doses may be administered. Then contact the manufacturer and report the excursion and stability determination through myCAvax. Pediatric Providers

1.1 Q: Where can I find guidance on K-12 schools returning to in-person instruction safely?

A: The CDC and the California Department of Public Health released guidance to enable all schools to safely provide full in-person instruction to all students. In general, K-12 students and staff are required to wear masks indoors. Masks are optional outdoors but are recommended in crowded outdoor settings. 1.3 Q: Do Providers need consent to administer the COVID-19 vaccine to non-emancipated minors?

A: In most cases, yes. Non-emancipated minors need the consent of the parent, legal guardian, or other adult having legal custody of the minor to receive the COVID-19 vaccine. COVID-19 vaccine Providers are responsible for verifying that informed consent has been obtained for non-emancipated minors, either in person or in writing. Emancipated minors do not need parental consent to receive the COVID-19 vaccine.

1.4 Q: Does a provider who currently has a parent or guardian’s written and signed authorization for general medical care of a non-emancipated minor on file require additional consent to provide the COVID-19 vaccine to that minor?

A: No. If a Provider already has a parent or guardian’s written and signed authorization for general medical care for a non-emancipated minor on file, a separate consent for the COVID-19 vaccine is not required. However, the Provider may request it at their discretion.

1.5 Q: Can Providers accept written, phone, or video consent from a parent or legal guardian if they do not accompany non-emancipated minors receiving the COVID-19 vaccine?

A: Yes. Allowable consent includes:

• Parent or legal guardian accompanies the minor in person. • If the parent or legal guardian cannot accompany the minor, signed written consent is

allowable. The written consent must verify the parent or legal guardian has been provided the Pfizer EUA Fact Sheet.

• Phone or video consent is allowable if the parent or legal guardian confirms that they have been provided the Pfizer EUA Fact Sheet or the Fact Sheet is read to them.

1.8 Q: Are new pediatric Providers required to use My Turn?

A: No. My Turn is an optional platform for clinic management. Pediatric Providers that opt out of using My Turn need to enroll in an immunization information system and can do so through CAIR Enrollment if they have not yet enrolled. 1.9 Q: Can pediatric Providers also vaccinate family members of pediatric patients?

A: Yes. Although not required, pediatric Providers are encouraged to vaccinate family members of pediatric patients. Pediatric Providers must report all vaccine administration data to the immunization registry (IIS) through My Turn or the California Immunization Registry (CAIR). Vaccine Program Management

2.1 Q: Are healthcare workers required to receive the COVID-19 vaccination?

A: The Vaccine Incentive Program is a $116.5 million initiative implemented by the State to reinforce the value of COVID-19 vaccination for all Californians. The Vaccine Incentive Program has offered eligible Californians who received the COVID-19 vaccine a chance to receive cash prizes, dream vacations, virtual prepaid gift cards, and free food and merchandise deals. A: My Turn Volunteer is a digital platform designed to register Californians as volunteers in support of statewide COVID-19 vaccination efforts. Through My Turn Volunteer, Clinic Vaccination Directors can recruit, vet, match, and request both medical and general support volunteers to support their event needs. A: The myCAvax vaccine management system simplifies and streamlines vaccine management processes. The myCAvax system offers enhanced reporting, ordering, and inventory services that improves the online experience with a fully centralized vaccine management system. COVID-19 vaccine Providers can login into myCAvax at the myCAvax website. 2.21 Q: Can COVID-19 vaccine Providers activate their locations on VaccineFinder’s public-facing map?

A: Yes. COVID-19 vaccine Provider locations may now be activated on VaccineFinder’s public-facing map so patients can locate COVID-19 Vaccines. Provider locations will also appear in Google and Facebook searches and will be translated into Spanish Translation Provided by Univision. Providers can activate their profile and provide updates by referring to our Manual log-in or File Upload quick-start guides. Updates submitted before 7:00AM will display the same day. Provider Enrollment

3.1 Q: Who is eligible to become a COVID-19 Provider in the California COVID-19 Vaccination Program?

A: All current and prospective COVID-19 vaccine Providers must hold the appropriate credentials and licensing in the jurisdiction where vaccination will take place, meet federal and state requirements, and have the capacity to properly maintain and administer the COVID-19 vaccine. The California Department of Public Health is working with Local Health Jurisdictions and the California Medical Association to ensure that all eligible Providers can receive and administer COVID-19 vaccines. 3.4 Q: Are school districts, private schools, and charter schools eligible to enroll in the California COVID-19 Vaccination Program?

A: Yes. School districts, private schools, and charter schools are eligible to enroll in the California COVID-19 Vaccination Program through myCAvax. Schools that cannot meet the provider requirements nor wish to enroll in the COVID-19 vaccine program, but would like to become a vaccination site, can partner with an approved provider or their local health department to administer the COVID-19 vaccine. 3.5 Q: Where can I access support tools for myCAvax enrollment and other functions?

A: Once enrolled, COVID-19 vaccine Providers may access a variety of resources and training materials such as job aids, videos, weekly Office Hours, and recorded demos to support system users as they navigate through the myCAvax platform. Information and resources may be found at myCAvax.

3.6 Q: What is VaccineFinder and where can I get enrollment assistance?

A: VaccineFinder is a system operated by the U.S. Centers for Disease Control and Prevention (CDC) to track vaccine inventory. Onboarding to VaccineFinder is currently a manual process. Please refer to Provider Enrollment and Reporting Inventory at Provider Enrollment in VaccineFinder for assistance with initial onboarding, as well as daily reporting of doses on hand. This job aid includes links to a training video that addresses account setup and reporting. If your location has received or is expecting to receive COVID-19 vaccine in the next two weeks but has not received the initial email to complete onboarding, please email [email protected] for assistance with onboarding. For technical assistance, once onboarding is complete, email [email protected] or call 855.886.4317 for assistance with account login issues, password resets, file upload errors, and more. To change a Provider’s contact information or location in VaccineFinder, contact the COVID Call Center at [email protected] to update the Section A or Section B email information in the CDC Provider Agreement. Once the data is forwarded to the CDC, VaccineFinder will be updated with the new email.

3.7 Q: Do we need to enroll in myCAvax if we are receiving doses from the Federal government?

A: No. Only COVID-19 vaccine Providers receiving doses from the State, or their Local Health Department are required to enroll in myCAvax. If you are an approved COVID-19 vaccine Provider only receiving doses from the Federal government, contact the CDC for enrollment questions and instructions. Ordering

4.1 Q: Which COVID-19 vaccines are available for ordering in myCAvax?

A: The Moderna 140-dose shipments and Pfizer 450-dose shipments are available for Standard Orders in myCAvax. The Janssen from Johnson & Johnson, smaller orders of Moderna, and the Pfizer COVID-19 vaccine can be requested through the myCAvax Vaccine Marketplace and Small Order Requests in myCAvax, dependent upon availability.

4.2 Q: Can COVID-19 Providers who signed the TPA agreement continue to place vaccine orders and administer vaccines?

A: Yes. COVID-19 Providers who signed the TPA agreement can continue to place vaccine orders in myCAvax and administer vaccines. Vaccine orders will be approved by Local Health Jurisdictions and the California Department of Public Health. For more information, please visit Ordering Vaccines.

4.3 Q: When should COVID-19 vaccine Providers submit weekly vaccine order requests in myCAvax?

A: COVID-19 vaccine Providers should submit weekly vaccine order requests by Monday by 5pm. Providers can submit order requests after Monday at 5pm, but vaccine shipments may be delivered later. For more information, please visit Ordering Vaccines.

4.4 Q: What is the approval process for COVID-19 vaccine order requests?

A: After vaccines orders are placed on Monday, Local Health Jurisdictions review and approve COVID-19 vaccine order requests, and the California Department of Public Health finalizes approvals. 4.5 Q: Where can COVID-19 Providers go to receive vaccine quickly?

4.6 Q: When will COVID-19 vaccine Providers be able to order smaller quantities of vaccine in myCAvax?

A: Eligible COVID-19 vaccine Providers, in participating Local Health Jurisdictions, can order smaller quantities of vaccine in myCAvax. The “Small Order” option will appear on the myCAvax Order Request Form for Providers who are designated to receive smaller shipments by their Local Health Jurisdictions. Please note: Providers should only submit small order requests when a standard order cannot be accommodated, or when vaccine doses cannot be found on the Vaccine Marketplace. 4.7 Q: Are Pfizer 450-dose shipments available to all California COVID-19 Vaccination Program Providers?

A: Yes. The Pfizer 450-dose shipments are available to all California COVID-19 Vaccination Program Providers. The 450-dose shipment includes 3 trays containing 25 vials each.
